STATEMENT OF WORK
REPAIR HICKAM AVE PHASE III
TRAVIS AFB CALIFORNIA
Base Project Number: XDAT 15-1135
DATE: 22 August 2018
1.0 INTRODUCTION
The Contractor shall be responsible for all professional services, permits, equipment, labor, tools, materials, and ancillary items necessary to complete the tasks to repair Hickam Ave Phase III.
2.0 SCOPE OF WORK
The performed tasks shall be based on the project description, other data furnished in this SOW, and information covered during Pre-CON meeting. THE SCOPE OF THIS PROJECT INCLUDES FULL DEPTH REPAIR OF THE ROAD AND CURB AND GUTTER REPAIR OF HICKAM AVE PHASE III. This shall include, but is not limited to:
a. DRAWING 1: Site map of the area to be repaired
b. DRAWING 2: Pavement Section Detail
c. DRAWING 3: Topographical survey data
d. DRAWING 4: Utility Data
e. DRAWING 5: Sidewalk and Curb Detail
2.1 Scope & Applicable Documents: The Contractor shall adhere to the following guidance during design and construction phase of this project: Unified Facilities Criteria (UFC) 4-22-03, International Plumbing Code (IPC), Uniform Building Code (UBC), those sections of the Southern Standard Building Codes not incorporated in the UBC/International Building Code (IBC) for our respective area, International Concrete Repair Institute (ICRI) Guidelines, Air Force Standards, Department of Defense (DOD) Directives, California State Statutes, and the Travis AFB Facility Excellence Plan (Base Regulations), Occupational Safety and Health Administration (OSHA) Requirements, Department of Environmental Protection (DEP) Regulations, Environmental Protection Agency (EPA) Regulations, and ABA Accessibility Compliance Requirements. If there is a conflict between these codes, regulations and specifications the most stringent requirements shall apply at all times. Documents listed by and/or provided by the Government are for reference only and do not relieve the Contractor from adherence with applicable codes not specifically addressed.
2.2 Background: Hickam Avenue is a major access to shops and industrial buildings on Travis AFB. Lack of maintenance for many years lead to the present deteriorated surface condition. The road surface is predominantly medium level alligator cracks, with some sever spots on the verge of becoming potholes.
Utility cuts and large patches cover a considerable area. All the pavement markings are badly faded. The road surface, in general is considered highly degraded.
2.3 TASK 1: As part of the proposed effort, the Contractor shall implement means and methods that will limit disturbance to the base activities to the greatest extent practicable throughout the project performance.
The government provided documents and existing field conditions shall be reviewed by the contractor to identify any potential discrepancies and develop necessary requests for information (RFI) prior to construction mobilization. Repair efforts shall be in strict accordance with the contract documents.
Substitutions and/or deviations will not be accepted unless approved by the Contracting Officer (CO) in writing. Clean up all construction debris, restore disturbed areas, dispose of all waste off site at the conclusion of work. The task descriptions below are summaries of work to be completed in order to result in a completed project.
a. Demolish existing 4” of asphalt, remove 12” existing base course and over excavate 1” of soil for full depth road repair shown in Drawing 1.
b. Compact existing soil and install geotextile layer.

c. Place 12” of Class 2 base course, and place 3” binder course and 2” wearing course asphalt lifts (CALTRANS HMA mix) per pavement section detail shown in Drawing 2.
d. Ktr shall include traffic plan, layout and application of pavement markings.
e. Demolish and replace existing curb and gutter in select areas of known distress as shown in
Drawing 1.
f. Demolish and replace existing sidewalk in select areas of known hazard as shown in Drawing 1.
g. Place valley gutters on both sides of the road to support drainage, as shown in drawing 1.
h. Pave to drain (min 1% slope, max 3% slope) matching pavement grade to existing utility covers (no raising or lowering). Note: several utility covers are above natural grade and will have to be feathered into overall drainage grade of asphalt. Covers will not be lowered. Field verification of these covers to be accomplished at site visit. Contractor to conduct boundary, topography survey before demolition to establish grading plan.
i. Submit scaled striping plan for government approval prior to layout of markings.
j. Provide temporary raised reflective pavement markings prior to opening paved road to traffic.
k. Provide thermoplastic pavement markings, and permanent raised markings per ktr approved striping plan, after paving of all phases is complete.
l. Dispose of asphalt millings at a location off of Travis AFB and provide Construction & Debris report within 30 days of disposal for 60 CES/CEI tracking of recycled materials.
m. Ktr shall provide on-site vacuum sweeper during all milling operations and to prepare surface for asphalt lift. Area must be swept and clean of debris before government acceptance.
n. Ktr to provide detour/signage plan with traffic control as pre-construction submittal for governmental approval at least 4 weeks prior to start of scheduled milling.
o. Ktr flaggers and appropriate phasing will be utilized to minimize these closures. If a closure cannot be avoided, the pre-construction submittal will provide proposed detour route showing location and type of all signage and keep the closure time as short as possible.
p. Ktr shall submit a phasing plan with proposal.
NOTE: The elevation survey (Attachment 3) is to be used for determined cut/fill quantities for proposal only!
This is not to be used for ktr to establish actual grade plan for paving.
The Period of Performance shall be no longer than 90 calendar days.
2.3.1 Deviations will not be accepted or considered by the Government without formal approval in writing from the CO. Criteria Drawings detailing the contracted work are included in SOW Appendix A.
a. Site / Utility Verification: Verify actual existing site conditions and identify potential conflicts prior to starting work.
b. Protect Non-Work Areas & Equipment: Install protective coverings and barriers to protect nonwork areas and equipment remaining in the work area from damage, construction debris and dust during performance of the work.
c. Layout: The contractor shall layout the work defined in the contract documents.
d. Clean up: Remove protective coverings and clean up all areas affected by construction activities to restore original conditions
e. Disposal: Dispose of all construction waste off site at the conclusion of work.
All work shall be in compliance with all Public Laws (P.L.), Executive Orders (E.O.), Military Standards (MIL-STD), Air Force Instruction (AFI), Code of Federal Regulations (CFR), Department of Defense Instructions (DODI) and Department of Defense Directives (DODD) or other higher authority documents as applicable, as listed in MIL-STD-3007F.
2.3.2 Permits: (Where Applicable) The Contractor shall develop, coordinate, and be responsible for labor, professional services and other costs associated with acquiring any and all required state, local, and other applicable permits; base access (including off-base easements and leases), agreements, licenses, and certificates required to perform and complete the project. The Contractor shall maintain a library of these documents on site as well as at the corporate facility handling each project. The
Contractor shall comply with applicable permit conditions, environmental protection policies and appended guidelines. Permits for this project include:
a. SWPPP Plan Permit
b. Work Clearance Request/Digging Permit (Obtain a blank 60 AMW FM 66 (often called a digging permit) from 60 CES Customer Service, (707) 424-7525. Contractors shall request a digging permit at least ten (10) working days in advance. In-place utility locations shall be verified by the Contractor with on-site pit verification as needed prior to the start of construction activities.)
2.3.3 Construction: Upon approval by the CO for all preconstruction requirements, submittals, and applicable permits; the Contractor may commence on-site construction activities. The Contractor shall conform to all specifically reference and other applicable requirements during performance of the work as described in the Contract Documents:
1. Mobilization: Mobilize equipment, labor, tools, materials, and ancillary items necessary to complete the work defined in the Contract Documents. The Contractor shall be capable of mobilizing to begin work within 14 days of award.
2. Demolition / Construction: Perform awarded tasks as defined in section 2.3 Description of Work.
a. Protect work area with barricades, warning tape, signs and/or other appropriate methods for the work being completed to ensure the safety of pedestrians and Contractor personnel;
b. If utility relocation is required, the relocation shall be coordinated with and approved by the utility owner/operator and CO in advance of relocation efforts;
c. Care should be taken to avoid damaging other installed utilities. In the event that other utilities are damaged during performance of the contract, the Contractor shall repair the damage at no additional cost to the Government.
d. Remove and dispose of all demolition and general construction debris from the site (off base);
e. Perform ancillary efforts required to complete the work in order to provide clean, complete and functional facility meeting the standard of quality for each trade utilized;
f. Limit disturbance to the natural vegetation, wildlife and base activities to the greatest extent practicable.
2.3.5 Government Furnished Equipment: None.
3.0 GENERAL REQUIREMENTS
Applicable provisions of the Contract Clauses in the contract will govern work under each section of the specifications.
3.1 Pre-CON Meeting: A Pre-CON meeting shall be accomplished with the Contractor, Contracting Officer, Civil Engineering Squadron (CES), and user. The meeting objective is to review project details, determine construction start date, security procedures for site, dumpster locations, and any applicable requirements during construction. The Contractor shall visit the job site to verify location, conditions, and dimensions prior to starting work.
3.2 Facility Occupancy: N/A
3.3 Bulletin Board: Immediately upon mobilization to site, the Contractor shall provide a bulletin board not less than 36 by 48 inches (915 by 1220 mm) in size for displaying the Equal Employment Opportunity poster, a copy of the wage decision contained in the contract, Wage Rate Information poster, and other information approved by the Contracting Officer. The bulletin board shall be located at the project site in a conspicuous place easily accessible to all employees, as approved by the Contracting Officer. Legible copies of the aforementioned data shall be displayed until work is completed. Upon completion of work the bulletin board shall be removed by and remain the property of the Contractor.
3.4 Identification of Employees: The Contractor shall be responsible for obtaining security clearances and badges for each employee, and for requiring each employee engaged on the work to display identification as approved and directed by the CO and Security Forces. The contractor shall submit personnel background check applications for badging within three (3) days after award. During performance of the contract, prescribed identification shall immediately be delivered to the CO for cancellation upon release of any employee. When required, the Contractor shall obtain and provide fingerprints of persons employed on the project. All Contractor and sub-contractor personnel shall wear identifying markings clearly defining the company for whom the employee works.
3.5 Job Site Tools and Equipment: All hand tools and equipment shall be maintained in good working conditions and properly stored when not in use. Construction attire shall be in accordance with applicable OSHA regulations.
3.6 Notification Requirements: The Contractor shall notify the Government a minimum of five (5) days in advance of mobilization. The Contractor is required to notify the CO and Government Project Manager of critical issues that may affect the contract performance and/or human health and the environment. For critical issues pertaining to human health and the environment, the Contractor shall immediately notify the CO and stop work. Work shall only resume at the direction of the CO following resolution of the issue.
3.7 Environmental Protection: Activities shall be planned and implemented in a manner that protects existing site utilities, structures, surface features, service operations, critical wildlife, and the general site environment. This includes:
1. The protection of trees, shrubs and other vegetation not in the affected zone from dust damage, soil compaction, and physical contact with machines and equipment. If appropriate, the Contractor shall conserve uncontaminated topsoil by removal, storage, or redistribution. All reasonable measures shall be taken to minimize and suppress fugitive emissions of dust, vapors, and other site materials during site work. All fill materials shall be non-contaminated. The Contractor shall conduct operations and activities with the intent of reducing the amount of pollution generated. Specific areas to be focused on are generation of solid waste, use of hazardous materials, use of ozone depleting chemicals, generation of hazardous waste, and use of energy and water. The Contractor shall plan, construct, operate, maintain, optimize, and commission systems necessary to control storm water run-on and runoff. At a minimum, the Contractor shall employ best management practices consistent with the California State Environmental Policies and guidance in Division 01 General Section 01 56 00 and Environmental General Requirements.
2. Before work begins, the contractor will clearly delineate the project disturbance boundaries which shall be entirely on paved or gravel surfaces. The Contractor shall ensure all equipment, materials and vehicle traffic stay on paved surfaces and are confined within the designated project disturbance boundary (including access roads and laydown).
3. Prior to the start of construction activities, a Service approved Biologist, provided by the government, shall provide education and training sessions for all individuals that will be involved with site preparation or construction. The training will focus on habitat sensitivity and identification of CTS. The training shall include species description and behavior, general measures to be taken to protect these species as they relate to the project, the penalties for non-compliance, and the boundaries of the project area. A fact sheet or other supporting materials containing this information will be prepared and distributed. Upon completion of training, employees will sign a form stating that they attended the training and understand all the conservation and protection measures.
4. Construction activities will occur between sunrise and sunset.
5. To protect birds under the Migratory Bird Treaty Act, the Government will provide for the project
(if occurring between the months of February-August) a biologist to perform a pre-construction site survey at least 14 calendar days before construction to determine whether any protected species or nesting birds are present that may be impacted by project activities. If nesting birds are present on or near the site, construction may be temporarily postponed until the nesting season is over.
6. All trash (food related items such as wrappers, bottles, cans, food scraps, etc.) will be placed in closed containers and removed from the project site on a daily basis. All fencing, flagging, debris, trash, and materials will be removed from work areas following completion of construction and habitat restoration activities.
7. The Contractor will utilize dust control measures whenever necessary. These measures include, but are not limited to, use of a water truck to spray disturbed areas during construction activities and covering of soil piles with plastic whenever they are not in use. No water will be allowed to run-off the site. If connecting to a hydrant, a backflow preventer is required.
8. Operators will be familiar with and exercise spill prevention and emergency spill response measures as required. Emergency response plans will be on site.
9. If California Tiger Salamanders (CTS) are observed on or near the construction site, stop all work and contact the Contracting Officer and 60 CES/CEIE Natural Resources Program Manager at 707-424-8354 or 707-464-5126 immediately. Reference General Section 01 56 00 paragraph 3.6.10.2.1.
10. The contractor will confine all equipment to designated work zones (including access roads and laydown) within the area to be disturbed.
11. Construction activities can be done in the wet season. If work is to be done in the wet season; no work can be done in the rain. Period of Performance shall be no longer than 120 calendar days.
3.8 Hazardous Waste Management: Contractor shall notify 60 CES/CEAN if any hazardous waste is encountered or generated. The Contractor shall stop all work in the event 60 CES/CEAN identifies noncompliance with federal and state regulations and shall correct any discrepancies immediately within two (2) hours of notification by 60 CES/CEAN. All hazardous waste shall be disposed of by the contractor in accordance with Division 01 General Requirements Section 01000 and General Section 01 56 00 Environmental General Requirements.
3.9 On-Site Field Oversight: The Contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure responsible oversight throughout accomplishment of the construction period. It is the responsibility of the Contractor to establish and maintain open communications between salient authorities associated with accomplishment of this contract.
3.10 Federal Holidays: Travis AFB observes the following Federal Legal Holidays;

3.11 Working Hours: The normal working hours for the base are from 7:30 AM to 4:30 PM, Monday thru Friday excluding Federal Holidays. All work shall be performed during normal duty hours. All other hours to include weekend and holidays shall be requested through the CO for approval. This request shall be submitted no less than three (3) workdays prior to the time requested.
3.12 Availability and Use of Utility Services: The Government will make all reasonably required utilities available to the Contractor from existing outlets and supplies, as specified in the contract.
3.13 Sanitation: The Contractor shall provide and maintain within the construction area minimum field-type sanitary facilities approved by the CO. Government toilet facilities will not be available to Contractor's personnel.
3.14 Clean Up: At the end of each working day the Contractor shall clean up the work site by the removal of all construction debris, waste materials, packaging material and the like. If the before mentioned items could become airborne they shall be placed in refuse containers immediately. Clean up of the work site shall include the construction area(s), construction office area(s), material storage area (s), parking and eating area(s), restroom facilities used by Contractor personnel, and any other area(s) affected by the construction process. Any dirt or mud which is tracked into the building, on to any Base street, parking area, sidewalk, patio, or driveway shall be cleaned away daily. Materials resulting from demolition activities which are salvageable shall be stored within the work area or at a supplemental storage area. Stored material not in trailers, whether new or salvaged, shall be neatly stacked when stored in an approved location.
3.15 Preparation of Progress Schedules and Reports: The reports contemplated by Clause 52.236-15, Schedules for Construction Contracts, shall be accomplished on and in accordance with instructions pertaining to AF Form 3064, Contract Progress Schedule, and AF Form 3065, Contract Progress Report.
Contract Schedules and Reports shall run from Monday through Sunday and be submitted to the CO.
4.0 GENERAL SAFETY REQUIREMENTS
4.1 Workplace: Provide a safe and healthful workplace for employees and the general public while performing the contract work.
4.2 Personal Protective Equipment: Supply each employee with personal protective equipment. This equipment includes but is not limited to protection for the eyes, face, head, ears and extremities. Use the proper protective equipment for the work performed.
4.3 Roads: Obey the posted base speed limits. Wear Seat belts on base in accordance with AFI 91-207.
4.4 Accidents: Report Contractor accidents involving injury to Air Force personnel or damage to Government property to the Base Law Enforcement Desk and the CO.
4.5 Safety Regulations: Familiarize yourself and comply with the general safety requirements in accordance with DOL regulations, OSHA 29 CFR Part 1926, as well as applicable Air Force Office of Safety and Health (AFOSH) standards and Air Force Instructions (AFI).
4.6 OSHA Inspections: The Contractor shall be subject to no-notice inspections under OSHA program by inspectors of the Department of Labor.
4.7 Protection and Maintenance of Traffic: During construction, the Contractor shall provide access and temporary relocated paths as necessary to maintain traffic that may be affected by construction activities.
The Contractor shall maintain and protect traffic on all affected roads/pedestrian paths during the construction period except as otherwise specifically directed by the CO. Measures for the protection and diversion of traffic, including the provision of watchmen and flagmen, erection of barricades, placing of lights around and in front of equipment and the work, and the erection and maintenance of adequate warning, danger, and direction signs, shall be as required by the State and local authorities having jurisdiction. The traveling public shall be protected from damage to person and property. The Contractor's traffic on roads selected for hauling material to and from the site shall interfere as little as possible with public traffic. The Contractor shall investigate the adequacy of existing roads and the allowable load limit on these roads. The Contractor shall be responsible for the repair of any damage to roads caused by construction operations.
4.8 Barricades: The Contractor shall erect and maintain temporary barricades to prevent public access to hazardous areas as required by OSHA, Caltrans, and CAMUTCD and/or like Agencies. Such barricades shall be required whenever safe public access to paved areas such as roads, parking areas, sidewalks or other work areas are prevented by construction activities or as otherwise necessary to ensure the safety of both pedestrian and vehicular traffic. The minimum required number of barricades shall be securely placed in a neat and orderly fashion, clearly visible with adequate illumination to provide sufficient visual warning of the hazard during both day and night. Barricades shall be maintained in an orderly fashion during the duration of their placement.
4.10 Fire Regulations and Preventative Practices: The Contractor shall comply with fire prevention practices as set forth by the National Fire Protection Association (NFPA), other recognized fire prevention agencies, and base regulations.
4.11 Explosive Operated Hand Tools: The Contractor shall comply with OSHA Standard 1910.215(c) when utilizing explosive operated hand tools. Storage of explosive cartridges on the installation shall be in metal containers and limited to one days' supply. The Contractor shall provide adequate controls to prevent loss/theft of cartridges used and stored on the installation.
4.12 Welding: Welding shall not be performed without first obtaining a welding permit issued by the Base Fire Department.
5.0 GENERATED REFUSE
5.1 Refuse: The Contractor shall take positive action to prevent work-generated refuse from entering the sewer system and water bodies. All Contractor generated refuse and waste shall be hauled from the construction site daily to a disposal area located outside the physical boundaries of the installation.
6.0 UTILITY OUTAGES
6.1 Definition: Utility outages shall be defined as a stoppage in electric, gas, potable water, sewer, chilled or hot water, steam, telephone, Energy Monitoring and Control System (EMCS), or any other similar utility whether serving all or a part of a single facility or of several facilities.
6.2 Outage Coordination: All utility outages shall be coordinated with the Using Agency through the CO or the COR. Notice of the necessity for an outage shall be provided by the Contractor to the CO, in writing, not less than seven (7) working days prior to the time of the required outage and written approval for the outage will be issued by the Contracting Officer. All work shall be coordinated and arranged to ensure that the outage will be of minimum duration.
6.3 Restoration: Once a utility outage is arranged and work begun, work shall continue until all utilities are restored to the affected facility or facilities.
6.4 Cancellation: In the event a scheduled utility outage is cancelled by the Government, notification will be given to the Contractor at least 24 hours in advance of the time for the outage to start. Rescheduling of a cancelled outage must be coordinated with written notification as described above.
7.0 EXISTING UTILITIES
7.1 Reference Drawings: Drawing 5 (see attached).
7.2 Travis AFB Electrical Utility System: The High Voltage electrical system on Travis AFB is owned by a private utilities contractor, City Light & Power. Any work affecting this system must first be coordinated with this company. (See contact information in Division 01General Requirements Section 01000).
8.0 DELIVERABLES / SUBMITTALS
8.1 Contract Progress Schedule: The Contractor shall submit a signed AF 3064 (or approved equivalent), Contract Progress Schedule, depicting an overall contract progress schedule for the main elements of work for the period of performance and submitted as described in the Table of Deliverables. The line items, associated project percentages, and dates shall accurately depict the planned work to be performed. The Contractor shall also provide a line graph depicting actual construction progress (solid line) versus scheduled construction progress (dotted line) throughout the period of performance. The progress schedule shall be signed by the either the COR or the government contract inspector recommending approval and approved by the CO. If the Contractor requires reimbursement of a high dollar value item, the item shall be identified on the AF 3065, an appropriate percentage applied, and the item must be received and stored on site.
8.2 Contract Progress Report (AF 3065): The Contractor shall submit AF 3065 monthly as described in the Table of Deliverables. This report shall be signed by the Contractor’s on-site representative in the Remarks section of the AF 3065. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. The Contractor shall obtain either the COR or the Government contract inspector’s acceptance by signature of the actual reported progress prior to submission to the CO. Additionally, the Contractor shall submit an updated AF 3064 as an attachment to each submission of the AF 3065.
8.3 Material Approval (AF Form 3000): The Contractor shall prepare and submit the AF Form 3000 with all submittals to the Government in accordance with the AF Form 66). The form shall document the contents of the submittal and be signed by the contractor. The Contractor shall not start any work before all material approval submittals approved.
Upon receipt of each material submittal the Government has 14 calendar days to review and respond. The Contractor owns burden of managing project time and of soliciting material submittal approval in accordance with project schedule needs.
8.4 Construction Material Testing: The Contractor shall employ a qualified third party material testing company to verify that soil and concrete material requirements are met. Test reports will be submitted to the government when the formal reports are received. Field reports shall be maintained on site for government reference at all times.
The Contractor shall notify Government in writing 24 hours prior to all density tests performed by ktr’s quality control personnel or ktr’s independent testing firm. Government will accept nuclear testing method in lieu of other destructive testing methods. Ktr shall submit all compaction reports or field notes within 24 hours after density tests performed. Official test reports shall be submitted no longer than 30 days after density tests performed.
8.5 Hazardous Material and Hazardous Waste: The Contractor shall dispose of hazardous materials/waste in accordance with Division 01 General Requirements Section 0100 and General Section 01 56 00 Environmental General Requirements.
8.6 Transfer and Acceptance of Military Real Property (If Applicable): The Contractor shall prepare and submit a DD Form 1354, Transfer and Acceptance of Military Real Property document. The draft and final copies shall be submitted in accordance with the Table of Deliverables.
8.7 Pre-Final Inspection: The contractor shall request a Pre-Final Inspection through 60 CONS not less than 14 calendar days prior to the intended date of the Pre-Final. The date of the Pre-Final must allow adequate time to correct all deficiencies and conduct the Final Inspection prior to the construction completion date. A contractor-provided punch list, including the status and projected completion date for all open items, shall accompany this Pre-Final request.
8.8 Final Inspection: The Contractor shall conduct a final inspection with base personnel and publish the findings in a final inspection report for each project task upon substantial completion of work. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final inspection (punch list) report. A final inspection shall not be performed until the pre-final inspection (punch list) report has been resolved.
8.9 Delivery/Warranty: The Contractor shall complete all inspection and commissioning requirements prior to final inspection. Following final inspection, the Contractor shall deliver to the Government the warrant documentation. The warranty shall be issued in accordance with FAR 52.246-21.
9.0 CONTRACTOR’S QUALITY CONTROL
9.1 Quality Control: The Contractor is responsible for quality control and shall establish and maintain an effective quality control system in compliance with FAR Clause 52.246-12, Inspection of Construction. The quality control system shall consist of plans, procedures, and organization necessary to manage all delivery orders to produce end products which comply with the contract requirements. The system shall cover all construction operations, both on site and off site, and shall be keyed to the proposed construction sequence. The Government will hold the project manager responsible for the quality of work on the job and is subject to removal by the CO for non-compliance with quality requirements specified in the contract. The Contractor shall verify full compliance with the contract documents prior to requesting final inspection(s).
9.2 Field Measurements: The Contractor shall be required to make his/her own field investigations to verify existing conditions, dimensions and other information shown on Government furnished reference drawings, solicitation documents and construction documents. Deviations, errors, or omissions in the government supplied reference materials or the contract documents shall be identified and addressed by the contractor prior to the start of construction.
9.3 Siting Location: The contractor is responsible for field reconnaissance, surveys, site investigations and testing required to obtain information necessary for the accomplishment of the project in accordance with the contract documents. In-place utility locations shall be verified by the Contractor prior to the start of construction activities. Verification shall be accomplished by a Work Clearance Request / Digging Permit (reference paragraph 2.3.2.) and shall include on-site verification as needed.
10.0 INSPECTION AND ACCEPTANCE
10.1 Provisions: Government inspections and final acceptance shall be scheduled by the Contractor as the work is completed with the following additional provisions:
a. The Contractor shall give the Government a minimum of three (3) working days’ notice for all requested inspections.
b. The Contractor shall verify that the work to be inspected is in full compliance with the contract documents prior to the requested date of inspection. The presence of an unreasonable number of deficiencies, as determined by the construction inspector, may cause the inspection to be rescheduled.
c. The Contractor will be given a certificate of acceptance by the construction inspector for each successfully completed step of the work. The certificate will be signed by the construction inspector and shall be initialed and dated by the Contractor’s approved representative. One copy of the certificate will be given to the Contractor.
d. Contractor shall complete a CES approved Contractor Daily Report form and submit to construction inspector by 1000 hrs. the next day after work is performed and inspected by the Contractor’s Quality Control (CQC) Representative.
11.0 DETERIORATED CONSTRUCTION AND FINISH
11.1 Authority: The construction inspector will be the governing authority in determining if existing construction and finishes are deteriorated and require repair and/or replacement.
12.0 WATER CONSERVATION
12.1 Water Conservation: Conserve water to the greatest extent possible.
13.0 CONTRACTOR STORAGE/STAGING AREA
13.1 Operations and Storage Approval: Notwithstanding Contract Clause, FAR 52.236-10, Operations and Storage Areas, subject to approval by the CO and in turn, availability and need, a storage/staging area shall be provided at the subject project site.
13.2 Storage Area Clean Up: At work completion, clean the storage area. This area must be free of any materials, trash or debris before receipt of final payment. Existing fencing shall remain Government property.
14.0 CONTRACT ADMINISTRATION / POINTS OF CONTACT
14.1 Contracting Officer Authority: CO, the term used herein, does not include any representative not acting within the scope of his/her authority. Notwithstanding any of the provisions of this contract, the CO shall be the only individual authorized to in any way amend or modify the terms of this contract.
14.2 Field Changes: The Contractor and the Government may agree to perform a no cost field change. Field changes are made when the change appears to be mutually beneficial to all parties and would not require changing the negotiated items. All field changes must be approved by the CO prior to execution.
14.3 Closeout Documents: Five Percent (5%) of the Contract Value will be withheld as a final payment until all project deliverables such as As-Built Drawings, GIS Data, Final 1354, O&M Manuals, and Training Material are submitted to and accepted by the Government.
