STATEMENT OF WORK

Remove and Replace Brand Name 12” Backflow

Preventer JBSA Lackland AFB

BLDG. 2901

PROJECT SCOPE: The contractor shall provide all supervision, labor, transportation, materials, tools, incidentals, equipment, and quality assurance to replace and install a Brand Name 12” backflow preventer; AMES, Model: 2000SS-OSY 12IN Double Check Valve Backflow Preventer Assembly, Domestic OSY (Outside Screw and Yoke) Shutoffs with Cam Check Valves located at B2901 Golf course JBSA-Lackland, San Antonio TX. The work shall be in accordance with all applicable Federal/Local codes, manufacturer standards and specifications.

SITE DESCRIPTION AND SIGNAGE: The golf course will be occupied while work is being performed. The contractor shall provide barriers and warning signs to prevent unauthorized personnel from entering the work area while the contractor is replacing the backflow preventer system.

PROJECT LOCATION: 1800 Dimsted Pl JBSA Lackland, San Antonio TX

BACKGROUND: The 12” Backflow located at JBSA Lackland Golf Course B2901 is currently violating the Texas Commission on Environment Quality (TCEQ), which can potentially handout a notice of violations that can lead to consisting of fines, restrictions and potential removal of water service tap. The main objective for the contractor will be to replace the backflow and to bring it up to state-mandated requirements. The current backflow preventer is seized internally on the check valves and main control valves, failing all routine state mandated testing protocols. Replacement of the backflow prevention assembly will keep 502 CE within state mandated compliance, preventing any future violations, fines or discontinued system usage.

SCOPE OF WORK: The Contractor shall coordinate with the Facility Manager, or CE Project Manager, before the actual work is to commence to minimize mission impact or deficiencies that may occur during the normal work week, Monday through Friday 7:00 a.m. to 5:00 p.m. The Contractor shall provide the following items and incidental services:

A. Contractor shall isolate and decommission the existing 12” backflow preventer and replace with AMES, Model: 2000SS-OSY 12IN Double Check Valve Backflow Preventer Assembly, Domestic OSY (Outside Screw and Yoke) Shutoffs with Cam Check Valves.

B. The contractor shall safely remove the existing assembly and associated components, reconnect piping, and ensure all fittings, valves and supports comply with Texas Administrative Code Title 30, Chapter 344. The contractor shall perform pressure testing and certification by a licensed, TCEQ-approved backflow prevention assembly tester. Upon completion of the 12-inch backflow installation, the contractor shall restore the system to full operation and provide 502 CE PM and 502 CE Plumbing shop with documentation of the installation and testing. The form will provide the initial testing documentation that shows the Backflow Test Report.

AMES Model 2000SS-OSY Double Check Valve Assembly

This Back Flow Preventer shall be a full-featured commercial industrial product for the

PERIOD OF PERFORMANCE: 30 calendar days for Project to be complete. A ten-day notice before work begins is necessary for coordination with the building’s manager.

HOURS OF WORK: Normal duty hours are from 7:00 AM to 5:00 PM, Monday through Friday, except on government holidays. The contractor should coordinate work with 502 CE Project Manager.

PROJECT SCHEDULE PLAN: Contractor shall send a progress schedule plan to CE Project Manager and mobilize plan of start date and completion date after contract has been awarded. The progress schedule plan should include the contractors’ working schedule.

INSPECTION AND ACCEPTANCE: A representative from 502 CES will conduct daily inspections of work progress. A final inspection of the completed work will be conducted by the project manager, contractor and designated 502 CES personnel. All discrepancies identified during inspections shall be corrected prior to final payment.

CONTRACTOR’S WARRANTY: The contractor shall guarantee all materials and workmanship for a period of one year from the date of project completion and shall within that period correct all defects in materials/workmanship within 36 hours of being notified. Correction of all defects under this warranty shall be at the expense of the contractor. Equipment with over a year warranty, all paperwork shall be turned into the CE PM at the time of Project Completion.

SAFETY: All Contractor operations shall be conducted and performed in accordance with Department of Labor, OSHA requirements found in 29 CFR 1910 (1910.146 and 1910.147) and 29 CFR 1926, and Air Force Occupational Safety & Health (AFOSH) AFOSH standards including 91-125.

DIGGING PERMIT: If Contractor requires digging permit, 502d CE will help coordinate the permit.

OUTAGES: If Contractor requires a water outage permit, 502d CE will help coordinate the permit.

SITE MAINTENANCE: The Contractor shall protect adjacent property, buildings, and their contents from dust, dirt, or other material. Work areas shall be maintenance din a neat, clean and safe condition and shall, at a minimum, be cleaned at the end of each shift. All streets and roadways in/or adjacent to the site shall always remain free of project generated trash and debris.

GOVERNMENT FURNISHED SERVICES: The Government will provide utilities (water and electricity) and emergency fire and police security response.

POLICE PROTECTION: The 802nd Security Forces Squadron shall provide security and law enforcement response and services to the contractor.

FIRE PROTECTION: The JBSA Lackland Fire Department shall provide initial response, EMS, and fire protection services to the Contractor.

TRAFFIC REGULATIONS: Contractor employees shall comply with all base traffic regulations and are subject to random vehicle speed control checks. Contractor’s personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones, except hands-free phones, is prohibited while driving on military installations in San Antonio, TX.

DAILY CLEANING: The Contractor shall always keep the site free from debris and shall perform daily cleaning of all areas. The Contractor shall collect all trash, debris, refuse, garbage, etc., that is generated and place it in appropriate containers with lids or approved covers on a periodic basis. The materials shall be hauled from the site by appropriate means daily, unless otherwise approved by the designated Planning Technician. Disposal shall be outside the limits of Government property. Disposal shall be by sanitary landfill or other approved methods and shall conform to all local, state, and federal guidelines, criteria, and regulations. Upon completion of the work, the Contractor shall leave the work site and storage area(s) in a clean, neat, and workmanlike condition satisfactory to the Structural Shop Supervisor.
