SUBJECT: Request for Present and Past Performance Information, Solicitation No. FA0021-16-R-0001
1. Air Force Special Operation Command is in the process of selecting contractors for award of a Combined Contract Close Air Support contract for the period of 12 months for the base year, plus three (3) 12 month option period.
2. One of the considerations in proposal evaluation is the verification of the offeror’s Present and Past Performance on contracts, which reflects the offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an offeror, for the evaluation of the offeror's performance on those contracts.
3. Our areas of interest in the offeror are summarized in the attached questionnaire, which we have requested be forwarded by the offeror to his customers. We need your written response by 09 Dec 2015 12:00PM CST. This schedule will allow us sufficient time to analyze the information prior to start of negotiations. It is important that you return the completed questionnaire directly to the Government.
4. To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and emailed to: Capt Amanda Szabolcs: amanda.szabolcs@us.af.mil or Mrs. Jaime Collins: jaime.collins.2@us.af.mil completed questionnaire will become a part of the official source selection records.
5. Your assistance is greatly appreciated, and your prompt response will be one of the keys to the successful and timely completion of this award process.
