Disclaimer: Due to the nature of the EPA 600 method, asbestos may not be detected in samples containing low levels of asbestos. We strongly recommended that analysis of floor tiles, vermiculite, and/or heterogeneous soil samples be conducted by TEM for confirmation of “None Detected” by PLM. This report relates only to the samples tested and may not be reproduced, except in full, without the written approval of SAI. This report may not be used by the client to claim product endorsement by NVLAP or any other agency of the U.S. government. Estimated MDL is 0.1%.

Facility Summary Report:
09/17/2013 - Work Order 11316 - Renovate Northend Upstairs and Middle Section, - The following materials are positive for asbestos containing materials. In the cardio room, black mastic at 5% Chrysotile on the east. All mirror mastic must be assumed positive for Asbestos until proven otherwise through lab analysis. The area is inaccessible until the mirror is removed from the wall.
09/17/2013 - Work Order 11316 - Hazard Abatement survey entire hangar - The following materials were found to contain asbestos containing materials above the 1%. Through out the hangar, there is a black tar like mastic applied to the concrete floor. This material may have been used to glue the wooden gym floor which has since been removed. This material contains 8% chrysotile All of the Window Glazing throughout the hangar is positive for Asbestos Containing materials. The window glazing is 3% chrysotile
This inspection did not include any attic areas. There may be asbestos containing materials in this particular area. Most materials are fiberglass insulation. The lean-to area of this hangar has recently been renovated and all asbestos materials were removed.
All light fixtures must be checked for PCB Ballasts. If ballasts are not marked “NO PCBs” then ballasts must be handled as PCB Waste, or recycled at an approve EPA PCB Recycling Facility . PCB’s must be packaged IAW DOT regulations and Manifested. Recycling Certificate Required if Recycled.
HID Lamps are known to contain Mercury in small amounts. All lamps must be removed from fixture packaged IAW DOT regulations and manifested to an approved EPA recycling facility or handled as Universal Waste in the State of Texas.
06/19/2013 - Work Order 11291 - Demolish Rooms at South end of Hangar - All Wall mastic and Mirror Mastic is Positive for Asbestos Containing Material. The wall mastic is mixed in with the black mirror mastic. Since the materials are mixed, both materials are Positive and must be abated by a Certified Asbestos Abatement team. All other suspect materials such as wall, texture, Float Mud Joint Compounds, Sheetrock, Ceiling Tiles and Floor underlayment felt and Sealer material are NAD, No Asbestos Detected. This assessment does not include any hidden inaccessible areas such as materials behind walls. If there are materials found behind the existing walls, they must be sampled for asbestos or assumed positive before demolishing.
The fixtures in the room must be inspected for PCB Ballasts. If the Ballasts are not marked "No PCBs", then the ballasts must be handles as PCB Waste. All fluorescent light bulbs regardless of tip color must be either recycled or disposed of as Universal Waste. All manifests for PCBs, Asbestos and Light Bulbs containing mercury, will be manifested and only signed by 902 CES/CEAN authorized personnel as the certifiers of waste.
In between the aerobic room, there is vibration Cloth that has been torn and left protruding from the duct. This is assumed asbestos and must be abated by a certified asbestos abatement team.
07/15/2011 TYMX111056_Renovate North End of Hangar - All Sheetrock, Joint Compound, Ceiling Lay in tile and Window
