Solicitation Number FA2263-20-R-0002 RFP Attachment
SECTION L
Instructions, Conditions, and Notices to Offerors or Respondents
1 PROGRAM INFORMATION
1.1 Program Structure and Objectives
This acquisition is for the continued operation of the Air Force Primary Standards Laboratory (AFPSL) located in Heath, Ohio, and the development and distribution of calibration technical manuals for the Air Force Metrology Calibration Program (AFMETCAL). The AFPSL is the highest echelon metrology and calibration laboratory in the Air Force. It provides a critical link between the National Institute of Standards and Technology (NIST) and Air Force Precision Measurement Equipment Laboratories (PMELs) and houses a measurement capability in practically every measurement discipline with a precision and accuracy unequalled in the Air Force. The AFPSL complex consists of laboratories with rigid environmental controls necessary for precise measurements in many measurement disciplines. The AFPSL provides calibration services and exchange standards to Air Force PMELs. It is the sole source of Air Force measurement standards, base reference standards, and other highly specialized calibration services. The AFPSL’s functions ensure Air Force measurements are accurate, uniform, reliable, and traceable to NIST. Supplies, services, and tasks required for sustainment of calibration services by the AFPSL and technical order developments are listed below.
A. Major Tasks The successful offeror shall perform calibration and calibration technical order development services.
a. Calibrate Air Force customer standards.
b. Perform specialized testing in support of Air Force Program Offices, Research and Development Laboratories, item managers, single managers, and AFMETCAL.
c. Develop, revise, and distribute calibration technical orders (TOs).
d. Provide technical assistance.
e. Perform laboratory scheduling and inventory actions.
f. Comply with AFMETCAL quality and facility requirements and maintain laboratory certification.
B. Support Tasks
a. Maintain storage areas with protection against loss, damage, and adverse environmental conditions to meet requirements of a Government approved bond room.
b. Maintain classified storage for end items and technical data with security classifications up to and including Secret.
c. Conduct inventory and property management.
d. Provide preservation, packaging, and transportation.
The pricing arrangement for this contract is firm fixed price for contract transition and special calibrations-project plans; fixed price with an incentive fee for exceptional performance for the operation of the AFPSL; cost plus incentive firm for the development and distribution of calibration technical manuals services and special calibrations-acceptance testing; and cost reimburseable for FMS calibration services and external calibration and repair services. The pricing arrangement was selected based upon the risk assessments and the nature of the contracting effort.The pricing arrangement for this contract is fixed price for the operation of the AFPSL and special calibration services, with an incentive fee for exceptional performance and cost containment, and cost reimbursement with no fee for external calibrations, FMS calibrations, and development and distribution of calibration technical manuals services. The pricing arrangement was selected based upon the risk assessments and the nature of the contracting effort. (Pricing Arrangment will be finalized upon ASP approval)
2 GENERAL INSTRUCTIONS
The offeror's proposal must include all data and information requested by this Information to Offerors (ITO) and must be submitted IAW these instructions. The offer shall be compliant with the requirements as stated in the Performance Work Statement (PWS), Contract Data Requirements List (CDRL), and RFP. Non-conformance with the instructions provided in this ITO may result in an unfavorable proposal evaluation.
The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal should not simply rephrase or restate the Government's requirements but, rather, shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of their facilities or experience and will base its evaluation on the information presented in the offeror's proposal.
Elaborate brochures or documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired.
The proposal acceptance period is specified in Section A of the RFP. The offeror shall make a clear statement in Section A of the Contract Documentation Volume IV that the proposal is valid through this date.
IAW FAR Subpart 4.8 (Government Contract Files), the Government will retain one electronic copy of all unsuccessful proposals. The Government will destroy extra copies of such unsuccessful proposals.
2.1 Point of Contact
The Contracting Officer (CO) is the sole point of contact for this acquisition. Address any questions or concerns you may have to the CO. Written requests for clarification may be sent to the CO at the address located in Section A of the model contract/solicitation.
2.2 Debriefings
The CO will promptly notify offerors of any decision to exclude them from the competitive range; whereupon, they may request and receive a debriefing IAW FAR 15.505. Offerors excluded from the competitive range may request a pre-award debriefing or they may choose to wait until after the source selection decision to request a post-award debriefing. However, offerors excluded from the competitive range are entitled to no more than one debriefing for each proposal. The CO will notify unsuccessful offerors in the competitive range of the source selection decision IAW FAR 15.506. Upon such notification, unsuccessful offerors may request and receive a debriefing. Offerors desiring a debriefing must make their request IAW the requirements of FAR 15.505 or 15.506, as applicable.
2.3 Discrepancies
If an offeror believes that the requirements in these instructions contain an error, omission, or are otherwise unsound, the offeror shall immediately notify the CO in writing with supporting rationale, as well as, the remedies the offeror is asking the CO to consider as related to the omission or error.
2.4 Electronic Reference Documents
All referenced documents for this solicitation are available on the Federal Business Opportunities (FedBizOpps) website at https://www.fbo.gov/. Potential offerors are encouraged to subscribe for real-time e-mail notifications when information has been posted to the website for this solicitation. This is a public access site, no special permissions are required to access the Bidder's Library. 2.5 Communications
Exchanges of source selection information between the Government and Oofferors will be controlled by the CO, Michaelke Hickey, email: michael.hickey.2@us.af.mil. Source Selection information will be transmitted via direct mailing or AMRDEC SAFE at https://safe.amrdec.army.mil/safe/guide.aspx. Email through AMRDEC SAFE may be used to transmit such information and must include “Source Selection Information - See FAR 2.101 and FAR 3.104, For Official Use Only (FOUO)” in the subject line of the email.
3 PROPOSAL PREPARATION INSTRUCTIONS
3.1 Organization/Number of Copies/Page Limits
The offeror shall prepare the proposal as set forth in the Proposal Organization Table (Table 3.1). The titles and contents of the volumes shall be as defined in this table, all of which shall be within the required page limits and with the number of copies as specified in Table 3.1. The attachments identified in the table should be separately bound in three-ring, loose-leaf binders, as necessary. The contents of each proposal volume are described in the ITO paragraph as noted in the table below:

3.1.1 Page Limitations
Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal. Page limitations shall be placed on responses to evaluation notices (ENs). The specified page limits for EN responses will be identified in the letters forwarding the ENs to the offerors. When both sides of a sheet display printed material, it shall be counted as two pages. Additionally, each volume shall contain a table of abbreviations used, with their respective explanations. Tab indexing shall be used to identify all sections. The tables of contents, tab indexes that contain no information other than section information, and tables of abbreviations, glossaries (as required), are excluded from the total page count.
3.1.2 Page Size and Format
(a) A page is defined as each face of a sheet of paper containing information. When both sides of a sheet display printed material, it shall be counted as two pages. Page size shall be 8.5 x 11 inches, not including foldouts. Pages shall be double spaced. Except for the reproduced sections of the solicitation document, the font shall be Times New Roman and the text size shall be no less than 12 point. Tracking, kerning, and leading values shall not be changed from the default values of the word processing or page layout software. Use at least 1-inch margins on the top and bottom and 3/4-inch side margins. Pages shall be numbered sequentially by volume. These page size and format restrictions shall apply to responses to ENs. These limitations shall apply to both electronic and hard copy proposals. For Past Performance Information Tool printouts, use the default page size and format from the tool.
(b) Legible tables, charts, graphs, and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc. These displays shall be uncomplicated, legible, and shall not exceed 11 by 17 inches in size. Foldout pages shall fold entirely within the volume, and count as a single page. Foldout pages may only be used for large tables, charts, graphs, diagrams, and schematics not for pages of text. For tables, charts, graphs and figures, the text shall be no smaller than 8 point.
3.2 Pricing Related Data
All cost or pricing information shall be addressed ONLY in the Cost/Price Proposal and Contract Documentation Volumes. Cost trade off information, work hour estimates and material kinds and quantities may be used in other volumes only as appropriate for presenting rationale for alternatives or design and trade off decisions.
3.3 Classified Information
The proposal shall not contain Classified information.
3.4 Cross-Referencing
To the greatest extent possible, each volume shall be written on a stand-alone basis so that its contents may be evaluated with a minimum of cross referencing to other volumes of the proposal. Information required for proposal evaluation which is not found in its designated volume will be assumed to have been omitted from the proposal.
Cross-referencing within a proposal volume is permitted where its use would conserve space without impairing clarity.
The offeror shall provide a cross reference matrix indicating, by ITO or PWS paragraph number, the corresponding proposal paragraph in that section which addresses the referenced item.
3.5 Indexing
Each volume shall contain a more detailed table of contents to delineate the subparagraphs within that volume. Tab indexing shall be used to identify sections.
3.6 Glossary of Abbreviations and Acronyms
Each volume shall contain a glossary of all abbreviations and acronyms used, and with an explanation for each. Glossaries do not count against the page limitations for their respective volumes.
3.7 Binding and Labeling
Each volume of the proposal should be separately bound in a three-ring, loose leaf binder permitting the volume to lie flat when open. Staples shall not be used. A cover sheet should be bound in each book, clearly marked as to volume number, title, copy number, solicitation identification, and the offeror's name. The same identifying data should be placed on the spine of each binder. All unclassified document binders shall have a color other than red or other applicable security designation colors. Be sure to apply all appropriate markings including those prescribed IAW FAR 52.215-1(e), Restriction on disclosure and use of data, and FAR 3.104-4, Disclosure, Protection, and Marking of Contractor Bid or Proposal Information and Source Selection Information.
3.8 Electronic Offers
The content and page size of electronic copies must be identical to the hard copies. For electronic copies, indicate on each CD the volume number and title. Use separate files to permit rapid location of all portions, including subfactors, exhibits, annexes, and attachments, if any. The offeror shall submit volumes I, II, IIIIVII, and V in electronic format, using recordable CDs. Each volume shall be in a different directory on a CD. If files are compressed, the necessary decompression program must be included. In the event that hard copies and electronic copies of a proposal are submitted and if there are any discrepancies between the hard copies and the electronic copies of the proposal, the hard copies will be used for evaluation. The electronic copies of the proposal shall be submitted only using Microsoft Office 2010 or newer, and Adobe Acrobat or newer:
1. Word documents - Set "Save as type" to "Word Document (*.docx)" only.
2. Excel spreadsheets - Set “Save as type” to “Microsoft Excel Workbook (*.xlsx).”
3. Submit any Microsoft Project files in Microsoft Project 2010 or newer. There are no special “save” functions required for later versions of Project.
4. Offerors shall submit PDF documents using only Adobe Acrobat 9.0 or later. Do NOT submit any documents in PDF format that are copied as “images.” When creating PDF files always create to enable textual search and copy functions.
3.9 Distribution
The "original" hard copy proposal shall be identified. Two. TwoAll hard and two electronic proposals (and all 3 copies of Cost/Price) shall be addressed to the Contracting Officer and mailed to:

Be sure to advise the Administrative Contracting Officer (ACO) and the Defense Contract Audit Agency (DCAA) for prime and subcontractors that the proposal is "For Official Use Only" and "Source Selection Information--See FAR 2.101 and FAR 3.104".
4 PROPOSAL ORGANIZATION
4.1 Volume I Executive Summary
In the executive summary volume, the offeror shall provide the following information:
4.1.1 Narrative Summary
The narrative summary of the entire proposal should be concise, to include addressing the significant risks, and highlighting any key or unique features, excluding cost/price. The salient features should tie in with Section M evaluation factors/subfactors. Any summary material presented here shall not be considered as meeting the requirements for any portions of other volumes of the proposal.
4.1.2 Table of Contents
Include a master table of contents of the entire proposal.
4.2 Volume II - Technical Volume
4.2.1 General
The Technical Volume should be specific and complete, include only the requested narrative and proposed manning. DO NOT include cost price figures. All pricecost information must be in the cost price proposal. Legibility, clarity, and coherence are very important. Your responses will be evaluated against the Technical subfactors defined in Section M, Evaluation Factors for Award. Using the instructions provided below, provide as specifically as possible the actual methodology you would use for accomplishing/satisfying these subfactors. All the requirements specified in the solicitation are mandatory. By your proposal submission, you are representing that your firm will perform all the requirements specified in the solicitation. It is not necessary or desirable for you to tell us so in your proposal. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation. 4.2.2 Format and Specific Content
Mission Capability and Proposal Risk will be addressed in the Technical volume. In this volume, address your proposed approach to meeting the requirements of each Technical subfactor, as well as the risks in your proposed approach in terms of mission capability/ performance, cost, and/or schedule.
4.2.3 Technical
In the technical volume, address your technical solution for meeting the Government’s minimum performance or capability requirements of each technical subfactor.
(If you want to evaluate the risk of the proposed technical approaches on an acceptable/unacceptable basis, consider including the following paragraph. In Section M, the Measures of Merit will need to specify that the proposed technical approach must be evaluated as low to moderate risk (or just low risk if you’re really risk averse) in order to be determined “technically acceptable.”)
4.2.3.1 Technical Risk
Address Technical Risk associated with those aspects of your proposed technical approach you consider to have the potential for disruption of schedule, increased costs, degradation of performance, the need for increased Government oversight, or the likelihood of unsuccessful contract performance. Classify each risk IAW DoD Source Selection Procedures Table 3, Technical Risk Rating. Provide the rationale for each risk and its rating, including quantitative estimates of the impact on cost, schedule, and performance. Describe the impact of each identified risk in terms of its potential to interfere with or prevent the successful accomplishment of other contract requirements (for example: SOW PWS or specification requirements), whether or not those requirements are identified as subfactors. Propose a realistic "workaround" or risk mitigator for identified risks that will eliminate or reduce risk to an acceptable level. Identify and classify any new risks introduced by such risk mitigation.
4.2.4 Volume Organization
The Technical volume shall be organized according to the following general outline:

(5) Subfactor One: Staffing (Manning level and Labor Category) Approach
(6) Subfactor Two: Recruitment, Training, and Retention Staffing (Recruitment, Training, and Retention Plans)

(8) Subfactor Four: Calibration Technical Orders Quality Plan
(9) Subfactor Five: Measurement Area Assurance
4.2.5 Subfactor One: Staffing Approach (Manning level and Labor Category) The offeror shall identify organizational structureprovide a staffing approach to include the quantity of personnel, US Department of Labor plus the knowledge, skills, abilities, and experience of personnellabor category, and qualificationsof personnel required in each area of the organizational structure, refer to Technical Exhibit 2A, to include calibration technicians capable of performing at the Air Force’s highest echelon calibration laboratory, technical writers, schedulers, lab supervisors, quality assurance personnel, engineers, health physicist, etc.
4.2.6 Subfactor Two: Staffing (Recruitment, Training, and Retention Plans) The offeror shall describe provide their approach to recruit and retain a qualified workforce to include detailed approach to meeting competition/demand forof local on-site labor market/poolnational labor categories to include calibration technicians, technical writers, schedulers, lab supervisors, quality assurance personnel, engineers, health physicist, etc. The offeror shall address how newly hired employees will be trained and qualified to perform the work required., as well The offeror shall provide as, the plans its approach for cross-training, advancement training, and continuing training education of all employees to ensure continuity of AFPSL operations.
4.2.7 Subfactor Three: Quality Plan
The contractor offeror shall describe their approach to meet quality program requirements IAW PWS paragraph 1.4, TO 00-20-14 and ISO/IEC17025 calibration laboratory quality standards.
4.2.8 Subfactor Four: Calibration Technical Orders Quality Plan The offeror shall describe their quality plan addressing technical order functions described in the PWS paragraph 5.2. The offeror shall describe their approach to develop and publish new, rewrite, and revision/change technical orders and distribution in the preliminary Technical Order Production Plan submitted.
4.2.9 Subfactor Five: Measurement Area Assurance
The offeror shall describe the ir approach to meeting Measurement Area Capability Document (MACD) requirements framework, PWS paragraph 5.1.8.8.1,, to include approaches used to validate measurement uncertainty and ensure traceability, to record, maintain, and analyze historical data, provide statistical processes and analysis to validate measurement uncertainties, and processes to validate measurement uncertainties against other DoD services and industry ensure measurement capability uncertainties are maintained. in accordance.
5 VOLUME III - PAST PERFORMANCE
5.1 Past Performance Information (PPI) Tool
Each offeror shall submit past performance via the Past Performance Information (PPI) Tool available on FBO.gov. Instructions for use of the PPI Tool are found in Attachment 2: Past Performance Information Tool Instructions.
Each offeror shall submit a past performance volume with its proposal, containing past performance information IAW the format contained in Attachment ____ (download and use the current version of the HYPERLINK "https://cs.eis.af.mil/airforcecontracting/records_contract/EZ/EZ_Docs/Forms/Past_Performance_Information.aspx" Attachment for Section L,) that contains instructions for use of the PPI Tool. Use of this PPI Tool will save the Past Performance evaluation team a significant amount of time and effort upon proposal receipt since the offerors will submit their PPI electronically in a format that can be uploaded into EZSource. The PPI Tool is available to all teams using EZSource and its use is highly recommended. Make sure you read and tailor the PPI Tool instructions for your solicitation, and upload the tool to your FedBizOpps solicitation page.
In order to upload the PPI tool to FedBizOpps, perform the following steps:
1. Access the FedBizOpps ( HYPERLINK "https://www.fbo.gov/" https://www.fbo.gov/) website
2. Go to your team’s specific solicitation page.
3. Upload the PPI tool from HYPERLINK "https://cs1.eis.af.mil/sites/afcc/records_contract/ez/EZ_Docs/Forms/AllItems.aspx?RootFolder=%2Fsites%2Fafcc%2Frecords%5Fcontract%2FEZ%2FEZ%5FDocs%2FPast%5FPerformance%5FInformation&FolderCTID=0x012000275BAF38D663AC48BC3196DB89C381F9&View=%7b2BAD56B2-C1CB-42B2-A916-117AD9A44651%7d" The AFCC website to the “All Files” column on the solicitation’s “Notice Details” tab with the solicitation posting.
4. The PPI tool displays as a link for the offerors to download the tool.). This information is required on the offeror and all subcontractors, teaming partners, and/or joint venture partners who are proposed to perform ____ (insert percentage) percent or more of the proposed effort based on the total proposed price or perform aspects of the effort the offeror considers critical to overall successful performance. Offerors are cautioned that the Government will use data provided by each offeror in this volume and data obtained from other sources in the evaluation of past performance.
5.2 Consent/Authorization Letters
Along with the information required in this section, the oOfferors shall submit a consent letter executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing release of adverse past performance information to the offeror so the offeror can respond to such information. For each identified effort for a commercial customer, the offeror shall also submit a client authorization letter, authorizing release to the Government of requested information on the offeror's performance.
5.3 Early Proposal Information
Each offeror is requested to submit the past performance information using the PPI Tool for each relevant contract 10 calendar days prior to the date set for receipt of proposals. Failure to submit early proposal information will not result in offeror disqualificationEach offeror is requested to submit the information shown in Attachment ____ (Attachment 2 to this template) for each relevant contract ____ (insert number) days prior to the date set for receipt of proposals. 5.4 Recent and Relevant Contracts
Offeror shall submit past performance information on Submit information IAW Attachment ____ (same Attachment number as the one referenced in 5.1 above): Past Performance Information on four (4) recent contracts that you consider most relevant in demonstrating your ability to perform the proposed effort. Offerors shall complete a separate "FACTS Sheet" for each active or completed contract (with preferably at least one year of performance history) in the past five (5) years, that the offeror considers relevant in demonstrating its ability to perform the proposed effort. If the total number of such contracts exceeds four (4), the offeror shall address its four (4) most relevant contracts. Also include information on four (4) recent contracts performed by each of your teaming partners and significant subcontractors that you consider most relevant in demonstrating their ability to perform the proposed effort. For all submittals, the performance on efforts identified shall be restricted to those completed or ongoing during the past five (5) years from the issuance date of this RFP (with preferably at least one year of performance history). Include rationale supporting your assertion of relevance. For a description of the characteristics or aspects the Government will consider in determining recency and relevance, see Section M, paragraph 2.3.2.2. We don’t normally see a statement like this.
5.4.1 Specific Content
Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort and to what aspects of the proposed effort they relate. This may include a discussion of efforts accomplished by the offeror to resolve problems encountered on prior contracts as well as past efforts to identify and manage program risk. Merely having problems does not automatically equate to an unacceptable/a limited or no confidence rating, since the problems encountered may have been on a more complex program, or an offeror may have subsequently demonstrated the ability to overcome the problems encountered. The offeror is required to clearly demonstrate management actions employed in overcoming problems and the effects of those actions in terms of improvements achieved or problems rectified. This may allow the offeror to be considered more favorably than without evidence of problem resolution. For example, submittal of quality performance indicators or other management indicators that clearly support that an offeror has overcome past problems is required. Categorize the relevant information into the specific Technical subfactors and Price factor used to evaluate the proposal. 5.4.2 Organizational Structure Change History
Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. A pamphlet or other commercial document describing such reorganizations may suffice. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.
6.0 VOLUME IV - PRICE
6.1 General Instructions
6.1.1 Price Reasonableness and Realism
This section is to assist you in submitting data other than certified pricing data that is required to evaluate the reasonableness and balance of your proposed price. Compliance with these requirements is mandatory and failure to comply may result in rejection of your proposal. Note that unreasonably low or high proposed prices, initially or subsequently, may be grounds for eliminating a proposal from competition either on the basis that the offeror does not understand the requirement or has submitted an unrealistic proposal. Additionally, unbalanced pricing poses an unacceptable risk to the Government and may be a reason to reject an offeror’s proposal. Offers should be sufficiently detailed to demonstrate their reasonableness and balance. This section is to assist you in submitting data other than certified cost or pricing data that is required to evaluate the reasonableness and balance of your proposed cost/price. Compliance with these requirements is mandatory and failure to comply may result in rejection of your proposal. Note that unrealistically low or high proposed costs or prices, initially or subsequently, may be grounds for eliminating a proposal from competition either on the basis that the offeror does not understand the requirement or has submitted an unrealistic proposal. Offers should be sufficiently detailed to demonstrate their reasonableness and balance. The burden of proof for credibility of proposed prices rests with the offeror.
6.1.2 Price Data
Data beyond that required by this instruction shall not be submitted, unless you consider it essential to document or support your price position. All data relating to the proposed price, including all required supporting documentation, must be included in the section of the proposal designated as the Price volume. Under no circumstances shall this data and documentation be included elsewhere in the proposal.
6.1.3 Cost or Pricing Data Requirements
IAW FAR 15.403-1(b) and 15.403-3(a), data other than certified cost or pricing data may be required to support a determination of price reasonableness. Data shall be provided IAW FAR 15.403-5. If, after receipt of proposals, the CO determines that there is insufficient data available to determine price reasonableness and none of the exceptions in FAR 15.403-1 apply, the offeror shall be required to submit additional cost or pricing data.
6.1.4 Rounding
All dollar amounts provided shall be rounded to the nearest dollar. All loaded labor rates shall be rounded to the nearest penny.
6.2 Volume Organization
The Price volume shall consist of the following sections:
SECTION 1 - Table of Contents; summary descriptions of estimating, purchasing, and accounting systems; changes to estimating, accounting practices, or Cost Accounting Standards (CAS) Disclosure Statement.
SECTION 2 - Cost or pricingpPricing information and supporting data, to include estimating methodology.
SECTION 3 - Other information such as Government Furnished Property/Government Furnished Equipment (GFP/GFE), base support, long lead costs, termination costs, development/production schedule, inflation rate summary and explanation, and special tooling/test equipment. List each exception to the ground rules and assumptions provided in the solicitation and each qualification of the Price volume, if any. Provide complete rationale for any exceptions.
7.0 VOLUME V CONTRACT DOCUMENTATION
7.1 RFP/Representations and Certifications
The purpose of this volume is to provide information to the Government for preparing the contract document and supporting file. The offeror's proposal shall include a signed copy of the RFP, and Sections A through K. This includes:
7.1.1 Section A - Solicitation/Contract Form
Completion of block 153 and signature and date for block 127 of the SF 331447. Signature by the offeror on the SF 331447 constitutes an offer, which the Government may accept. The "original" copy should be clearly marked under separate cover and should be provided without any punched holes.
7.1.2 Section B - Supplies or Services and Prices
Completed pricing information in Section B of the model contract. Comment by HALL, MARCIA A CIV USAF AFMC AFLCMC/AZA: Make sure this is what you really want the offeror to do. Since there is a pricing matrix, typically, the TEP tab is removed and the resulting matrix goes on contract so when you activate a CLIN, you use the pricing on the matrix. It’s not a simple task to “enter” the prices in Section B unless you provide the model contract as a fillable pdf. Otherwise, they will either hand write the dollar amount and scan it or have to find a typewriter somewhere to type the amount in.
7.1.3 Section K - Representations, Certifications, and other Statements of Offerors Include completed representations, certifications, acknowledgments and statements.
7.2 Exceptions to Solicitation Requirements
Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Each exception shall be specifically related to each paragraph and/or specific part of the solicitation to which the exception is taken. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, price, and specific requirements of the solicitation. This information shall be provided in the format and content of Table 7.2.

7.3 Other Information Required
7.3.1 Authorized Offeror Personnel
Provide the name, title, and telephone number of the company/division point of contact regarding decisions made with respect to your proposal and who can obligate your company contractually. Also, identify those individuals authorized to negotiate with the Government. Also provide the name, title and telephone number of the CEO and Division President.
7.3.2 Government Offices
Provide the mailing address, telephone and fax numbers and facility codes for the cognizant Contract Administration Office, DCAA, and Government Paying Office.
7.3.3 Company/Division Address, Identifying Codes, and Applicable Designations Provide company/division's street address, county and facility code; CAGE code; DUNS code; TIN; size of business (large or small); and labor surplus area designation. This same information must be provided if the work for this contract will be performed at any other location(s). List all locations where work is to be performed and indicate whether such facility is a division, affiliate, or subcontractor, and the percentage of work to be performed at each location.
7.3.4 Attachments to the RFP
The offeror shall provide the following as attachments to the model contract:
7.3.4.1 Subcontracting Plan - Participation of Small Businesses (SB)., Historically Black Colleges and Universities, or Minority Institutions (HBCU/MI) If the offeror is other than a small business, the offeror shall submit a Small Business Subcontracting Plan IAW FAR 52.219-9 that also identifies and specifies the extent of offeror's commitment to the participation of small businesses (SB), historically black colleges or universities (HBCU) and minority institutions (MI), whether as joint venture members, teaming arrangement partners, or subcontractors. If applicable, submit a copy of your approved Master Plan. In the event the offeror has negotiated a comprehensive subcontracting plan pursuant to DFARS 219.702, the offeror must submit the information that identifies and specifies the extent of its commitment to the participation of SB, HBCU and MI.
7.3.4.2 Make-or-Buy
A make or buy program is not applicable to the proposed effort. The offerors will not be required to submit a make or buy plan because the proposed contract is a service contract for the operation of the AFPSL.
7.3.4.3 GFE
The Government plans to provide the items listed in Attachment ____ (insert number) Tech Exhibit 5b (GFE List) of the solicitation as GFE. If the offeror requires the use of Government furnished items other than those specified, the offeror shall provide a listing including quantity, federal stock number, nomenclature, date needed and duration of availability, rental value per FAR 45.1 and FAR 45.2, reason for need, and cross reference to cost/price volume paragraphs which pertain to GFP and/or base support. The offeror shall also provide the written authorization from the cognizant CO, as applicable. The offeror shall supply this information in the format shown in Table 7.3.4.3
7.3.4.4 Associate Contractor Agreements
The offeror shall be required to enter into a lease agreement with the local Port Authority, as well as with the Inertial Guidance Contractor at the facility.
