About this file

This is a Contractor Responsibility Verification form based on FAR 9.103 and 9.104 requirements. The document serves as a template for prospective contractors to demonstrate their responsibility and qualifications to perform federal contracts before award.

The form requires contractors to provide evidence and certifications across six key responsibility factors: (a) Financial Resources—documentation such as profit/loss statements, balance sheets, lines of credit, or statements from financial institutions confirming sufficient resources; (b) Meeting Delivery Requirements—confirmation of ability to comply with required delivery or performance schedules considering all existing commitments; (c) Performance Record—submission of minimum two past performance examples within the last three years, capability statements for new entrants, or exemption documentation for contractors with prior government CPARS or SPRS ratings; (d) Business Integrity and Ethics—certification that SAM.gov Responsibility/Qualification section information regarding exclusions, proceedings, and integrity matters is current and accurate; (e) Organizational Structure—confirmation of adequate organizational structure including experience, accounting and operational controls, and technical skills; and (f) Production Capabilities—documentation of technical equipment and facilities or ability to obtain them. A responsible company official must certify under penalty that all information provided in factors (a) through (f) is correct and accurate. This form does not specify particular products, services, response dates, or award dates, functioning instead as a standardized responsibility assessment tool applicable across federal contract opportunities.
