Dear Mr. Powell:

This report serves as a summary of work as outlined in the Summary of Work requirements for Contract Number FA4417-11-P-0184, project number FTEV 11- 1114A awarded to Porter Scientific, Inc. (PSI) on September 8, 2011. The project pertained to the inspection and repairs of four (4) water towers to include the inspection, reporting, minor repairs, and/or recommendations to the tanks and the Cathodic Protection systems. The specific details called for under this contract are defined below in the Summary of Work performed for all In-Service and Out-Of- Service requirements.

SUMMARY OF WORK:

A. PSI furnished all labor, materials, tools, supervision and equipment, and performed all operations necessary to accomplish all work per the contract specifications scope of work dated August 23, 2011. Only one (1) Water Tower was out of service at any given time, and all down time on the Water Towers was kept to a minimum. We maintained multiple facilities within the specifications for utility outages and coordinated the schedule with the Contracting Officer representative. All repairs and maintenance work were completed in accordance with AWWA Standard D100 and D102.

B. We performed work Per Florida statute DEP 2005, 62-555.350 stating “exterior of water towers shall be checked annually, to ensure that hatches are closed, locked and screens are in place, by personnel under the responsible charge of a professional engineer licensed in Florida.” The inspection reports included are the detailed written findings, recommendations, with colored photographs, along with all recommendations and including minor repairs of exterior. E.g.

scrape and paint rust, replaced screens, hatch closing, etc. that were completed, and are provided along with the reports (two (2) copies and one (1) CD, in Word format).

Overview:

Porter Scientific verifies that all the original elements and modifications of the contract have been completed as spelled out with the following detailed summary of the procedures and processes used to completely satisfy the contract requirements. The information gained from these inspections should be used to compile recommendations for ongoing maintenance and to offer a contractual agreement to provide the required services. In this report, you will find descriptions of the conditions of the tanks along with photographs to support the recommendations.

Project summary details:

1. The notice to proceed (NTP) was issued on December 15th, 2011 with a modified performance period (PP) of 100 days. The NTP, PP, and work schedule issued were all depending on the availability to activate the cathodic protection systems without causing any damage to the existing tanks that required a one year waiting period prior to startup.

2. The week on February 27th, 2012, the initial inspections of the tanks were performed to determine the extent of the repairs required and this entailed three days, one of mobilization and two of field inspections.

3. During the week of March 13th, 2012, cathodic protection inspections were performed with detailed report attached in appendix.

4. Also during the week on March 13th, 2012, based on the initial inspections of the tanks, exterior repairs, minor scraping, and touchup of the tank exteriors were performed. Summary recommendations are provided for any issues found during the inspection.

This final report, which includes each individual tank’s inspection report summary recommendation, represents the cumulative effort as defined per the original contract.

General Information

INTRODUCTION

On 28 February 2012, the Bldg 90335 Tower #2 250,000 Gallon Elevated Water Storage Tank was inspected by UTILITY SERVICE COMPANY INC. for the Hurlburt Field Air Force Base, Hurlburt Field, AFB, Florida. The purpose of the inspection was to determine the condition of the Protective Coatings and Structure and evaluate the tank for compliance with Sanitation guidelines and Safety & Security regulations.

The information gained from this inspection will be used to compile recommendations for ongoing maintenance and to offer a contractual agreement to provide this service. In this report, you will find a description of the condition of this tank along with photographs to support the recommendations.

Protective Coating Conditions

EXTERIOR COATING

The exterior surfaces are coated with an Acrylic Polyurethane coating system. The exterior coatings average 10 – 12 mils dry film thickness. The primer, intermediate and finish coat adhesion is good.

RISER, COLUMNS AND STRUCTURAL MEMBERS

The protective coating on the riser, columns and structural members is good condition. There is mildew present on struts at all levels. Spot failures due to failed coatings with minor oxidation and corrosion on the all surfaces to include several anchor bolts, riser rods and especially along the access ladder and its standoffs.

BOWL, BALCONY, SIDEWALLS AND KNUCKLE

The protective coating on the bowl is in good condition but has several areas of oxidation and corrosion. The protective coating on the balcony is in good condition but has several areas of oxidation and corrosion along the floor plates where water has failed to drain and on the handrail top rail and uprights. Sidewall coating is in good condition with minor spot failure and considerable chalking diminishing the color and gloss retention of the base coat and logos. The protective coating on the knuckle is in good condition again with areas of coating failure resulting in oxidation and surface corrosion, primarily around the additional apparatus welded in place.

DOME

The protective coating on the dome is in good condition. There are areas of minor spot oxidation and corrosion on some of the dome plates and weld seams primarily around the additional apparatus welded in place. There a consistent thinning of the coating through out the dome.

In summary the exterior protective coating is providing a protective barrier; however it has begun to show wear due to age, environmental exposure and lack of preventative maintenance. Now is the time to conduct spot touch up operations of all affected areas and prepared overcoat the tank to protect the structure and salvage the existing coatings. This will extend the life cycle of the existing coating out another seven to eight years with the goal of not having to blast the exterior back to bare metal.

RECOMMENDATIONS

• Immediate touch up of all affected areas. The exterior should receive a power wash and power tool cleaning, and then receive a spot prime an intermediated coat and full finish coat in the next 2 to 3 years.

• The application of a single coat system to all exterior surfaces that meet the applicable requirements of AWWA Standard D102-06.

INTERIOR COATING

The interior surfaces are coated with an Epoxy coating system. Only a visual inspection of the wet interior was conducted with only the coated surface area above the water line in the head space observed.

ROOF AND AREA ABOVE HIGH WATER LEVEL

The protective coating on the roof plates and the area above the high water level is in good condition.

SIDEWALLS

Not observed.

FLOOR

Not observed.

WET RISER

Not observed.

In summary the interior coatings appear to be in good condition.

RECOMMENDATIONS

• The interior should receive a SSPC-SP10 “Near White” blast cleaning, and then receive two coats of an NSF approved epoxy coating at the 10 – 12 year mark.

• Continue to perform a washout inspection every two years to inspect interior surface areas and remove sediment accumulation.

Structural, Sanitation, Safety and Security Conditions

STRUCTURAL

Columns

All columns are in good condition and serviceable.

Riser

The riser is in good condition and serviceable.

Struts

All struts and brackets are in good condition and serviceable.

Riser Rods

All riser rods and brackets are in good condition and serviceable.

Wind Rods

All wind rods are in good condition and serviceable.

Balcony / Catwalk / Handrails

The balcony is in good condition and serviceable. The handrails are in good condition and serviceable. Water is ponding in areas and additional drains holes are necessary.

Manways / Hatches

All manways and hatches are in good condition and serviceable.

Welds / Bolts / Rivets

All structural welds are in good condition and serviceable.

Ladders

Exterior – Access and Dome, structurally diminished and in poor condition and of questionable serviceability.

Interior – Structurally sound and in good condition and serviceable but could use supports or replace with more ridge model.

Riser – Not observed.

Overflow Assembly

The overflow assembly is functional and in good condition and serviceable. A screen is present, in poor condition and serviceable but no flapper is present as per FDEP requirements.

Target and Float Assembly

Float system is inoperable and should be removed.

Vents / Finial Ball

The roof vent is sufficient, functional and in good condition and serviceable.

Antennas

Local RF antenna mounted to the balcony handrail.

Cathodic Protection / Telemetry

Cathodic Protection System is present, operational, in good working condition and serviceable. CP plates need to be address and resealed.

Interior Structure

Seams / Welds – Welds appear to be structurally sound and in good condition with no apparent cracks or holes.

Rafters / I Beams – N/A

Painters Ring – N/A

Spider Rods – N/A

Riser Grate / Protective Handrail – Not Observed.

Anchor Bolts / Nuts / Chairs

The anchor bolts and nuts are structurally sound, in good condition and serviceable.

Foundations

The foundations are in good condition and serviceable with only minor cracks and chips.

All the foundations are above grade with sufficient run off and drainage. The grout between foundations and the tank columns are in good condition and serviceable. The riser grout is in poor condition and unserviceable.

RECOMMENDATIONS

• Install balcony drain holes.

• Install access ladder.

• Install dome ladder.

• Install overflow pipe flapper and screen assembly.

• Remove target and float assembly.

• Adjust and reseal CP plates.

• Repair riser foundation grout.

SANITATION

Roof Hatch

The roof access hatch is in good and serviceable condition and of a style that can be locked.

Roof Vent Screen

The roof vent screen is in good condition and serviceable.

Overflow Assembly Screen & Flapper

An overflow screen is present, in poor condition and unserviceable. A flapper is not present and should be installed as per ADEM

Protective Fill Pipe Cover

Not observed.

Cathodic Protection Plates

CP plates need to be address and resealed.

RECOMMENDATIONS

• Install overflow pipe flapper and screen assembly.

SAFETY & SECURITY

Safety Climb System

The access and dome ladders have OSHA required and approved safety climb fall arrest systems that are present in good condition and serviceable, interior ladder is void of a required safety climb fall arrest systems.

Ladder Gate Climb Prevention Shield

A Ladder Gate Climb Prevention Shield is present, in good condition and serviceable.

Access Manways

Riser access manways is bolted, secure and water tight. Roof hatch is locked and secure. All access manways are in good condition and serviceable.

Balcony / Catwalk / Handrails

The balcony is in good condition and serviceable. The handrails meet OSHA requirements and are in good condition and serviceable. Water is ponding in areas and additional drains holes are necessary.

Riser Grate / Protective Handrail

Not observed.

Aviation / Warning Lights / Beacon

A double rotating airfield beacon, double globe aircraft warning light and solar powered single globe aircraft warning light are all present. Operability not observed due to daylight hours.

Tank Site

The tank site is fenced and secure.

RECOMMENDATIONS

• Install interior ladder safety climb fall arrest system.

• Install balcony drain holes.

SUMMARY

In summary, this water storage tank is in generally good condition serviceable from a structural, sanitary, safety & security standpoint. Minor renovations are required to correct the coating deficiencies identified on the Exterior and Interior surfaces. If not corrected, these areas will continue to degenerate. It is recommended that corrective action is taken in regards to the Exterior and Interior surfaces with proposed Protective Coating Systems. The identified Structural, Sanitary, Safety, & Security deficiencies should be corrected and brought up to code as well.

Additionally, an Annual Washout & Visual Inspection Program should be considered, separately, or as part of an Annual Maintenance Program for comprehensive Tank Maintenance.

RECOMMENDATIONS

• Immediate touch up of all affected areas. The exterior should receive a power wash and power tool cleaning, and then receive a spot prime an intermediated coat and full finish coat in the next 2 to 3 years.

Manways / Hatches

All structural welds are in good condition and serviceable.

Ladders

Exterior – Access and Dome, structurally diminished due standoffs in poor condition and of questionable serviceability.

Interior – Structurally sound and in good condition and serviceable but should be replaced with a single straight unit capable of receiving a safety climb system.

Riser – Not observed.

Overflow Assembly

The overflow assembly is functional and in good condition and serviceable.

Target and Float Assembly

Float system is inoperable and should be removed.

Vents / Finial Ball

The roof vent is sufficient, functional and in good condition and serviceable.

Antennas

Local security camera mounted to the balcony handrail.

Cathodic Protection / Telemetry

Not all the foundations are above grade with sufficient run off and drainage and should be exposed. The grout between foundations and the tank columns are in good condition and serviceable. The riser grout is in poor condition and unserviceable..

RECOMMENDATIONS

• Weld repair balcony handrail uprights.

• Install balcony drain holes.

• Repair/replace access and dome ladder standoffs as necessary.

• Install 24” bolted riser manway with davit arm.

• Install interior ladder.

• Remove target and float assembly.

• Adjust and reseal CP plates.

• Remove spider rod assembly.

• Repair riser foundation grout.

• Expose all foundations.

Roof Hatch

The roof access hatch is in good and serviceable condition and of a style that can be locked.

Roof Vent Screen

The roof vent screen is in good condition and serviceable.

Overflow Assembly Screen & Flapper

An overflow screen is present, in good condition and serviceable.

Protective Fill Pipe Cover

Not observed.

RECOMMENDATIONS

INTRODUCTION

On 28 February 2012, the BLDG 91134 TOWER #7 250,000 Gallon Pedisphere Water Storage Tank was inspected by UTILITY SERVICE COMPANY INC. for the Hurlburt Field Air Force Base, Hurlburt Field, AFB, Florida. The purpose of the inspection was to determine the condition of the Protective Coatings and Structure and evaluate the tank for compliance with Sanitation guidelines and Safety & Security regulations.

The information gained from this inspection will be used to compile recommendations for ongoing maintenance and to offer a contractual agreement to provide this service. In this report, you will find a description of the condition of this tank along with photographs to support the recommendations.

EXTERIOR COATING

The exterior surfaces are coated with an Acrylic Polyurethane coating system. The exterior coatings average 10 – 12 mils dry film thickness. The primer, intermediate and finish coat adhesion is good.

PEDESTAL

The protective coating on the pedestal is in good condition. There is diminished color and poor gloss retention throughout. There is a minimal amount of mildew present on the pedestal throughout its circumference but primarily along the foundation. Spot failures due to failed coatings with minor oxidation and corrosion on the all surfaces to include several anchor bolts

BOWL AND SIDEWALLS

The protective coating on the bowl is in good condition with diminished color and minimal gloss retention throughout. The sidewall coating is in good condition. There is diminished color and minimal gloss retention throughout. The protective coating on the knuckle is in good condition again with areas of coating failure resulting in oxidation and surface corrosion

DOME

The protective coating on the dome is in good condition. There are areas of spot oxidation and corrosion on some of the dome plates and weld seams. There is consistent thinning throughout the dome.

In summary the exterior protective coating is providing a protective barrier; however it has begun to show wear due to age, environmental exposure and lack of preventative maintenance. Now is the time to conduct spot touch up operations of all affected areas and prepared overcoat the tank to protect the structure and salvage the existing coatings. This will extend the life cycle of the existing coating out another seven to eight years with the goal of not having to blast the exterior back to bare metal.

RECOMMENDATIONS

• Immediate touch up of all affected areas. The exterior should receive a power wash and power tool cleaning, and then receive a spot prime an intermediated coat and full finish coat in the next 2 to 3 years.

The interior surfaces are coated with an Epoxy coating system. Only a visual inspection of the wet interior was conducted with only the coated surface area above the water line in the head space observed.

ROOF AND AREA ABOVE HIGH WATER LEVEL

The protective coating on the roof plates and the area above the high water level is in good condition but is experiencing varying levels of failure. There is considerable oxidation and corrosion along the roof weld seams, at the CP plates and blanket rust is visible on the shell walls above the water line.

SIDEWALLS

Not observed.

PASSAGE TUBE

Not observed.

FLOOR

The protective coating on the floor is in poor condition with failure throughout. There are areas of Not observed.

In summary the interior coatings appear to be in good condition.

INTERIOR DRY COATING

The interior surfaces of the dry riser are coated with an Epoxy coating system. Only a visual inspection of the wet interior was conducted. The interior coatings average 10 mils dry film thickness.

The adhesion properties of the existing two coat system are good.

PEDESTAL, BALCONY, BOWL, STRUCTURAL MEMBERS

The protective coating in the dry pedestal is in good condition with minor abrasions in the lower portions, some mildew throughout. The protective coatings on the pedestal balconies are in good condition but the coatings on the catwalk and handrails is showing varying levels of failure and will require more extensive corrective action once the heavy amount of bird excrement are removed.

PASSAGE TUBE

The protective coating in the passage tube is good condition with minor abrasions, mildew throughout and staining along the length from both bird excrement and from the rusting and corrosion occurring on the structures at the upper portions.

In summary the dry interior coatings are in good condition but require some minor preventative maintenance. This area of the tank was recently over coated with the exception of the upper area in the access tube (this area is showing dirt & mildew buildup from condensation). The exterior of the ductile iron inlet pipe is showing 20% coating delamination. This is mainly coated for aesthetics as the bituminous primer is adequately protecting the substrate.

RECOMMENDATIONS

• The dry interior should receive a power wash and power tool cleaning as necessary, and then receive a spot coat in the applicable areas. Primarily the upper portion of the dry passage tube.

• The inlet pipe is recommended to be spot prepped (SSPC-SP2 & SP3) and touched up with epoxy paint.

STRUCTURAL

Column / Pedestal / Pillar

The pedestal is in good condition and serviceable.

Fill Pipe

The fill pipe is in good condition and serviceable.

Balcony / Catwalk / Handrails

The catwalks are in good condition and serviceable. The landings are in good condition and serviceable. The handrails are in good condition and serviceable.

Manways / Hatches /Accessways / Doors

The passage tube dome access manway hatch is in good condition and serviceable.

The wet interior access manway hatch is in good condition and serviceable. Wet interior access manway is clamped, secure and water tight.

Welds / Bolts / Rivets

All structural welds are in good condition and serviceable.

Ladders

Dry Interior - Structurally sound and in serviceable condition.

Wet Interior - Structurally sound and in serviceable condition.

Overflow Assembly

The overflow assembly is adequate, functional and in serviceable condition.

The roof vent is sufficient, in good condition and serviceable.

The foundations are in good condition and serviceable with only minor cracks and chips.

The foundation is below grade in some areas around the circumference and allows for insufficient run off and drainage. The grout line around the tank/foundation junction is in poor condition and should be repaired.

RECOMMENDATIONS

• Adjust and reseal CP plates.

• Repair foundation grout and caulk.

• Access tube repair. Floating type (not permanently affixed to the roof). Need to cut collar off that is welded to the roof & install a stiffener/doubler plate (1/4in thick x 8 in wide ring), solid weld doubler plate to access tube exterior. (Included is a drawing from another tank of what this repair entails, just for reference only.)

Roof Hatch

The roof access hatch is in good and serviceable condition and of a style that meets FDEP requirements as well as can be locked.

Roof Vent Screen

The roof vent screen is sufficient, in good condition and serviceable.

Overflow Assembly Screen & Flapper

Overflow screen and flapper are present, in good condition and serviceable.

In summary, this water storage tank is in generally good condition serviceable from a structural, sanitary, safety & security standpoint. Minor renovations are required to correct the coating deficiencies identified on the Exterior and Interior surfaces. If not corrected, these areas will continue to degenerate. It is recommended that corrective action is taken in regards to the Exterior and Interior surfaces with proposed Protective Coating Systems. EXTERIOR COATING

The exterior surfaces are coated with an Acrylic Polyurethane coating system. The exterior coatings average 10 – 11 mils dry film thickness. The primer, intermediate and finish coat adhesion is good.

RISER, COLUMNS AND STRUCTURAL MEMBERS

The protective coating on the riser, columns and structural members is good condition. Spot failures due to failed coatings with minor oxidation and corrosion on the all surfaces.

BOWL, BALCONY, SIDEWALLS AND KNUCKLE

The protective coating on the bowl is in good condition. The protective coating on the balcony is in good condition but has several areas of oxidation and corrosion along the floor plates where water has failed to drain and on the handrail top rail and uprights, also along the underside of the balcony floor plates. Sidewall coating is in good condition with the color and gloss retained. The protective coating on the knuckle is in good condition.

DOME

The protective coating on the dome is in good condition. There are areas of minor spot oxidation and corrosion on some of the dome plates and weld seams primarily around the additional apparatus welded in place and at the CP plates.

In summary the exterior protective coating is providing a protective barrier; however it has begun to show wear due to age, environmental exposure and lack of preventative maintenance. Now is the time to conduct spot touch up operations of all affected areas and prepared overcoat the tank to protect the structure and salvage the existing coatings. Exterior – Structurally sound and in good condition and serviceable.

Interior – Structurally sound and in good condition and serviceable.

Riser – Not observed.

Overflow Assembly

The overflow assembly is functional and in good condition and serviceable. A screen is present, in poor condition and serviceable but no flapper is present as per FDEP requirements.

Target and Float Assembly

Float system is inoperable and should be removed.

Vents / Finial Ball

The roof vent is sufficient, functional and in good condition and serviceable, but should be replaced with a secure unit.

Antennas

Local security camera mounted to the balcony handrail.

Cathodic Protection / Telemetry

Seams / Welds – Welds appear to be structurally sound and in good condition with no apparent cracks or holes.

• Remove target and float assembly.

• Install 24” secure vent assembly.

Roof Hatch

The roof access hatch is in good and serviceable condition and of a style that can be locked.
