SECTION 1- SUMMARY OF WORK

1.1 DESCRIPTION OF WORK

1.1.1 The Contractor shall provide all management, labor, material, equipment, transportation, supervision, and minor designs to accomplish simultaneous steam and condensate repairs and other miscellaneous repairs and minor construction work on real property at Hanscom Air Force Base (HAFB), MA.

1.1.2 The Contractor shall accomplish all work in accordance with (IAW) the technical specifications and standard construction details provided by the Government as part of this solicitation package. The Contractor shall execute the contract on a delivery order by delivery order basis. The technical specifications will follow the United Facilities Guide Specification (UFGS) format.

1.1.3 Work shall include but not be limited to the following:

1.1.3.1 Steam and Condensate Pipe in Trench – Replacing and reinsulating steam piping, replacing jacketing, replacing condensate piping and associated components. Replacement of ten percent (10%) of the steam pipe.

1.1.3.2 Direct Buried Steam and Condensate Distribution – Replacing existing underground steam condensate piping with direct burial Class A pre-engineered underground conduit system in its entirety as specified in SECTION 33 61 13 –

PRE- ENGINEERED UNDERGROUND HEAT DISTRIBUTION SYSTEM.

1.1.3.3 Junction Box – Replacing of condensate collection systems in manholes.

Replacement of components in manholes including flash tanks, traps, valves, strainers, radiator vents, and steam pumps.

1.1.3.4 Trenches – Providing new cast in place footings, slabs, and covers for new manhole foundation walls, sidewalks, and other miscellaneous pads.

Providing thrust blocks for piping as necessary. Installing new roller supports, guides, and hanger rods. Reuse of existing support angle and beams that are in good condition. Cleaning and preparing for painting with a minimum one (1) coat of galvanized paint. Removing and storing of trench covers. Resealing all trench cover joints. Inspecting trench covers for failures. Installation of twenty-five percent (25%) of new pipe support angle/beams and ten percent (10%) of the trench covers.

1.1.3.5 Site Work – Removing and disposing or stockpiling of old and/or installing new or salvaged concrete or granite curbing. Resurfacing, full-depth reconstruction and new full-depth construction of bituminous concrete pavements to include but not limited to roadways, parking lots, and sidewalks. General landscaping to include installing topsoil, surveying, seeding, sodding, mulch, and specified plantings.

1.1.3.6 Water Lines – Installing, repairing, and replacing water lines, equipment and appurtenances, to include work on water mains and building service lines up to and including building entrances into mechanical rooms associated with failed or outdated components located in roadways, walks, and parking and landscape areas. Installing ductile iron and copper pipe associated with failed or outdated components located in roadways, walks, and parking and landscape areas.

Installing, repairing, or replacing turf sprinkler systems.

1.1.3.7 Sanitary Sewer – Installing, repairing, and replacing existing sanitary sewer systems, equipment and appurtenances, to include work on sewer mains and building service laterals up to and including building service drops and connections to existing lines and manholes associated with failed or outdated components located in roadways, walks, and parking and landscape areas.

Installing PVC sewer pipe associated with failed or outdated components located in roadways, walks, and parking and landscape areas. Cleaning and inspecting of existing sewer lines.

1.1.3.8 Storm Drainage – Installing, repairing, and replacing existing storm drain systems to include catch basins, manholes, trench drains, culverts, drainage swales, and detention/groundwater recharge and treatment systems associated with failed or outdated components located in roadways, walks, and parking and landscape areas. Installing reinforced concrete, HDPE, and PVC drain pipe.

Cleaning of existing storm drainage systems.

1.1.3.9 Electric and Communication – Installing new conduits and wiring for powering manhole sump pumps and associated controls and alarms. Relocating and replacing existing conduits that interfere with the installation of the steam and condensate piping.

1.1.3.10 Mechanical Rooms – Replacing valves, pumps, piping, traps, strainers, tanks and insulation in mechanical rooms. Installing water treatment equipment.

1.1.3.11 Temporary Heat – Providing continuous steam supply and condensate recovery to and from affected facilities via bypass piping, temporary boiler(s) or temporary steam and condensate piping.

1.1.3.12 Miscellaneous – Other miscellaneous work items including but not limited to the following: pavement markings, signage, and guardrails. The principal items of work listed will in no way limit the responsibility of the Contractor to perform all items of construction specified in the Statement of Work (SOW), on the drawings, or in the Technical Specifications.

1.1.4 The Contracting Officer (CO) will issue a request for proposal (RFP) describing basic requirements for each individual delivery order (DO) on an as needed basis.

Following review of the Contractor’s DO proposal, the CO will negotiate with the Contractor and award an individual DO. The Government will require prompt response and execution of DOs.

1.2 WORK BY THE GOVERNMENT

1.2.1 The Government will not be obligated to issue any particular type of work under this contract and will reserve the right to accomplish work using Government workforces or by other contracts, as the Government deems necessary or desirable, and to do so will not breach or otherwise violate this contract.

1.3 COMMENCEMENT OF WORK

1.3.1 The Contractor shall negotiate and execute DOs within fifteen (15) calendar days from the time of contract award.

1.3.2 The Contractor shall have thirty (30) calendar days from the time of award to mobilize and be fully operational.

1.3.3 Upon notification from the CO of a requirement, the Contractor shall prepare and submit a DO proposal to the CO within fourteen (14) calendar days from the date of the site visit. If the CO determines the requirement to be a quick response, the Contractor shall submit the DO proposal to the CO within seven (7) calendar days.

For more complex projects, the CO will grant additional time.

1.3.4 The Contractor shall provide a two (2) calendar day turnaround for DOs being negotiated during the final month of the Government Fiscal Year. The Contractor shall provide a same day turnaround for DOs being negotiated during the last day of the Government Fiscal Year.

1.4 CONTRACTOR PERSONNEL

1.4.1 The Contractor shall employ a sufficient number of qualified personnel to adequately oversee all work under this contract. When work is being performed, the assigned superintendent or an appointed alternate shall be available at any work site within sixty (60) minutes of notification. All key management staff shall be fluent in English.

1.4.2 The Contractor shall submit a list of qualified personnel for Government approval at contract award. In the event of a personnel change, the Contractor shall submit a revised listing of qualified personnel for Government approval.

1.4.3 Contractor’s Contract Manager (CM): The Contract Manager shall be responsible for the overall performance of this contract. The contract manager shall have a minimum of ten (10) years of documented applicable construction experience with a minimum of at least five (5) years being in a relevant construction management position. The name of this person and alternate(s), who shall act on behalf of the CM when absent, shall be designated in writing and submitted to the CO for approval. The CM will receive each RFP from the CO.

1.4.4 Contractor’s Project Manager (PM): The Contractor shall provide a Project Manager that is dedicated to this contract, and has decision-making, obligation, negotiation, and signature authority pursuant to this contract. The PM shall notify the Contracting Officer (CO) of any projected absence over two (2) days and designate a qualified replacement.

1.4.5 Contractor’s Project Superintendent (PS): The Project Superintendent shall provide on-site supervision. The PS shall visit each active project site daily, submit detailed Construction Progress Reports (AF Form 3065) for each DO, schedule and coordinate subcontractors and material suppliers and attend all status meetings, site visits and pre-final/final inspections. The PS can be the CM, but cannot be the Quality Control Manager or the Safety Manager. Subcontractors shall not perform duties of the PS. The Contractor shall increase the number of PSs as necessary to ensure adequate daily supervision for all active projects.

1.4.6 Contractor’s Quality Control Manager (QCM): The Quality Control Manager shall provide quality control management for all active DOs and executes the QC Plan.

The QCM shall visit active project sites and verify that the materials and workmanship are in accordance with approved construction drawings, material submittals and technical specifications. The QCM shall prepare and coordinate material submittal sheets and shop drawing submittals, prepare QC reports, schedule and coordinate testing procedures and attend all status meetings, site visits and pre-final/final inspections. The QCM shall have complete authority to take any action necessary to ensure compliance with the contract requirements. The QCM shall be someone other than the Contract Manager or Project Superintendent. The QCM shall have a minimum of five (5) years of relevant construction quality control experience and shall have completed a course in Construction Quality Management for Contractors. The name of the QCM and alternate(s), who shall act on behalf of the QCM when absent, shall be designated in writing and submitted to the CO for approval.

1.4.7 Contractor’s Safety Manager (SM): The Safety Manager shall ensure full compliance with all safety requirements on all active projects. The SM cannot be the Project Superintendent. The SM shall meet the requirements of EM 385-1-1 section 1.

The SM’s training, experience and qualifications shall be required as per EM 385-1-1 paragraph 01.A.17, entitled SITE SAFETY AND HEALTH OFFICER (SSHO), and associated subparagraphs.

1.5 CONTRACTOR EMPLOYEE REQUIREMENTS

1.5.1 The Contractor shall ensure that the employees and subcontractor employees have current, valid licenses and certifications necessary to perform their functions.

Any cost or time associated with obtaining licenses or certifications shall be at the Contractor’s expense.

1.6 CONFLICT OF INTEREST

1.6.1 The Contractor shall not employ any person who is nor was an employee of the United States Government, if the CO determines that the employment of that person would create a conflict of interest.

1.7 SUBCONTRACTORS

1.7.1 All subcontractor firms shall be licensed in the State of Massachusetts in their specialty where required. They shall have a minimum of two (2) years’ experience in related projects of similar size, scope and complexity.

1.8 AVAILABILITY

1.8.1 Throughout the life of this contract, whenever there are active DOs, the CM shall be available to meet with the CO promptly after notification by telephone during normal business hours. The CM shall be available to meet with the CO within twenty-four (24) hours when there are no active DOs. The CO must be able to contact the CM by phone at all times. The CM shall carry a cellular phone for immediate response to contract requirements.

1.8.2 The CM shall provide overall contract management, including full authority to develop DO proposals, negotiate and sign DOs, provide subcontract purchasing and administration, review material submittals and shop drawings, and shall attend all weekly status meetings, site visits and pre/final inspections. The Contractor shall notify the CO and the Government Project Manager (GPM) when the CM plans to be absent from the site for more than two (2) consecutive work days. Prior to returning to the project, the CM shall notify the CO and the GPM.

1.9 RESPONSE TIMES

1.9.1 The Contractor shall resolve punch-list items on individual delivery orders within ten (10) calendar days of notification by the Government unless more timely completion is noted elsewhere in the contract requirements.

END OF SECTION

SECTION 2 – GENERAL REQUIREMENTS

2.1 MANAGEMENT PLAN

2.1.1 The Contractor shall include in his response to the initial RFP a detailed Management Plan. The Management Plan shall include the following:

2.1.1.1 Briefly describe the home office organization, responsibilities and lines of authority established for the contract as well as the resources available and your capabilities to support this contract concurrently with other on-going or new work, including your capabilities to manage and execute concurrent delivery orders.

Include an organizational chart detailing the organization described in your proposal. Include names of assigned personnel/subcontractors and their respective areas of responsibility. Clearly delineate on-site from off-site personnel.

The Contractor shall include Quality Control and Safety in the chart.

2.2 CODES

2.2.1 All construction accomplished pursuant to this contract shall comply with all applicable federal, state, and local laws and codes including but not limited to the following (any exception must be approved in writing by the CO prior to construction start-up):

2.2.1.1 Instructions, manuals, handbooks, regulations, guidance, policy letters, and rules (including all changes and amendments);

2.2.1.2 Executive Orders (EOs);

2.2.1.3 Air Force Engineering Technical Letters (AFETLs);

2.2.1.4 Air Force/DOD instructions, manuals, handbooks, regulations;

2.2.1.5 Unified Facilities Criteria (UFCs);

2.2.1.6 American Society of Mechanical Engineers (ASME);

2.2.1.7 American National Standards Institute (ANSI);

2.2.1.8 National Association of Corrosion Engineers (NACE);

2.2.1.9 National Plumbing Code (NPC);

2.2.1.10 National Fire Protection Association (NFPA);

2.2.1.11 National Electrical Code (NEC);

2.2.1.12 International Building Code (IBC);

2.2.1.13 Commonwealth of Massachusetts State Building Code;

2.2.1.14 Commonwealth of Massachusetts General Law;

2.2.1.15 Massachusetts Department of Transportation (MassDOT) Standard Specification for Highways and Bridges (unless the CO directs the Contractor otherwise);

2.2.1.16 Life Safety Code;

2.2.1.17 Environmental Protection Agency (EPA) regulations;

2.2.1.18 National Environmental Policy Act (NEPA);

2.2.1.19 Commonwealth of Massachusetts Department of Environmental Protection Regulations (DEP);

2.2.1.20 Conservation Commission requirements for the local towns having jurisdiction of the work area;

2.2.1.21 Air Force, AFMC, and DoD Environmental Policy;

2.2.1.22 Americans with Disabilities Act (ADA) Accessibility Guidelines; and

2.2.1.23 HAFB specific design requirements.

2.2.2 The Contractor shall comply with procedures as identified in the latest version of the Steam and Condensate IDIQ Technical Specifications. The Contractor shall utilize the applicable specification sections in generating his contract proposal.

2.3 ORDER OF PRECEDENCE

2.3.1 Requirements included in the Statement of Work (SOW) shall take precedence over the contract specifications, drawings, sketches and other information provided with each delivery order. For conflicts between requirements included in the SOW, the most stringent requirements shall apply, unless specifically requested in writing by the Government. Likewise, for conflicts between requirements outside the SOW, the most stringent requirements shall apply, unless specifically requested in writing by the Government.

2.4 MANUFACTURER’S INSTRUCTIONS

2.4.1 Manufacturer’s instructions pertaining to the use or installation of products, materials, or equipment used or installed in the execution of work under this contract form a part of these specifications as though specifically set forth therein. These instructions apply whether furnished as a normal, usual, or customary practice of the manufacturer or if furnished in response to a requirement stipulated within. In the event of conflict between the specification of drawings and manufacturer’s instructions, the Contractor shall bring such conflict to the attention of the CO for resolution before proceeding with the work involved.

2.5 QUALITY CONTROL (QC)

2.5.1 The Contractor shall provide, inspect, and document the level of quality that has been established by the minimum standards of this SOW and ensure compliance with the Contract Clause entitled “Inspection of Construction” and other contract requirements. The Contractor shall be responsible for quality control and shall establish and maintain an effective quality control system consisting of plans, procedures, personnel, control, instructions, tests, records, forms and organization necessary to manage all delivery orders to produce end products which comply with the contract requirements. The system shall cover all construction operations, both on-site and off-site, including work by subcontractors, fabricators, suppliers and purchasing agents.

2.5.2 The QCM shall direct the execution of the Contractor’s QC Plan.

2.5.3 The Government will reserve the right to require the Contractor to make changes in his QC Plan and operations including removal of personnel, as necessary to obtain the quality specified.

2.6 QC PLAN

2.6.1 The Contractor shall submit a QC Plan to the CO within thirty (30) calendar days of award of the contract. The Contractor shall submit any changes to the accepted QC Plan to the Government in writing for approval by the CO. The QC Plan shall include, but not limited to the following:

2.6.1.1 Description and details of the QC organization, including an organizational chart showing clear lines of authority;

2.6.1.2 The name, qualifications (in resume format), duties, responsibilities, and authorities of each person assigned a QC function;

2.6.1.3 A copy of the letter to the QC Manager signed by an authorized official of the firm describing their responsibilities and delegating sufficient authorities to adequately perform the functions of the QC Manager, including authority to stop work not in compliance with the contract.

2.6.1.4 Copies of letters issued by the QC Manager to other various quality control representatives outlining duties, authorities, and responsibilities;

2.6.1.5 Procedures for scheduling, reviewing, certifying, and managing submittals, including those of subcontractors, off-site fabricators, suppliers, and purchasing agents;

2.6.1.6 Description and details of the Contractor’s inspection system to monitor the execution of any task completed under this contract. Description of specific areas to be inspected on a scheduled or unscheduled basis, how often inspections shall be accomplished, and titles of the individuals who shall perform the inspections. Procedures for tracking preparatory, initial, and follow-up control phases and control, verification, and acceptance tests including documentation;

2.6.1.7 Description and details of procedures for tracking construction deficiencies and their causes from identification through acceptable corrective action. These procedures shall establish verification that identified deficiencies have been corrected;

2.6.1.8 Reporting procedures, including proposed reporting formats.

2.7 QC COORDINATION MEETING

2.7.1 After the Pre-Construction Conference, before issuance of any delivery order under this contract, and prior to acceptance by the Government of the QC Plan, the Contractor shall meet with the CO or authorized Government Representative and discuss the Contractor’s Quality Control System. The Contractor shall submit the QC Plan for review a minimum of five (5) calendar days prior to the Coordination Meeting.

During the meeting, a mutual understanding of the system details shall be developed, including the forms for recording the QC operations, control activities, testing, administration of the system for both on-site and off-site work, and the interrelationship of Contractor’s management and control with the Government’s quality assurance.

The Contractor shall prepare meeting minutes and signed by both the Contractor and the CO. The minutes shall become a part of the contract file.

2.7.2 Government acceptance of the Contractor’s QC Plan will be required prior to the award of any delivery order. Construction will be permitted to begin only after acceptance of the QC Plan or acceptance of an interim plan applicable to the particular feature of work to be started. Work outside of the features of work included in an accepted interim plan will not be permitted to begin until acceptance of the QC Plan or another interim plan containing the additional features of work to be started.

2.7.3 Prior to submittal, the Contractor’s Quality Control (QC) representative shall check and approve all items, and sign and date each item indication action taken. If the delivery order requires shop drawings, each of the Contractor’s appropriate reviewing officials shall sign or initial the drawings as evidence of review. The CO may refuse any shop drawings submitted without evidence of the Contractor’s review, coordination, and approval. The CO’s approval shall not relieve the Contractor from responsibility for any errors or omissions in such drawings, nor from responsibility for complying with the requirements of this contract.

2.8 DOCUMENTATION

2.8.1 The Contractor shall maintain, on-site, current records providing documented evidence that the required quality control inspections, tests and other activities have been conducted by the Contractor and the necessary corrective actions taken. These records shall include the work of subcontractors and suppliers. Documentation shall be in a format acceptable to the CO and made available to the Government throughout the performance period of this contract.

2.9 DAILY REPORTS

2.9.1 The Contractor shall develop and maintain daily reports for each DO, each daily report indicating the following:

2.9.1.1 Contractor and subcontractors working on the project, their area of responsibility and their number of personnel working;

2.9.1.2 Operating plant/equipment with hours worked, idle, or down for repair;

2.9.1.3 Work performed each day, giving location, description, and by whom;

2.9.1.4 Test and/or control activities performed with results and references to specifications/drawings requirements. List deficiencies noted along with corrective action;

2.9.1.5 Quantity of materials received at the site with statement as to acceptability, storage, and reference to specifications/drawings requirements;

2.9.1.6 Submittals reviewed, with contract reference, by whom, and action taken;

2.9.1.7 Off-site surveillance activities, including actions taken;

2.9.1.8 Job safety evaluations stating what was checked, results, and instructions or corrective actions;

2.9.1.9 Instructions given/received and conflicts in plans and/or specifications;

2.9.1.10 Weather conditions encountered;

2.9.1.11 Any delays encountered; and

2.9.1.12 Contractor’s verification statement that equipment and materials incorporated in the work and workmanship comply with the contract.

2.9.2 The Contractor shall submit one (1) copy of the daily report for each DO to the Government within twenty-four (24) hours. Records shall be signed and dated by the QCM. Reports shall include copies of test reports and copies of reports prepared by subordinate QC personnel.

2.10 CONTRACTOR USE OF PREMISES

2.10.1 Contractor Storage Location:

2.10.1.1 The Government will provide, if required, an area to stage materials (not necessarily at the construction site), store equipment and tools, and maintain miscellaneous items. The Contractor shall secure this area. Any losses to the contained equipment/materials will not be the fault of the Government.

2.10.1.2 The Contractor shall maintain the contractor area in a neat and orderly appearance. The CO will instruct the Contractor to clean the storage area if it is deemed necessary. The Contractor shall respond within two (2) hours of being notified by the CO, and immediately clean the area. This cleanup shall be considered fulfillment of the basic contract and charges shall not be applied back to the Government.

2.10.1.3 The Contractor shall post a notice of who to contact in case of emergency on all storage trailers and buildings.

2.10.1.4 The Contractor shall cover stored material and protect against physical and climatic damage or loss.

2.10.1.5 The Contractor shall not leave equipment or materials on elevated locations without prior approval.

2.10.1.6 The Contractor shall store equipment removed in performance of work where directed by the CE Steam & Condensate (S&C) IDIQ Inspector, and reinstall in work as required by drawings and specifications. Equipment temporarily removed shall be protected, cleaned and replaced equal to or better than its condition prior to starting work.

2.10.1.7 The Contractor shall not store construction debris in the outside storage area or hold in this area for resale.

2.10.2 General Operation Requirements:

2.10.2.1 The Contractor shall limit use of the premises to construction activities in areas indicated; allow for Government occupancy and access.

2.10.2.2 The Contractor shall confine operations to areas within contract limits indicated. Portions of the site beyond areas in which construction operations are indicated are not to be disturbed.

2.10.2.3 The Contractor shall keep driveways and entrances serving the premises clear and available to the Government at all times. The Contractor shall not use these areas for parking or storage of materials. The Contractor shall schedule deliveries to minimize space and time requirements for storage of materials and equipment on site.

2.10.2.4 The Contractor shall sweep job site and haul routes daily (or more frequently as required to by the CO) as required to remove all spillage, mud, and soil tracked from work site or dumpsite. The Contractor shall cover all loads subject to spillage such as earth fill, rubble, trash, materials taken to off base land fill, etc., during hauling to prevent spillage. The Contractor shall sweep spills immediately. The Contractor shall coordinate haul routes with the CE S&C IDIQ Inspector to minimize impact to the base.

2.10.2.5 The Contractor shall perform the work so as not to interfere with Government operations.

2.10.2.6 The Contractor shall protect all Government property within the work site, except for property required to be demolished. The Contractor shall protect property that is to be demolished until its scheduled demolition time. Protection shall include, but not be limited to, protection from construction generated dust, debris, water, freeze damage, and vibration.

2.10.2.7 The Contractor shall maintain and repair pavements, drives, or turf areas utilized during construction for access roads or storage areas, to the satisfaction of the CO. Cost associated with the above work shall be incidental to the contract.

Where landscape rock areas are repaired or installed, the Contractor shall provide commercial grade, spun weed barrier under rock to prevent weed growth. The Contractor shall make sure landscape rock is free from dirt and debris, and repair all wheel ruts, etc.

2.10.2.8 The Contractor shall maintain lawns, landscaping, trees, shrubs, etc.

within the work area. The Contractor shall repair any lawn areas damaged as a result of construction to the satisfaction of the CO. If possible, the Contractor shall keep vehicles off grass areas. The Contractor shall protect areas under and around trees to prevent damage to tree roots. As appropriate, the Contractor shall rope off or place barriers around any trees and shrubs to protect from construction damage. The Contractor shall place the tree barrier at the drip line of the tree. The Contractor shall ensure vehicles, construction materials, etc. are kept a safe distance from trees to ensure minimal damage from soil compaction.

2.10.2.9 All utility crossings of streets, sidewalks, curbs, or gutters shall be by trenchless boring unless specified otherwise. When pavement road cuts are permitted, only half of the road shall be closed at any time. The road closures shall be coordinated in advance and the road re-opened as soon as possible to traffic.

The Contractor shall complete final asphalt patching within seven (7) days of initial cutting, weather permitting.

2.11 TRAFFIC AND WORK COORDINATION

2.11.1 The Contractor shall coordinate all work phases with the Government Representative. The Contractor shall submit a detailed time schedule within five (5) calendar days after receipt of a written Notice to Proceed (NTP). The Contractor shall perform work in an orderly manner with minimum disturbance and inconvenience to both vehicular and pedestrian traffic. The Contractor shall maintain access to fire protection at all times. The Contractor shall coordinate and provide traffic control for any temporary altered traffic patterns. The Government will not provide personnel, signage, equipment, or logistical assistance for traffic control.

2.11.2 Within five (5) calendar days of each Delivery Order NTP, the Contractor shall submit a schedule of anticipated requirements for road closures and traffic impacts.

This shall include partial road closures and parking area impacts. This schedule shall be separate from any other required construction schedule. The Contractor shall list each request with the following:

2.11.2.1 Anticipated date;

2.11.2.2 Approximate duration;

2.11.2.3 List of roads, facilities, and affected parking areas;

2.11.2.4 Reason for the shutdown (work to be accomplished during the shutdown); and

2.11.2.5 Traffic Control Plan.

2.11.3 The Contractor shall submit a specific request for each road closure or traffic impact twenty-one (21) calendar days prior to each required shutdown. This request shall include the information identified above plus the shutdown requested time and duration.

2.11.4 The Government will not permit any traffic impacts at the Hartwell and Vandenberg Gates during peak traffic times (Peak AM traffic: 0700 to 0830 and Peak PM traffic: 1530 to 1700).

2.12 WORK HOURS

2.12.1 Government work hours will ordinarily be from 7:30 AM to 4:15 PM, Monday through Friday. The Contractor shall not commence work in housing areas before 8:30 AM and shall terminate work by 4:30 PM. Access to work sites will be restricted to these hours and days. Work that requires inspection during accomplishment or that prevents inspection of previous work after accomplishment normally will not be allowed during other than the normal work hours specified above.

2.12.2 The Contractor shall submit a written request to work other than normal hours seventy-two (72) hours in advance for Government Representative approval. The Contractor shall vary these hours when approved by the CO to meet weather and temperature conditions and operational requirements.

2.13 JOB SITE VERIFICATION/GOVERNMENT FURNISHED RECORD DRAWINGS

2.13.1 Record drawings showing existing facilities and underground utilities will be available to the Contractor through the CE S&C IDIQ Inspector. The Contractor shall verify all dimensions and actual conditions when developing individual Task Order proposals. Failure to verify the dimensions and locations shall be at the Contractor’s risk and shall not relieve the Contractor from accomplishing the work required by the contract at the price awarded by the Government.

2.14 MATERIAL SUBMITTALS & SHOP DRAWINGS

2.14.1 The Contractor shall identify all material and articles requiring approval contemplated by the Materials and Workmanship Clause.

2.14.2 The Contractor shall submit a Schedule of Submittals using a Submittal Log no later than five (5) calendar days after contract award, before making any submittals and prior to construction commencement.

2.14.3 Within ten (10) calendar days of award of a DO, the Contractor shall submit for CO approval electronic copies of all required material/shop drawing submittals using the AF Form 3000, Material Approval Submittal. All material submittals and shop drawings shall be attached to an AF Form 3000 with the DO title, project number, and DO number clearly indicated. One (1) copy will be returned to the Contractor.

2.14.4 Once the Government has approved a specific item of material or equipment on this contract, the item is normally considered to be approved for the remaining duration of the contract. Unless otherwise directed by the CO or the individual DO, the Contractor shall reference the previous approval on the AF Form 3000 and shall not need to resubmit on this item. If, at any time during the duration of this contract, the Contractor shall desire to change materials from those originally approved, the Contractor shall resubmit the new materials to the CO for approval prior to incorporation into a project.

2.14.5 The QCM shall verify and sign all material submittals and shop drawings prior to submission to the CO.

2.14.6 Submittals shall be legible and shall include all technical data needed to verify the item meets requirements of the UPB line item description, specifications, and applicable codes.

2.14.7 The Contractor shall not order or install materials prior to approval.

2.14.8 Contract Submittal Register: The Contractor shall develop and maintain a Contract Submittal Register to record and track all preapproved material submittals.

The Contractor shall number all material submittals consecutively throughout the life of the contract. For resubmittals after disapproval, or resubmittals upon request of the CO, the Contractor shall use the original submission number and an alpha, beginning with “(A)” after the original submission number.

2.15 PERMITS

2.15.1 Work Clearance Request: The Contractor shall acquire a DigSafe number prior to submitting the AF Form 103 for Government approval by calling either 811 or 888- DIG-SAFE (888-344-7233) and incorporate it on the AF Form 103. The Contractor shall initiate, prepare and submit an AF 103, Work Clearance Request, commonly referred to as a digging permit. The Contractor shall submit this request at least fourteen (14) calendar days prior to any excavation activities at the work site. The GPM will assist the Contractor by providing points of contact; however, the Contractor shall be solely responsible for executing the digging permit and coordinating utility markings with the CE shops, Base Communications, and outside utility companies.

2.15.2 The Contractor shall provide drawings and details showing the exact location and description of the intended type of excavation. The Contractor shall maintain and stake the boundaries of all required utility locations after marking have occurred. The Contractor shall allow ten (10) calendar days for processing the digging permit between the various marking activities and coordinating utility markings. The Contractor shall maintain the coordinated AF Form 103 at the excavation site and shall comply with instructions for hand excavation and other methods of safeguarding the buried utilities.

2.15.3 Burn Permits: The Government will not allow the burning of material and debris on HAFB.

2.15.4 Environmental Permits: The Contractor shall obtain all required Environmental Permits and file required notifications to regulatory agencies in accordance with (IAW) Federal, State, and Local regulations. The Contractor shall correct, solely at his expense and to the satisfaction of the appropriate regulatory agency, all violations he has cause. This includes any fines, penalties, or related actions imposed on the Contractor as well as compensating the Government for any fines, penalties, or related actions imposed as a result of the Contractor’s actions.

2.16 EXCAVATION

2.16.1 The Contractor shall verify as-built and field conditions. The Government will not guarantee the accuracy and adequacy of existing as-built drawings.

2.16.2 The Government will exercise reasonable attempts to identify/locate buried lines. The Contractor shall ensure that the appropriate CE shops and private utility purveyors have marked existing underground utilities prior to starting excavation.

2.16.3 Any lines identified by flags or on the drawings/digging permit shall be avoided by the Contractor. The Contractor shall take caution within three (3) feet of a utility mark out. To preclude accidental damage, the Contractor shall locate the exact location of all known utilities by hand digging three (3) feet on each side of all utility lines prior to any excavation with power equipment. Since depth of the utility will not be provided, the Contractor shall assume responsibility for any depth determination necessary.

2.16.4 If the Contractor hits anything within three (3) feet of a utility mark out, he shall immediately replace the utility back to its prior working condition. If the Contractor hits a utility that is unmarked or is outside the three (3) foot mark out, the CE shops will repair it. If the damaged utility requires an outside source to affect the repairs and the Contractor is responsible for the damage, the Contractor shall furnish payment within a twenty-four (24) hour period of having the work done. If an emergency situation, or if the CO determines the Contractor is not responding within a reasonable time, the Government will reserve the right to repair the damage and bill the Contractor at CE shop rates.

2.16.5 The Contractor shall immediately notify the CO, GPM, and fire department when any utility line is damaged. The Contractor shall notify base communications in the event of damage to any existing base communication lines.

2.16.6 The Contractor shall backfill all excavations immediately when possible, and always within one (1) week of completion of work.

2.16.7 The Contractor shall temporarily repair road cuts to assure a smooth transition until accomplishing final repairs.

2.16.8 The Contractor shall complete final grading within one (1) week of the completion of work involving disturbance to grounds/back filling. The Contractor shall complete final landscaping within three (3) weeks of the beginning of a planting season to all areas available for such work.

2.17 TESTING OF MATERIALS

2.17.1 All material or system tests that are charged as a direct DO expense shall be certified and submitted in the original form. The Contractor shall notify both the GPM and the CO at least twenty-four (24) hours prior to conducting tests.

2.17.2 The Contractor shall take casting and testing of concrete strength test cylinders, beams and cores in the presence of and as directed by the GPM. The Contractor shall notify both the GPM and the CO at least twenty-four (24) hours prior to the placement of any concrete. The Contractor shall label test cylinders, beams and cores immediately with black permanent marker to clearly indicate the cylinder number, project number and date. Cylinders, beams and cores shall remain on the project site until they are tested.

2.17.3 The Contractor shall perform proof rolling of subgrade material and compaction tests of fill material in the presence of the GPM, who will specify the location where each test is to be taken. Compaction tests shall typically be required for each lift. The Contractor shall notify both the GPM and the CO of the scheduled time and date for testing each lift at least twenty-four (24) hours in advance. Tests performed without the GPM present will not be accepted.

2.18 PROJECT SCHEDULING & COORDINATION

2.18.1 The Contractor shall submit work schedules to the CO/CA and the GPM prior to the start of work.

2.18.2 Before any work is initiated under an individual DO, The CM shall coordinate with the CO and the GPM and agree on a sequence of procedure; space for storage of materials and equipment; delivery of materials; location of smoking areas, eating areas and restrooms for the Contractor’s employees and any other incidentals to the project.

2.18.3 Each DO shall be diligently worked according to the approved progress schedule and without extending performance periods due to insufficient resources.

2.18.4 When specifically requested by the Government, the work shall, so far as practicable, be done in definite sections or areas and confined to limited areas that shall be completed before work in other areas is begun.

2.19 COORDINATING WITH GOVERNMENT ACTIVITIES

2.19.1 Utility Outages: The Contractor shall submit a schedule of anticipated requirements for impacted utility shutdowns within five (5) calendar days of each

Delivery Order NTP. This schedule shall be separate from any other construction schedule otherwise required. The Contractor shall include the following for each shutdown requirement:

2.19.1.1 Anticipated date;

2.19.1.2 Approximate duration;

2.19.1.3 List of buildings (or parts of buildings) or other affected facilities; and

2.19.1.4 Reason for the shutdown (work to be accomplished during the shutdown).

2.19.2 The Contractor shall submit a request for a specific utility shutdown twenty-one

(21) calendar days prior to each specific shutdown. The Government will require a thirty (30) calendar day notification for electrical power shutdowns affecting the following buildings: 1607, 1435, and 1646. This request shall include the information listed above as well as the requested time and duration of the shutdown.

2.19.3 The Contractor shall schedule major utility shutdowns which would disrupt any building’s occupants for an extended period for either a weekend or after the building’s normal operating hours whenever possible.

2.20 NOISE CONTROL

2.20.1 The Contractor shall comply with all Federal, State, and local laws, ordinances, and regulations relative to noise control. The Government may require that operations that generate excessive noise be scheduled at other than standard work hours.

2.21 CONSTRUCTION SITE MAINTENANCE

2.21.1 The Contractor shall store all supplies and equipment so as to prevent theft or mechanical/climatic damage. The Contractor shall maintain storage in a neat and orderly manner.

2.21.2 The Contractor shall dispose of all trash and debris generated during the contract. Disposal shall be by sanitary landfill or other approved methods and shall conform to Federal, State and local guidelines and regulations.

2.21.3 The Contractor shall perform mowing and trimming operations on his job site or storage site at no additional cost to the Government. The Contractor shall mow and trim vegetation when it reaches a height of five (5) inches. The Contractor shall mow and trim to a height of three (3) inches. The Contractor shall mow with a rotary mower that leaves the clippings evenly distributed on the soil surface. The Contractor shall mow during normal duty hour periods and in such a manner that the soil and grass shall not be damaged. Towed mowers and self-propelled riding mowers shall not be operated within three (3) feet of trees and shrubs. Areas next to trees and shrubs shall be mowed with hand-propelled mowers.

2.21.4 Prior to completion of the work each day, the Contractor shall handle, transport, store, and dispose of all waste materials and rubbish generated by any work under this contract. If the Contractor temporarily stores waste material or rubbish in a dumpster controlled by the Contractor, the Contractor shall secure the dumpster in such a manner that prevents unauthorized use. The Contractor shall post signs on the dumpster(s) noting it as a Contractor Dumpster and Not for Public Use. On a minimum weekly basis, the Contractor shall remove and dispose of any waste or excess material resulting from any contract requirement. Location of a dumpster at the site will be as directed by the GPM.

2.21.5 The Contractor shall clean all mud, dirt, debris, foreign objects, or spills of any kind from the Contractor’s operations (including subcontractors and suppliers) on base roads, streets and parking lots the same day.

2.21.6 The Contractor shall have all refuse removal trucks covered and secured prior to leaving the job site. If any materials are observed falling off the trucks, the Contractor truck shall stop and reload the truck to prevent any damage to any other vehicles caused directly/indirectly by falling debris. The Contractor shall bear the monetary responsibility to repair all damage associated with falling debris.

2.21.7 There are no approved concrete truck cleanout sites on base. The Contractor shall instruct the truck to wash out into the Contractor’s dumpster only. If the concrete truck cleans out anywhere else on base, the clean-up charges will be billed back to the Contractor and against the individual DO.

2.21.8 The base refuse contract supplies dumpsters for Government produced refuse only. The Contractor shall not dump refuse from any DO in the Government dumpsters. If the CO discovers DO refuse in Government dumpsters, he may instruct the Contractor to immediately remove the refuse or have the Government resources remove it at the Contractor’s expense.

2.21.9 The Contractor shall clean up each project site at the end of each day. The Contractor shall pick up and properly dispose of all debris (whether blown in or Contractor generated) on the job site. The Contractor shall neatly stack and secure all materials to prevent wind gusts from blowing materials around the base. If the CO deems the site is unsightly the Contractor will be instructed to halt construction and clean up the site.

2.21.10 Trees, Shrubs, Grass and Irrigation Systems: The Contractor shall take appropriate measures to prevent injury to plants and subsurface irrigation systems on the project site unless designated to be removed. The Contractor shall not remove or prune any plants without approval from the CO or his designated representative. The Contractor shall replace plants that are damaged during construction at no expense to the Government with a one (1) year warranty. The Contractor shall replace damaged shrubs or hedges with plants of equal size, type, and value. The Contractor shall repair any subsurface irrigation system damages caused during construction.

2.21.11 Upon the CO’s request, the Contractor shall install visual screening outside the construction site to maintain a neat appearance.

2.22 CONTRACTOR PARKING

2.22.1 The Contractor’s employees, suppliers, and subcontractor vehicles shall not park vehicles on grass surfaces at any time. In addition, the Contractor shall not drive vehicles over grass surfaces unless indicated during the site visit or pre-performance conference.

2.23 TRANSPORTATION OF CONSTRUCTION EQUIPMENT

2.23.1 The Contractor shall not drive any construction equipment with metal wheels or tracks (i.e. roller, excavator, etc.) on base streets. The Contractor shall load all equipment on a trailer and hauled to the location.

2.24 BASE FIRE REGULATIONS

2.24.1 The Contractor shall comply with HAFB Fire Regulations. The Contractor shall use no explosives or fire in performing the work. The Contractor shall understand and comply with current Occupational Safety and Health Administration (OSHA) welding and cutting requirements.

2.25 SECURITY REQUIREMENTS

2.25.1 The Contractor shall comply with all security regulations imposed by the installation commander and/or the agency occupying the space where the Contractor shall perform the work.

2.25.2 The Contractor shall maintain a current listing of employees and provide a list of the key personnel within thirty (30) calendar days of contract award, to the CO. The list shall include employee's name and date of investigation if contract work involves unescorted entry to Air Force restricted or other sensitive areas designated by the Installation Commander. The list shall be validated and signed by the Contractor representative with the authority to bind the Contractor, and provided to the Contracting Officer prior to the contract start date. The Contractor shall provide updated listings when an employee's status or information changes.
