PERFORMANCE WORK STATEMENT
FOR
AVIATION SUPPORT EQUIPMENT LOAD TESTING
433rd Maintenance Squadron/Aircraft Maintenance Squadron
Joint Base San Antonio Lackland AFB
8 September 2023
1 Introduction
Non-Personal services to perform preventive maintenance (PM) inspections, repairs, load tests, certification service on aviation support lift equipment for the 433rd MXS/AMXS, Joint Base
San Antonio-Lackland in accordance with all applicable Air Force Standards, Air Force
Instructions (AFI), and Technical Orders (TO), laws, regulations, standards, and commercial instructions by equipment manufacturer and requirements described in this Performance Work
Statement (PWS) with the focus on safety, timeliness, and customer service.
1.1 Mission
The 433rd Maintenance Squadron (MXS) and Aircraft Maintenance Squadron (AMXS) are responsible for the maintenance of 9 assigned C-5M Galaxy airframe. The 433 MXS/AMXS primarily performs flight line tasks to include daily mission launch and recovery, pre- and post-flight inspections, maintenance troubleshooting, aircraft systems repair, and minor modifications.
1.2 Scope
The contractor shall perform annual preventive maintenance (PM) inspections, load tests, certification, as well as unscheduled and/or repairs, if necessary, to ensure reliable and continuous safe operation of all aircraft support equipment. The contractor shall provide project management, supervision, all personnel, equipment, tools, materials, transportation and all other items and services necessary to successfully perform maintenance and repairs. The contractor is encouraged to use innovative approaches to accomplish PWS requirements efficiently and effectively in a timely manner at reduced cost and in a way that fosters pride and ownership in the work performed. All work performed by the contractor in this PWS shall conform applicable specifications and standards. It is the contractor’s responsibility to ensure compliance with the most recent version of the document below.

2 General Requirements
This section describes the general requirements for this effort. The following sub-sections provide details of various considerations on this effort.
2.1 Non-Personal Services
The Government shall neither supervise contractor employees nor control the method by which the contractor performs the required tasks. Under no circumstances shall the Government assign tasks to, or prepare work schedules for, individual contractor employees. It shall be the responsibility of the contractor to manage its employees and to guard against any actions that are of the nature of personal services or give the perception of personal services. If the contractor believes that any actions constitute, or are perceived to constitute personal services, it shall be the contractor's responsibility to notify the Contracting Officer (CO) immediately.
2.2 Business Relations
The contractor shall successfully integrate and coordinate all activity needed to execute the requirement. The contractor shall manage timeliness, completeness, and quality of problem identification and provide corrective action plans for timely identification of issues, and effective management of subcontractors. The contractor shall seek to ensure customer satisfaction and professional and ethical behavior of all contractor personnel.
2.3 Contract Administration and Management
The following subsections specify requirements for contract, management, and personnel administration.
2.3.1 Contract Management
The contractor shall establish clear organizational lines of authority and responsibility to ensure effective management of the resources assigned to the requirement. The contractor must maintain continuity between the support operations at 433rd Maintenance Squadron/Aircraft Maintenance
Squadron and the contractor's corporate offices.
2.3.2 Contract Administration
The contractor shall establish processes and assign appropriate resources to effectively administer the requirement and respond to Government requests for contractual actions in a timely fashion. The contractor shall assign work effort and maintaining proper and accurate time keeping records of personnel assigned to work on the requirement.
2.3.3 Personnel Administration
The contractor will not have access to government equipment on non-workdays such as holidays or days that the base is closed for weather or security. The contractor shall provide initial and refresher training as required to meet the PWS requirements and provide necessary infrastructure to support contract tasks.
2.4 Subcontract Management
The contractor shall be responsible for any subcontract management necessary to integrate work performed on this requirement as applicable. The prime contractor will manage work distribution to ensure there are no Organizational Conflict of Interest (OCI) considerations.
2.5 Contractor Personnel, Disciplines, and Specialties
The contractor shall accomplish the assigned work by employing and utilizing qualified personnel with appropriate combinations of education, training, and experience. The contractor shall match personnel skills to the work or task with a minimum of under/over employment of resources. The contractor shall ensure compliance with the labor categories as defined in the
Labor Categories in the DoL Wage Determination, labor rates, and man-hours utilized in the performance of the services in this PWS.
2.6 Location and Hours of Work
The Joint Base San Antonio (JBSA) Lackland normal workdays and hours are Monday through
Friday except US Federal Holidays from 6:00am – 3:00pm. Upon notification by the COR, the contractor will be expected to perform all pick-ups and delivery of equipment during normal working hours if necessary. During situation of inclement weather, maintenance will be halted for the safety of all personnel to include contracted employees. During situation of heightened security, government personnel will coordinate and transport the equipment to the contractors at one of the bases main entry points.
2.6.1. HOLIDAYS OBSERVED
Unless specifically authorized in writing by the Government Representative, no services will be provided, and no charges will be incurred and/or billed on this contract on any of the holidays listed below.

3 PERFORMANCE REQUIREMENTS
The following section specifies the Performance Objectives and Elements for the contract.
3.1 Preventive Maintenance (PM) Services
The contractor shall conduct annual PM inspections, load tests, and certification for all aviation support equipment listed under para 7.1, Load Testing Equipment List. The services shall be intended to maintain the equipment in a safe and reliable operating condition until the next scheduled maintenance. All service shall be completed within the month prior to the expiration date of the current certification date. The contractor shall maintain a schedule as stated in section.
3.3.1. The COR will notify the contractor of when the equipment is coming due for PM services.
The contractor shall coordinate, in advance, with the COR of all deliveries and pick-ups during the normal working hours identified in section 2.6. The contractor shall complete all services and returned all equipment back to 433 MXS/AMXS within 48 hours after initial pickup. All services shall be conducted on all deployed equipment upon its return and updated on the PM schedule. Upon completion of the all the services, the contractor shall provide a PM inspection, load tests, and certification report as stated in para 3.3.2. If repairs are required outside the normal scope of work due to load test failures and/or worn out/broken parts, the contractor shall immediately notify the COR on the next duty day within the normal JBSA Lackland hours listed in para 2.6, a written repair estimate/report shall be furnished as stated in para 3.3.3.
3.2 Repair Services.
The COR will notify the contractor by phone or in person of inoperative equipment requiring repairs. The contractor shall report to the equipment location to survey the condition of the equipment during regular duty hours.
The contractor shall provide a written repair estimate report as specified in para 3.3.3. All estimates must be submitted in writing to COR for review/concurrence and forward to CO/COR for final approval prior to performing any repair work. The contractor shall repair, inspect, load test, and re-certify all equipment resulting in removal and repairs of old parts or the installation of new parts of any component in the load controlling path, a load path in any lifting device, or any load critical areas of aircraft support equipment that requires a load test. After approval from
CO and receipt of repair parts (if gov’t furnished), the contractor shall return equipment back to
433 MXS/AMXS within 3 business days. All unserviceable parts and/or hardware removed to facilitate the repairs are considered property of the United States Air Force and shall therefore be returned to the COR in a clean and ready condition, i.e., free of excess dirt or grease, fluids drained, etc.
3.3 Contractor-Furnished Reports
The contractor shall furnish the following reports to the COR:
3.3.1 Preventive Maintenance Schedule
The contractor shall prepare, maintain, and provide to the COR a current schedule for all support equipment listed in para 7.2 of this PWS. The initial/annual PM schedule shall be provided to the COR within two weeks of the initial/annual performance start date. A revised PM schedule shall be updated and provided to the COR along with the written PM inspection, load test, and certification reports as stated in para 3.3.2 for all unscheduled repairs and equipment recently returned from deployment.

The contractor shall prepare and submit a report within two normal workdays after completion of work. The reports shall identify:
- Pickup/Delivery Date (if required)

- Maintenance work performed.
- Overall condition of the equipment
- Name of the technician(s) who performed the work.

3.3.3 Written Repair Estimates Report
The contractor shall provide written estimates and report for all repairs not covered under the basic PM services at no additional charge within 48 hrs. after initial survey of the condition of the equipment. The reports shall identify:
- Initial Survey Date
- Pickup Date (if required)
- Estimated Time of Completion
- Delivery Date
- Name of aviation support equipment
- Description of Repair work performed o Estimated Cost of Parts (if not provided by the government) o Estimated Cost of Labor + number of hours - Overall condition of the equipment.
- Name of the technician(s) who performed the work - new load test/certification as stated in para 3.3.1.
- Length of Warranty on contractor provided parts.

3.4 Government-Furnished Reports
The COR shall furnish the following reports to the COR:

The Service Summary (SS) includes Performance Objectives that describe the desired result or outcome the contractor shall achieve. The performance threshold states the minimum acceptable level of the performance in terms of quality, timeliness, and quantity in realistic and achievable standards. These thresholds are critical to mission success.

5 SPECIAL REQUIREMENTS:
5.1 Government-Furnished Materials and Property:
5.1.1 Air Force & DoD Publications
The COR will either provide or assist the contractor with Air Force published TO’s, AFI’s and other DOD publications which may not be readily available to the contractor through other means. Except for those items, the contractor shall furnish everything needed to perform this contract according to all terms and conditions.
5.1.2 Repair parts:
Government will provide parts to the maximum extent practicable for all equipment biased on availability in the AF’s Supply system. Contractor may propose alternate parts for government review and acceptance. Government parts take priority over contractor supplied parts.
5.1.3 Workspace:
The contractor may take equipment to be serviced/tested at the contractor off-site work facility.
However, due to the size and transportability issues of some of the equipment, all PM inspection, load test, and repair work for the equipment identified below are allowed to be performed on-site at the 433 MXS facility located at Bldg. 814.

5.2 Security Requirements:
The Joint Base San Antonio-Lackland Base Defense Operations Center (BDOC) provides a 24-hour a-day law enforcement function for JBSA Lackland and our housing area. The BDOC provides normal police functions that are associated with any civilian police force (i.e., traffic control, alarm response, accident, and incident investigations, etc.). The JBSA Lackland BDOC dispatcher is in Building 5275 and can be reached at ext. 671-2018. All security related questions should be directed to the Operations Section at the number listed below:

5.2.1 Entry Requirements during Periods of Increased Security:
Entry requirements identified in this document apply during normal day-to-day security conditions. These procedures may change with little or no notice due to an increased state of security affecting your ability to enter the installation. If you are denied entry to the base due to enhanced security, you should immediately contact the COR or contracting personnel.
*Note: If security procedures are tightened and you are not allowed to enter the installation, please remember the individuals working at the gate are simply following procedures.
5.2.2 Legal Authority for Entry:
Authority for contractors to enter JBSA Lackland is granted by the Commander, Joint Base San
Antonio. Under the authority of the Internal Security Act of 1950, Section 21, contractors, and their equipment are subject to lawful search-and-seizure procedures upon entry or exit from the installation.
5.2.2 Base Entry Points:
Contractors are only allowed entry to the base for contract related business. Keep in mind that installation entry procedures can and will change with short notice dependent upon the current security posture.
5.2.3 JBSA Lackland - Main Gate/Luke Visitors Control Center (Gateway East):
Located at the intersection of Military Drive and Luke Blvd., this gate is open 24 hours a day, seven days a week. The Visitors Control Center (VCC) is open 7-days a week. NOTE: There may be times that the hours change due to manning, FPCON conditions, furloughs, etc. Temporary visitor passes and contractor badges can be accomplished at the VCC. Badges are issued between the hours of 0730- 1630, Monday thru Friday by the contract security manager. Traffic through the Valley Hi gate entrance is limited to authorized personnel with correct identification, contractor badges or temporary passes. No commercial trucks, towing trailers or equipment are authorized thru Valley Hi or Luke Gates: These gates are open 24 hours a day, seven days a week.
Temporary visitor passes and commercial vehicle passes (if needed) can be accomplished at the
Growdon gate. This is the only entrance for commercial trucks, oversized vehicles, towing trailers, or equipment as well as primary entrance for all contractor and vendor vehicles.
Commercial and oversized vehicles must enter using the Growdon gate, this gate is open 5 a.m.
to 5 p.m. daily.
5.2.4 Contractor/Vendor Entry Requirements:
Contractors and vendors requiring access to JBSA will be issued either temporary passes or identification badges from Contract Security Manager. Contractors must maintain current personal identification such as a driver's license or company ID card on them while they are on
JBSA Lackland. At a minimum, this ID should include a picture, the physical description of the individual, date of birth, and a signature. All personnel issued contractor/vendor badges must display their badge(s) on the front of their outermost garment (i.e., shirt, jacket etc.) located on the upper torso while working on JBSA. If this should be a safety hazard, the badge must be displayed upon request. The badge allows movement to and from work sites only. The badge does not authorize entry/access to restricted areas.
The requirements of the weight/load testing are located at but not limited to the following 433rd
Support Equipment/MXG Buildings/Hangars 814, 826, 826 South, 825, 894, 898, 876 and the
Crash Damage and Aircraft Recovery (CDAR) building. See section 7.1 Load Testing Equipment list for location of equipment that require testing. The fall protection I-Beams are in building 826
South and cannot be moved, therefore, inspection would be performed in that building for that piece of equipment. Any equipment stored in the CDAR building will be brought by the customer to Bldg. 814 for testing and picked up upon completion of testing. Most inspections will take place in Bldg. 814 either on equipment already located in that facility or other equipment that needs to be brought in by the customer. For any items that are too long or large to test in Bldg.
814 will be brought to the area between the Liquid Oxygen Pad (LOXPAD) and Bldg. 814 to be tested using a crane. The facilities will be occupied while the work required by this contract is performed. Most of the work will be accomplished outside of the buildings. The contractor shall provide safety barriers and signs to prevent unauthorized personnel from entering the work area while the Contractor is performing Weight/Load Testing. Contractors Vendors who need access into a restricted area must be escorted in accordance with AFI 31-101, The Air Force Installation
Security Program. JBSA units or agencies requesting access for contractors or vendors (i.e., 502
CONS, 502 CBS, 502 CS, 502 LRS, 502 MDG, 433 MXG, AAFES, AF Exchange, DECA, U.S.
Army Corps of Engineers, etc.) must first submit a contractor form 74, Application for Personnel to Work Under Department of Defense Contract to Building 5725. The contractor form 74 may be completed by the contractor or vendor; however, it will only be accepted by either VCCs from the sponsoring agency. Sponsoring agency is a DoD or military approving agency, to include units listed above that can verify a need for installation access for the purpose of a job/contract.
Contractors who are non-US citizens must also submit an INS Form I-9 along with other original supporting documents. Blank contractor form 74s will be supplied by the sponsoring agency and available at the VCCs. If the contractor or vendor completes the contractor form 74, the original copy should be given to the sponsoring agency. Contract security manager will receive and file the original copy at the time appointment is made at the VCC to receive a contractor’s badge. The
CO or contract administrator will verify that the contract number is correct, and that the contractor is working on that contract, all other blocks are completed, and the form is clearly’legible. The original contractor form 74 will then be hand carried by authorized personnel between the base sponsor and the VCC, where the form will be checked for completeness, legibility, and all required signatures prior to acceptance. Processing the contractor form 74 and a background check could take up to 5 duty days from the time it is submitted to the VCC. When the contractor form
74 is accepted at the VCC, an automated PACS pass or AF Form 75, Visitor Pass will be issued until the badging appointment date/time, not to exceed a maximum of 5-days, or over a weekend
(see paragraph 18for weekend work requirements).
Badges will not be issued to personnel waiting processing of the contractor form 74. If your
JBSA Lackland contract is over thirty (30) days, you may be issued a contractor badge. If the
JBSA Lackland contract is less than 30 days, or the need for JBSA Lackland access is less than
4-days per week, 8-hours per day, the completed contractor form 74 will be processed as a
“Temporary.” The JBSA Lackland sponsor (Military/Civilian CAC card holder) will need to be present at the VCC each time an individual is issued an automated AF Form 75, Visitor pass for
JBSA Lackland access. It is the Contract Foreman’s responsibility to ensure that all contractor badges and other forms of identification issued by JBSA Lackland are returned to Security
Forces in a timely manner after completion of the contract or if an employee is terminated from their job. Any items issued by JBSA Lackland should be returned in person (preferred method) or by mail to 502 SFS, 2602 Luke Blvd., Lackland AFB 78236.
5.2.5 Contractor Badges:
JBSA Lackland Contractors are issued Passes or PACS cards. Each contractor badge allows the holder different privileges that are defined below. Direct all questions regarding contractor badges to 502nd SFS at (210) 671-2575. NOTE: Contractors/Vendors do not have escort authority for anyone unless indicated on their badge, and they may not conduct interviews about base contracts or events without first discussing the issue with the 433 AW Public Affairs office at (210) 925-8338. All media or press requests must be approved by Public Affairs before media are granted entry onto JBSA Lackland.
5.2.6 Base Speed Limits:
Speed limits on JBSA are strictly enforced. Speed limits may change due to inclement weather i.e., snow or ice on roads.
30 MPH unless otherwise posted.
10 MPH in parking lots.
15 MPH on the flight-line, unless otherwise posted.
15 MPH in the housing area.
5.2.7 Emergency Vehicles:
Vehicles with "Alert Force" signs and yellow lights on top, as well as ambulances, security forces, fire department, civil engineering vehicles have the right of way when their lights are flashing, and horns or sirens are sounding. You are required to come to a complete stop on the right-hand side of the roadway when approached by an emergency vehicle.
5.2.8 Items Prohibited:
Items prohibited in the state of Texas are also prohibited on JBSA Lackland. Prohibited items are subject to confiscation. Administrative or judicial action, to include base debarment, may be initiated against any individual possessing a prohibited item on JBSA Lackland. If in doubt, don't bring the item on base or contact security forces. Examples of prohibited items are but not limited to: All Forms of illegal drugs, firearms; shotguns, handguns, machine guns, firearm silencers, ammunition of any type, blasting caps or similar items of explosive material; starter or flare pistols, pellet or BB guns, dynamite, tear gas and their dispensing devices, and other similar devices. Construction tools that require use of explosive materials are authorized if they are required to perform your duties. Contractors who require one of the prohibited items listed above may bring the item onto the installation in performance of the contract (i.e., bird control, wildlife management, etc.), but only when specified in contract. Knives with blades more than four inches, switch blade knives, underwater spear guns, bows and arrows, crossbows, slingshots, and blowguns, throw blade knives, blackjacks, nightsticks, clubs, brass knuckles, tomahawks, swords, bolas, fighting sticks, throwing discs, chains, and other similar items that can be used as a weapon.
5.2.9 Base Exercises, and Retreat:
JBSA Lackland has both a wartime and peacetime mission. To ensure we can perform both missions effectively, frequent exercises are conducted. Entry to or exit from the base may be delayed due to gate closures. Closures are usually for short periods of time. Should you or your employees get stopped at a cordon (exercise perimeter) or get told to evacuate your work area, you may identify yourself to the cordon guard as a contractor and state the reason you need to remain in the cordon area. The guard will pass the information up the chain of command and ask for an exemption to the evacuation requirement. Be aware this may take a few minutes and that it is not the guard's fault. If the exemption is approved, only company or properly marked vehicles will be allowed to cross or remain in the exercise cordon area. Remember, you are required to follow the final decision of the guard and will not attempt to persuade him/her to allow you to enter the area if authorization is not received. If the exercise causes delays or other problems with your contract performance, immediately notify the contract administrator or sponsoring agency.
Retreat: To show respect for the flag of the United States, all personnel driving a vehicle shall immediately pull to the right side of the roadway or traffic lane and stop while retreat (National
Anthem) is sounded. Once the last note has sounded, vehicles may be placed back in motion.
5.2.10 Contractor's Security Requirements:
Although JBSA Lackland has stringent entry and internal controls, we still experience incidents of vandalism and theft. Your equipment located within the boundaries of JBSA Lackland will be provided the normal protection you would expect from a civilian police agency. As in the civilian community, you are expected to provide a reasonable degree of protection (security) for your property. The government accepts no responsibility for lost or stolen materials, equipment, or tools, regardless of the item(s) location. The security of your property when it is contained within a restricted area is secondary to the protection afforded to the JBSA Lackland resources. Should you be a victim or witness to a crime or incident, please report it immediately to the Base Defense Operations Center (BDOC).
5.2.11 Flight-line Operations (JBSA Lackland):
If your job site is on or near the flight-line, the Airfield Manager, (210) 925-5880, maintains all instructions for flight-line operations. If the job site is near restricted areas, an escort may be required. Also, procedures for job sites on or near the flight-line may change dependent upon the current security level at the base.
5.2.12 Restricted Areas (JBSA Lackland):
JBSA Lackland has several areas containing high value or sensitive resources. These areas are posted with "Restricted Area" signs. In addition to signs, the areas are marked with red or orange ropes, stanchions, or red painted lines. Contract personnel may not enter these areas unless they are under escort by an authorized individual. If your contract involves work within a restricted area, the contract administrator or sponsoring unit and the airfield manager will coordinate actions with security forces to establish a "free zone." A "free zone" will permit contractors entry, free movement, and exit from the job site. See paragraph 17 for free zone procedures.
5.2.13 Entry Requirements inside Restricted Areas (JBSA Lackland):
Contractors will not be permitted to enter Restricted Areas.
5.2.14 Photograph Equipment:
Cameras, camcorders, etc., are not normally permitted on JBSA Lackland unless vetted and approved through Public Affairs.
5.3 Quality
This section describes the Quality Control components for this effort. The following sub-sections provide details of various considerations on this effort.
5.4 Quality Control Plan (QCP)
The contractor must ensure that all equipment returns to the COR 100% free of foreign objects
(FO). FO is designated as anything that is not a part of the equipment itself, (i.e., additional hardware, residue or scraps leftover from maintenance performed, etc.). The contractor shall employ a commercial quality control program and procedures to identify, prevent, and ensure non-reoccurrence of defective services. Through implementation of the contractor’s quality control program and procedures, the United States Air Force shall receive quality services meeting the requirements of the contract. The Contractor shall develop a QCP and maintain an effective commercial quality control program to ensure services are performed in accordance with this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The Contractors QCP is how he assures himself that his work complies with the requirement of the contract. The finalized QCP will be made available to the contracting officer upon request.
5.5 Identify Hazardous Materials:
In accordance with FAR 52.223-3 (Section I), all hazardous materials and quantities to be used during this project will be identified as part of the contractor’s proposal. After contract award, the contractor will provide material safety data sheets (SDS) to the CO as a submittal for approval before the Notice to Proceed is issued. Upon contract completion, the contractor will provide the CO with the quantities of hazardous materials used during contract performance. The prime contractor is responsible to ensure all sub-contractors employed during the contract comply with these requirements.
5.6 Disposal of Hazardous Waste:
Disposal of all hazardous waste is to be coordinated through 502nd CES/CEV, at (210) 671-
3658. The contractor will provide copies, for approval by CEV, of permits for transporters and treatment, storage, disposal facilities the contractor intends to use, or as an alternative, the contractor will utilize only those transporters/facilities listed on the Joint Base San Antonio-
Lackland AFB list of approved transporters/facilities. The contractor will provide all testing/analysis data and waste profiles to CEV for review and signature, SC Authorization
Request Forms (when applicable) to CEV for review and signature, and the contractor shall coordinate with CEV to have authorized CEV personnel sign the hazardous waste manifest for shipment of hazardous wastes.
5.7 Quality Assurance Surveillance Plan (QASP)
The Government shall monitor the Contractors performance under this Purchase Order in accordance with the Governments QASP.
