1.0. DESCRIPTION OF SERVICES. The Contractor shall provide all management, instructors, counselors, curriculum, facilities/lodging, media, meals, and childcare to conduct
Marriage Weekend Retreats, Family Weekend Retreats, Exceptional Family Member Program
(EFMP) Family Retreats, and Single Parent Family Retreats as further identified in the
Performance Work Statement.
Each event shall be facilitated by a Chaplain of the Hurlburt Field Chaplain Corps and led by the
Contractor. The Chaplain Corps is a religiously distinct component of the military, and provides for the free exercise of religion in a pluralistic environment. Therefore, the contractor shall work in an ecumenical environment sensitive to all participating in the events and abide by all
Department of Defense and Federal laws, rules, and guidelines, including those laws and guidelines pertaining to marriage.
Department of Defense policy states that all legally married military couples shall be treated equally and as such, events are open to all legally married military couples; and the contractor shall be willing to facilitate weekend events for all eligible military personnel and their dependents. The contractor shall ensure that no coercion to conform a participant to a particular religious denomination or faith group shall be manifested directly or indirectly. Proper respect for all religious faith groups shall be maintained.
1.1. MARRIAGE WEEKEND RETREATS. Seven (7) Events shall be scheduled per performance period and shall be within 30 miles of Hurlburt Field, Florida. Marriage Weekend
Retreats should focus on common stresses that military families endure (frequent moves, times of family separation, etc.). They should be designed to enhance communication, build teamwork, and strengthen conflict resolution skillsets. The curriculum may include a reasonable amount of prerequisites and homework for participants.
1.1.1. Description of Lodging. Contractor shall provide a facility that can provide lodging for a maximum of 80 participants (78 attendees and two facilitators) for two (2) nights. Rooms shall be provided in order to accommodate 39 couples, their families, and each facilitator. The number of participants at each event will be contingent upon the dates selected. Each couple shall have their own private room which must include no smaller than a queen sized bed, and depending on the location must include the following amenities: private bathroom, television, refrigerator, microwave, and internet access. For large families, additional rooms shall be required to meet hotel maximum occupancy guidelines. Guest check-in must be available to event participants by
3:00 p.m. on the first day of the event. Guest checkout shall be extended to 12:00 p.m. on the last day of the event. Average lodging rooms per night is as follows: Marriage Weekend Retreat (50), Family Weekend Retreat (50), Single Parent Weekend (30), EFMP (30).
1.1.2. Lodging Amenities. The contractor shall provide on-site recreational activities that must include the following: on-site fitness center, outdoor or indoor swimming pools, beach access within one quarter mile distance of lodging with two (2) beach chairs, and one (1) beach umbrella, and health spa access. Additionally, free parking shall be included for all participants.
1.1.3 Meeting Space. The lodging facilities shall include a meeting space capable of supporting a maximum of 80 participants, instructors, and facilitators for both instruction and activities.
Meeting dates/times are outlined in paragraph 1.1.4. Meeting space shall include onsite audio/visual support, a projector with computer connection capabilities, projection screens, extension cords, flip-charts with easels, writing instruments, pads of paper, and tables and chairs to support.

1.1.4.1. Activities to Reinforce Training. The contractor must design and provide activities within the times noted that build on and enhance the training sessions. These activities must occur outside of the meeting space noted in paragraph 1.1.3. Examples of activities may include, but are not limited to: kayaking or tandem bikes to build and reinforce teamwork, murder mysteries to reinforce conflict resolution, or describe a sandcastle/partner builds it to reinforce communication.
1.1.5. Alternate Dates. The Government may have to reschedule events due to unforeseen circumstances. The Government reserves the right to reschedule an event at no additional cost, provided that at least a ten (10) day notice is provided to the contractor in writing. The contractor shall propose dates for the rescheduled event(s).
1.1.6. Meals. The contractor shall provide five (5) meals for each participant. Meals shall consist of the following: two (2) hot breakfasts, one (1) lunch, and two (2) dinners. Lunch will not be provided on third day of event. All meals shall be buffet style consisting of at least three (3) entrée choices, five (5) choices of side items, and no less than three drink options to include water. Food shall be fresh, safe, unadulterated, and honestly presented. Dinners shall include two meat options, as well as a plant based protein option. The contractor shall support specific dietary requirements as requested (e.g., Vegan options, food allergies, etc.) prior to the event start by the event facilitator. The event facilitator will provide the dietary requirements to the contractor two weeks prior to the event start.

Guest check-in must be available to event participants by 3:00 p.m. on the first day of the event. Guest checkout shall be extended to

1.3. EFMP FAMILY RETREATS. One (1) Event shall be scheduled per performance period and shall be within 30 miles of Hurlburt Field, Florida. EFMP Family Retreats should be designed to offer families information and training on disability specific resources. Topics may include, but are not limited to: disability resource fair, special education, assistive technology, advocacy, transferring through transitions, legal and financial issues, parent panel, navigating the
Individual Education Plan (IEP)/504 process, sibling education, disaster preparedness, post-secondary education and employment, empowerment, understanding the systems, and coping strategies unique to EFMP families. Additional topics covering stressors may also include, but are not limited to: the mental/emotional trials that come with diagnoses, education, medical care, advocacy, case management, insurance, feelings of isolation and anxiety, personal and marital stress, military moves, and the impact on the typically developing siblings in the home.

1.5. ONSITE CHILDCARE. The contractor shall provide childcare, meals, and room(s) for children up to the quantities listed in the monthly event information document for Marriage
Weekend Retreats, Family Weekend Retreats, EFMP Family Retreats, and Single Parent Family
Retreats by a licensed and government approved childcare provider. Contractor must ensure childcare providers adhere to Florida Statutes Sections 402.26 - 402.319 and the Florida
Administrative Codes, Chapters 65C-20, 65C-22 and 65C-25 for operation of childcare facilities.
The list of childcare providers shall be submitted to the event facilitator for approval three (3) days prior to event start date. The number of rooms needed will be determined by the ages and individual special needs of the children attending. The numbers of children may be different at each event, and an estimated amount of children shall be presented by the Chaplain Corps to the contractor within 21 days prior to the start date of each listed event. The Chaplain Corps shall provide a confirmed total number of children and their associated ages for each event to the contractor at least seven (7) days in advance of the scheduled event. The numbers of children may be different at each event, but will accumulate to an average of 45 children per event throughout each contract year. Government shall not expect a credit for partial attendance or non-attendance of children confirmed for each event.
1.5.1. EFMP CHILDCARE. The contractor shall provide childcare by a licensed special needs trained professional for EFMP Family Retreats. Childcare provider must be licensed within the state of Florida. Providers will have at least one year of experience working with individuals with disabilities, experience with behavior management, trained in CPR/First Aid, trained in child abuse recognition/prevention, medication administration, and will need to provide developmentally appropriate activities.
1.5.1.1. SPECTRUM OF CHILDCARE RATIO DETERMINATION:
Routine Care = Low Need (Based on Florida State Standards for Childcare ratios as listed below)
• 1 to 4: from birth to 12 months of age
• 1 to 6: if no more than three are under 12 months
• 1 to 10: as long as at least five children are school-age and no more than two are under 12 months of age
• 2 to 8: if more than 4 children under 24 months old
• 2 to 12: if no more than 4 children are under 24 months old
More than Routine Care = Moderate Need (Childcare ratio of 1 to 3)
Intensive Care = High Need (Childcare ratio of 1 to 1)
Items to be considered to determine childcare ratio: severity/type of disability; risk of harm self/others; flight risk; age appropriate skills (eating, toileting); co-existing conditions (medical, developmental disability, delay or psychiatric), environmental stress and environmental support
(items that worsen/improve child or adolescent condition), emotional strength/stability and capacity for successful adaptation.
1.5.1.2. The childcare provider will provide care and developmentally appropriate activities for children and/or young adults with/without disabilities. Saturday night movie is acceptable but must include an alternative activity for those unable/unwilling to participate. The movie must be approved by the event facilitator before showing.
1.5.1.3. The contractor shall ensure childcare providers submit a childcare activity plan and alternative activities, carefully considering disabilities and unique care needs of the registered children. The plan shall be submitted to the event facilitator for approval three (3) days prior to event start date.
1.6. ADDITIONAL REQUIREMENTS.
1.6.1. Annual Familiarization. The contractor shall provide information and content updates for the Marriage Weekend Retreats, the Family Weekend Retreats, the EFMP Family Retreats, and the Single Parent Family Retreats at the beginning of each contracting year to all Chapel staff members. This familiarization will be comprehensive and include a full breakdown of the curriculum and will not exceed one (1) day. Chapel staff members will be able to provide input to the instruction to ensure that it best fits the audience needs. This will be conducted at the
Hurlburt Field Chapel prior to the first event for each contract year.
1.6.2. Instructor information and Biographies. The contractor shall provide information on the primary and secondary (if applicable) instructors, as well as a biography of each instructor, coach, and faculty member that will be in attendance at Marriage Weekend Retreats, Family
Weekend Retreats, EFMP Family Retreats, and Single Parent Family Retreats. Biographies at a minimum will include: name, education, experience, credentials, and a photograph. Instructor information and biographies will be due to the facilitators no later than 14 workings days prior to each event.
1.6.3. Changes. The contractor shall make a good faith effort to accommodate roster changes due to military obligations up to the day of the event. The Chapel staff will work to ensure that these changes are minimal and do not affect the number of rooms or child care providers that were required upon the original roster being sent.
1.6.4. Participant Survey. The contractor shall submit participant survey feedback to the event facilitator within seven (7) days at the conclusion of each retreat.
1.7. Deliverables. All deliverables must meet professional standards and meet the requirements set forth in contract documentation. The contractor will be responsible for delivering all end items specified. The following items are sample of deliverables that fall within the scope of the aforementioned tasks and all other stated deliverables.

1.7.1. Event Schedules. The contractor shall provide a schedule for each scheduled Marriage
Weekend Retreat, Family Weekend Retreat, EFMP Family Retreat, and, Single Parent Family
Retreat, respectively to the event facilitator.
1.7.2. Deliverable Instructions. Deliverables will be submitted electronically, if possible, and hard copy if wet signature is required, and will be compatible with AF-standard desktop applications.
2.0. Service Summary (SS). The contract service requirements are summarized in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The SS provides information on contract requirements, the expected level of contractor performance and the expected method of government validation and confirmation of services provided. These thresholds are critical to mission success.

3.0. General Information.
3.1. Alternative Dates. The Government may have to reschedule events due to unforeseen circumstances. The Government reserves the right to reschedule an event at no additional cost, provided that at least a ten (10) day notice is provided to the contractor in writing. The contractor shall propose dates for the rescheduled event(s).
3.2. Contractor Personnel. The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate or alternates, who shall act for the contractor when the manager is absent, shall be designated in writing to the contracting officer prior to the contract start date. The contract manager or alternate(s) shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contract manager and alternate(s) must be able to read, write, speak, and understand the English language.
3.3. Recognized Holidays. The contractor is not expected to work on any of the following holidays:

3.4. Reporting Full-Time-Equivalent (FTE) Data. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site. The contractor is required to completely fill in all required data fields at the System for Award Management (SAM) site https://sam.gov/SAM/.
3.5. Reporting Period. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September.
While inputs may be reported any time during the FY, all data shall be reported no later than 31
October of each calendar year. Contractors may direct questions to the SAM help desk.
3.5.1. Uses and Safeguarding of Information. Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
3.5.2. User Manuals. User manuals are available on the front page of the SAM link at https://sam.gov/SAM/.
3.6. Safety Contractor personnel shall comply with all applicable laws and safety regulations. Compliance with Occupational Safety and Health Administration
(OSHA) and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the Government shall assume no liability or responsibility for the contractor’s compliance or non-compliance with such responsibilities.
