Chapter 1.

45 SW HAZMAT Tracking Procedures for Construction and Service Contractors without LAN Access or CAC Cards

1.1. Overview

45th Space Wing (45 SW) Civil Engineering Squadron/Environmental Quality (CES/CEAN) Standard Operating Procedure (SOP), provides Hazardous Material (HAZMAT) tracking procedures for construction and service contractors that do not have a Common Access Cards (CACs) or Local Area Network (LAN) access.

1.2. Purpose

This SOP has been developed to effectively address the Air Force mandatory HAZMAT approval and tracking requirements in accordance with (IAW) Air Force Instruction (AFI) 32-7086.

1.3. Scope

This SOP applies to all service, construction and/or Indefinite Delivery and Indefinite Quantity (IDIQ) contractors that have the potential to use HAZMAT on any 45 SW Installation. It only applies to the afore mentioned contractors that do not have access to the Air Force LAN or CACs.

1.4. Applicable Supporting Documents

1.5. Roles and Responsibilities

1.5.1. Contractor

a. Provide to the Contracting Officer (CO) a proposed list of materials to be used during the duration of the contract.

b. Provide to the CO a completed AF Form 3952 and a manufacturer specific, up-to-date Material Safety Data Sheet (MSDS) for each proposed material that is deemed Hazardous by the 45 SW Hazardous Materials Management Process (HMMP) Team.

c. Provide to the CO a quarterly report NLT COB on the 10th day of the month of each new quarter (based on the fiscal year) for the materials purchased in the previous quarter and a purchase report for a partial quarter at the end of the contract, if the contract ends prior to the quarterly reporting period.

1.5.2. Contracting Office

a. Review the contractors proposed list of materials and provide a copy to the 45 CES/CEAN Installation Hazardous Materials Program (IHMP) Manager. The IHMP Manager is Mr. John Langett. He can be contacted at (321) 494- 9261 or john.langett.ctr@patrick.af.mil.

b. Receive and review the completed contractor provided AF Form 3952(s) and MSDS(s) for accuracy and correctness. Provide completed package to IHMP Manager.

c. Receive and review the contractors’ quarterly purchase report(s) and final purchase report and provide a copy to the IHMP Manager.

1.5.3 IHMP Manager

a. Review proposed list of HAZMAT with the HMMP Team to determine which materials will be tracked as HAZMAT, which materials will not be tracked and if any of the proposed materials are banned from use by the Air Force, HQ AFSPC or 45 SW.

b. Retrieve Material Stock Numbers (MSNs) for each material. Input all data provided by the contractor on the completed AF Form 3952(s) into the Enterprise Environmental, Safety and Occupational Health Management Information System (EESOH-MIS) and submit for HMMP review and approval.

c. Request issue and consumption data of all materials purchased by the contractor on a quarterly basis. This step will be accomplished through EESOH-MIS utilizing the Patrick AFB HAZMAT Pharmacy (HazMart).

1.6 Process

1.6.1. The following process shall be used by contractors that require the use of HAZMAT on 45 SW installations.

The process is being implemented IAW AFI 32-7086, Section 2.5.5.

1.6.2. Notice to Proceed

a. Upon receiving Notice to Proceed (NTP), the contractor shall submit to the respective CO a proposed list of HAZMAT they plan to use on the installation IAW FAR Clause 52.223-3. The CO will provide a copy of the list to the IHMP Manager. The IHMP Manager will coordinate review of the list via electronic correspondence or interoffice mail with the HMMP Team to determine which materials will be tracked as HAZMAT, which materials will not be tracked and if any of the proposed materials are banned from use. The IHMP Manager will provide the CO the determination(s) made by the HMMP Team. Expected time for HMMP review and determination is five working days.

b. The contractor shall prepare a Chemical/Hazardous Material Request/Authorization form (AF Form 3952) for each HAZMAT required to be a tracked. A blank AF Form 3952, a completed example of an AF Form 3952, accompanying directions on how to complete, and a list of processes can be found in Appendix C. A manufacturer specific up-to-date MSDS for the material shall accompany each completed AF Form 3952. This submittal can be provided in a hard copy or electronic format. Note: If provided in an electronic format each document must be kept separately (i.e. do not supply multiple MSDSs or AF Form 3952s as one electronic document).

c. The IHMP Manager will retrieve a MSN for each material. The IHMP Manager will generate a process or multiple processes under the contractors designated account in the EESOH-MIS. The processes will be established on an electronic version of the AF Form 3952 (e.g. painting-brush/roller process, painting-airless spraying process, painting-cleaning equipment process, etc.) The completed processes with applicable MSDS attached, will be submitted by the IHMP Manager for review by the 45 MDG Bioenvironmental Engineering Office and the 45 SW Ground Safety Office then approval by 45 CES/CEAN. Upon process approval, the CO will be provided a list of the contractors HAZMAT in the form of an Authorized User List (AUL) allowing the contractor to bring the approved HAZMAT onto the Installation. Expected time to build processes, add materials and receive approval in EESOH-MIS is ten working days.

1.6.3. Commencement and Duration of Contract

a. The contractor shall submit a quarterly purchase report NLT COB on the 10th day o f the month after the start of each new quarter (based on the fiscal year) for purchases made the previous quarter (e.g. Fiscal Year 2009, Fourth Quarter purchase report submitted 10 Oct 09 for reporting period 01 July 09 through 30 September 09). A template of this report will be provided by IHMP Manager and is part of the AUL. An example of the AUL/Purchase Report can be found in Appendix D of this SOP. If the contract is terminated before the last report due date, the contractor shall be submit a final purchase report for the partial quarter.

b. If at any time during the duration of the contract a material is substituted, a manufacturer changes, a container size changes, the quantity used is increased/decreased or a new material is required, section 1.6.2., b. must be completed by the contractor.

1.6.4. Contract End

a. If the contract ends before the last report due date, the contractor shall submit a final purchase report for the partial quarter.

b. At contract close out, the CO will notify the IHMP Manager that work has been completed and all materials and quantities are accounted for.

Appendix A Definitions

Dickinson Missile Tracking Annex, Antigua and Ascension Auxiliary Air Fields EESOH-MIS The current operating system used by the 45 SW to track HAZMAT request, purchase and use.

HAZMAT For purposes of AFI 32-7086, the term HAZMAT includes all items (including medical supply items, but excluding drugs in their finished form and pharmaceuticals in individually-issued items) covered under EPCRA (or other host nation, federal, state, or local) tracking requirement, the OSHA HAZCOM Standard, and all Class I and Class II ODS. It does not include munitions or HW.

HMMP For purposes of this SOP, the HMMP reviewers are functionals from Environmental Quality, Bioenvironmental Engineering and Ground Safety, and serve as the team focal point. The HMMP team lead is the CE Hazardous Materials Manager. The HMMP reviewers are responsible for ensuring the effective day-to-day management of hazardous materials.

MSDS A material safety data sheet (MSDS) is a document containing information regarding the properties of a particular substance. It is intended to provide workers and emergency personnel with procedures for handling that substance in a safe manner. It includes information such as physical data (i.e. melting point, boiling point, flash point, etc.), toxicity, health effects, first aid, reactivity, storage, disposal, protective equipment, and spill handling procedures.

Instructions. AF Form 3952, Chemical/Hazardous Material Request/Authorization Form, documents the hazardous material (HAZMAT) authorization process and establishes a standardized procedure for requesting and authorizing HAZMAT. Work area supervisors use AF Form 3952 to initiate a request for HAZMAT. This request provides information required to support the Air Force HAZMAT Management Process. Submissions of AF Forms 3952 must be accompanied by a copy of the manufacturer specific, up-to-date MSDS that the work area supervisor has used to conclude that the shop is prepared to properly use the specific material requested.

A. Part I—Material Request: After completing Part I, work area supervisors send an AF Form 3952 and a manufacturer specific, up-to-date MSDS for each HAZMAT that requires tracking to their respective Contracting Office.

The requestor should propose a Draw Amount and a Draw Frequency that is based upon the amount of material used during a process. For example, a shop may need to use 5 quarts of oil each time it performs an oil change on a fleet vehicle, which it does approximately once per month. The shop would enter “5 quarts” in Block 12 and “once per month” in Block 13

18. PROCESS—A process NAME and TYPE must be provided in this Block. The process NAME and TYPE MUST be retrieved from the closest matching name and type on the AFSPC EESOH-MIS Processes list. A description of the process should also be placed in this Block. Be sure to provide the following information:

i. Application method, including but not limited to: hand, brush, spray, spatula/putty knife, cloth, roller, dipping, pouring, squeeze bottle, hose, spray gun/nozzle, and vapor condensation.

ii. Type of industrial equipment, such as open tanks, closed tanks, vapor degreaser, spray booth, mechanical equipment, or liquid-tight equipment (closed system) in which the HAZMAT will be used.

iii. If the process involves transferring HAZMAT to industrial equipment specify whether the transfer will occur by pouring, pumping, or another method.

iv. Method by which the HAZMAT will be or has been mixed (hand; mixer, open container; mixer, closed container, etc.), heated (oven, soldering iron, torch, etc.), or abraded (wire brush, sander, grinder, etc).

19. AMOUNT OF MATERIAL USED PER PROCESS—This amount is different from, but related to, the DRAW AMOUNT in Block 12. For example, if the process is to change the 5 quarts of oil in a motor vehicle, then a user wishing to change the oil in three vehicles would draw 15 quarts of oil (BLOCK 12), but use only 5 quarts per individual process (per vehicle). Therefore, enter “5 quarts” in this block.

20. FREQUENCY OF PROCESS—How often the process is performed. Example: If a work area has to change the oil in one of its vehicles approximately once per month, so state. If this is a non-recurring (one-time) requirement, enter "One-time only." NOTE: Blocks 12 and 13 should be consistent with the data in Blocks 19 and 20. If these two sets of data are not consistent, the AF Form 3952 should be rejected until this can be resolved.

21. DURATION OF PROCESS—How long it takes to complete the process one time.

Include eye, face, body, foot, and hand protection; and manufacturer and model number for both respirator and cartridge.

DESCRIBE THE METHOD OF DISPOSAL FOR THE WASTE THAT IS GENERATED—Typical responses include, but are not limited to: totally consumed in use, partially consumed in use, recycled on-site, recycled off-site, drummed/containerized, sanitary sewer, storm sewer, industrial drain, bulk, and air emission. NOTE: "returned to DLA Class I ODS Defense Reserve" is mandatory for all Class I ODS refrigerants and halons. Class I ODS solvents are normally disposed of as hazardous waste.
