Mission Impact Statement - (SAMPLE)

MEMORANDUM FOR 96 MXG/MXO

FROM:

SUBJECT: Mission Impact Statement, Repair of Fuji FPM6000 Automatic Film Processor

1. Requirement: The Fuji FPM6000 Automatic Film Processor is in need of repair to complete radiographic operations in direct support of the 96th Test Wing aircraft and associated aerospace support equipment. Repair service is requested not later than 15 May 2014.

2. The loss of the capabilities of the Fuji FPM6000 Automatic Film Processor severely impedes the ability to support the mission of the 96th Test Wing. In the event of the film processor becoming totally inoperable, we would be forced to transport the film to Tyndall AFB to process.

This would require several additional man-hours for transportation, development and any re-shots which may be required. A replacement film processor has been on order since April 2013. We currently have an estimated delivery date of December 2014.

3. Alternative Solution: As a work-around, we are currently operating the Fuji FPM6000 Automatic Film Processor in “self-test bypass” mode which allows the rollers to continually operate instead of turning off when there is no film processing. This self-test bypass mode is meant to be used to carry out an urgent operation or continue with operation until the service personnel arrives, continued use in the self-test bypass mode could result in further damage to the machine.

4. This request will allow relief from extended downtime for test mission critical airframes. For the above reasons, priority handling of this matter is hereby requested. If there are any questions or concerns, please contact Mr./Mrs. XXXXXX XXXXXX at XXX-XXXX.

Sole Source Justification - (SAMPLE)

MEMORANDUM FOR 96 MXG/MXQCB

FROM:

SUBJECT: Justification for Other Than Full and Open Competition, Sole Source Justification for REQUIREMENT NAME, PR # XXXXXXX

1. Contractor Name is looking to procure REQUIREMENT NAME for the CUSTOMER ORG.

Under the contract, the contractor will provide the following equipment/ to the CUSTOMER

ORG:

(a) QTY(8) EA, ITEM DESCRIPTION/SPECS, MFR: XXXX, P/N: PART NUMBER

(b) QTY(8) EA, ITEM DESCRIPTION/SPECS, MFR: XXXX, P/N: PART NUMBER

(c) QTY(8) EA, ITEM DESCRIPTION/SPECS, MFR: XXXX, P/N: PART NUMBER Total Estimated cost: $XX, XXX.XX

2. The Statutory authority permitting other than full and open competition is FAR 6.302-1(a).

3. The primary system to be acquired is the REQUIREMENT NAME.

(a) The ITEM NAME(S) are/ is necessary to support CUSTOMER ORG test missions. The CUSTOMER ORG requires these ITEM(S) to operate in conjunction with previously acquired parts to meet new mission requirements and will operate NAME OF EQUIPMENT OR SYSTEM. The ITEM(S) from REQUESTED SOLE SOURCE MANUFACTURER are currently the only Commercial off the Shelf (COTS) parts that meets the Government’s technical requirements of functioning in conjunction with a same brand and model previously acquired to meet the mission requirements.

(b) The ITEM(S) from REQUESTED SOLE SOURCE MANUFACTURER are the only parts that are compatible and are able to be swapped out with each other without the need to re-adjust/fine tune the parts and pieces as well as re-writing programs to make other brands communicate with the system to continue the test should a failure occur. This would cause significant time delays to the test missions and timelines.

(c) The CUSTOMER ORG believes that award to any other brand would result in substantial duplication of cost to the Government that is not expected to be recovered through competition and unacceptable delays in fulfilling the Air Force’s requirements.

http://farsite.hill.af.mil/reghtml/regs/far2afmcfars/fardfars/far/06.htm

(d) An award to another brand would result in both substantial delay and increase in cost, as the award would jeopardize compatibility and performance with existing hardware. If another brand of part was acquired, mission integrity would be compromised and the necessary criteria would not be met. The increased cost for the government is approximately $XXXX.XX based on an estimated XX number of hours from a GS-XX employee ($XX.XX/hr.) and O/E-XX military personnel ($XX.XX/hr.) or some statement that expresses how a quantitative number cannot be determined for the actual cost to the Government.

(e) Accordingly, REQUESTED SOLE SOURCE MANUFACTURER is the only brand capable of providing the supplies and services described above without the U.S. Air Force experiencing unacceptable delays in fulfilling its requirements.

4. Market research conducted for this acquisition included both Google and GSA Advantage searches to determine if another product and/or manufacturer existed that offered a part that met the Government’s requirements as well as matching the technical capabilities for the ITEM(S) by SOLE SOURCE NAME MANUFACTURER, none were found. No other substitute will meet the Government’s minimum requirements.

5. The Government chose not to use technical data packages, specifications, statements of work, or full and open competition as the Government believes pursuing the purchase of another COTS ITEM(S) meeting the Government’s minimum requirements would result in substantial increased time delays and costs to the Government.

6. The source that expressed interest in this requirement is REQUESTED SOLE SOURCE

MANUFACTURER.

7. We will continue monitoring the industry through trade journals, technical symposia, and contact with industry representatives should a future need arise. Any future requirements will be researched thoroughly. If a product meeting the technical requirements is identified, a competitive acquisition for the requirement will definitely be considered.

8. The undersigned personnel certify that the justification is accurate and complete to the best of their knowledge and belief.
