About this file
This is a Past Performance Information Sheet template that offerors must complete for the sludge removal requirement (Solicitation FA440726QTW14) issued by the 375th Contracting Squadron at Scott Air Force Base, Illinois. Offerors are required to submit three separate completed sheets, each documenting a relevant past performance reference performed within the past five years from the RFQ issuance date.
The template requires offerors to provide general project information, customer/agency details with a point of contact willing to participate in performance interviews, contract value and performance period dates, and a detailed narrative describing work performed. The narrative must specifically address relevance to the RFQ's Statement of Work by explaining the scope (land securing for application), magnitude (handling annual volumes of 600,000 gallons or more), and complexity (EPA 40 CFR 503 compliance, IEPA regulations, soil testing, and permit compliance) of the effort. Offerors must also document evidence of performance quality through CPARS ratings, letters of recommendation, or other performance assessments. The form grants the U.S. Government permission to contact listed customer points of contact to verify information and inquire about performance on cited contracts. References must involve the disposal of liquid sludge (biosolids) and compliance with EPA and IEPA regulations. If work was performed by a subcontractor or teaming partner, offerors must clearly identify the performing company and describe its specific role.
