About this file
This is a Past Performance Questionnaire (PPQ) issued by the 60th Contracting Squadron at Travis Air Force Base, California for evaluation of contractors bidding on Local Telecommunication Services.
The PPQ requests references from recent customers of bidding contractors to provide performance ratings on similar work. Completed questionnaires must be submitted as PDF files to Mr. Don Salgado, Contract Specialist, at don_phillip.salgado@us.af.mil by 4 May 2026 at 12:00 PM PDT. The questionnaire collects offeror information (contractor name, address, and point of contact details), respondent information (the entity providing the reference), and contract information including contract name/number, description, performance period, annual and total contract values, and whether the contractor served as prime or subcontractor. Respondents rate contractor performance on a 1-5 scale across six evaluation criteria: quality of service, on-time delivery of service, management of service, communication, contract terms and condition adherence, and overall performance. Additional questions address whether the contractor received Corrective Actions Reports or similar deficiency reports and whether the respondent would award future contracts to the contractor. Ratings of 1 (Unsatisfactory), 2 (Marginal), or 5 (Exceptional) require narrative explanations. The questionnaire concludes with a remarks section and signature block for authorized respondent authentication.
