ADDITIONAL INFORMATION (Q&A’s, etc) PREVIOUSLY PROVIDED – BLDG 403 REMEDIATION

1. Is there guidance on how they want the proposal prepared and what all information should be included?

a. Proposal should outline confirmation of understanding as to what is required, and the cost to perform the required work, to the acceptable level (which is dictated by applicable Safety and

Health regulations)

2. Do we need to include personnel resumes?

a. Resumes are not required. What is required should be outlined in the Statement of Objectives

(SOO), which can be gone over in greater detail at the site visit.

3. Is a project schedule required with the proposal or after award?

a. Official project schedule is not required; however, timeframes for getting started and completed would be appreciated.

4. Is a draft work plan expected with the proposal? Or is that desired after award?

a. How you plan to proceed in general terms/context would be appreciated, but is not required.

5. Are there any page limitations for the proposal?

a. There are no page limitations to proposal

6. Do we need to include Reps and Certs, or only our DUNS and Cage codes?

a. Reps & certs are obtained through S.A.M., so these do not need to be filled out separately (if not completed as a part of SAM registration, then completing online reps & certs is highly encouraged).

7. Do we need to provide safety information such as OSHA forms?

a. I do not believe any OSHA related forms are required to be submitted (unless something of that nature is called for in the SOO).

8. Does the client need any information on past performance?

a. No past performance information is required; however, there may be a requirement for proof of certification for a certain type of discipline/capability/training, which would be called for in the

SOO.

9. What type of contract is this work being performed? T&M, Cost Plus, FFP?

a. Firm-fixed price is the type of contract that shall be awarded.

10. What is the period of performance for this work?

a. Estimated performance period is 30 days (but that is flexible/not static)

11. Are photographs permitted during the site visit?

a. Yes, photos are authorized (rules for taking pictures are outlined prior to on-site review of facility)

12. Is there a need to expedite the remediation activity?

a. Reasonable start timeframe and progress is expected (not above abnormal expectations)

13. What is the source of the hexavalent chromium contamination?

a. Bead Blasting media that has escaped from the booth enclosure to surrounding surfaces. (site visit will be helpful in further explaining this)

14. Is there any characterization data available regarding the hexavalent chromium?

a. Hexavalent Chromium present in blasting media due to primer coatings on metal surfaces that have been removed in the bead blast booth contained Hexavalent Chromium

15. How long has it been since the contamination event occurred?

a. Discovered October 2015

16. To what extent has contamination migration occurred?

a. Blast media has leaked into rest of building (site visit will help explain)

17. Has any stabilization activities taken place?

a. No

18. Is the facility planned to be returned to service?

a. Yes; however, the current Bead Blast Booth/media will be disposed of post-remediation, and replaced by a new Blast booth/media.

19. Is the facility permitted? Is there an applicable air permit with mandatory controls specified?

a. The 179th Airlift Wing does not hold any Air Permits with Ohio EPA as we are classified as a deminimis source. This includes the Paint Booth/Bead Blast facility. Documentation can be provided by the Environmental Office, if necessary, during the site visit.

20. What is the desired end state? Contamination fixed vs removed?

a. Contamination removed/100% of building interior

21. Has this area been remediated before? If so for what and when?

a. No

22. Can a more specific description of the dry media blast equipment and process be provided?

a. Bead blasting booth within the building. Plastic media is projected at high speed using compressed air from a hand held nozzle to metal piece needing corrosion removed.

23. Has monitoring for loose surface contamination and airborne contamination been performed? Has the perimeter of the work area been verified as clean?

a. Yes, aggressive air sampling was performed for 8 hrs for hex chrome and cadmium. All results below action level. Contamination contained within the building wall.

24. Are the results of any previous personal and environmental sampling efforts available?

a. Yes. Available during site visit.

25. How much residual contaminated DMB material is present within the work area?

a. Site visit will be able to answer this question

26. Can you provide a copy of AWI and EMO policies?

a. EMO policies cover 15 different program/media areas. If the Contractor wishes to specify which particular policies they are concerned with, the EMO would be more than happy to provide what we have on file.

a. The 179th Airlift Wing is part of a Department of Defense waste disposal contract with the

Defense Logistics Agency (DLA). The Contractor cannot utilize this contract for waste disposal.

The Government does not assume any responsibility or liability for waste generated during the course of this project. All waste is the responsibility of the contractor and must be handled and disposed of in accordance with all federal, state and local regulations.

30. SOO: Item 2.1 and Item 3.1: It is WAI’s understanding the remediation work area includes the interior surfaces of Building 403 (the Facility) and the exterior surfaces of the Bead Blast Unit (DMB) and the exterior surfaces of the Paint Spray Booth and exterior surfaces of fixed in-place equipment, including but not limited to Air Compressor, Cabinets, Lockers, Air Dryer, Paint Spray Booth exterior filter media, etc. Is this a correct understanding of the work?

a. Remediation is limited to the interior surfaces of the Facility and exterior surfaces of the equipment inside the facility. The remediation of the interior of the DMB shall be priced as a separate contract option. The interior of the Paint Spray Booth is not within the SOO.

31. Remediation work including the interior surfaces of the existing Bead Blast Unit (DMB), will be priced separate for a contract option. b)The interior surfaces of the existing Paint Spray Booth and fixed in-place equipment including but not limited to Air Compressor, Cabinets, Lockers, Air Dryer, etc., would be considered under this SOO. Is that a correct understanding?

a. (a) The interior of the DMB shall be priced as a separate contract option. b) The interior of the

Paint Spray Booth is not considered as part of the SOO. There is no evidence that the interior of the Paint Spray Booth having been exposed to the Contaminants of Concern (CoC). The exterior of the equipment within the Facility is the primary Areas of Concern (AOC) to include the fixed in-place equipment including, but not limited to air compressors, cabinets, lockers, air dryer, etc.

32. SOO: Item 4.2: Will the OANG maintain the hazardous waste manifest information for the minimum time period required by the Ohio EPA?

a. The Air Force requires installations maintain hazardous waste manifests no less than 50 years, which is well beyond the Ohio EPA requirement. The EMO maintains all manifests for the requisite time.

a. Anticipating award to be made within a week or two after receipt of proposals; notice to proceed would be issued after completion of pre-performance meeting and receipt of any required documentation necessary to be submitted prior to authorization of on-site work.

STATEMENT OF OBJECTIVE – ADDITIONAL GOVERNMENT COMMENTS

1. The OSHA 40-hour HAZWOPER training is required, as referenced in section 3.3.6; proof of training completion will be required as part of contract.

2. The criteria in section 3.2 is determined necessary at this time; you are free to subcontract out that part of the work, if unable to perform in-house. If this is something you are not able to get performed on a contract to OTS, note that in your proposal revision.

3. Sec. 3.5 - Certification by a CIH for Industrial Occupation would suffice. A CIH should be able to make that determination.

4. Sec. 4.1 – Noted on waste disposal responsibility confirmation

5. Sec. 4.2 – see attached documentation; the AWI and EMO policies are referenced, as to what drives the attached documentation to be completed. Any specific guidance on the documentation being completed properly, will be provided in the event a contract is established.

6. Sec. 4.4 - The Installation must abide by all Local, State, and Federal Regulations. In accordance with the

Resource Conservation and Recovery Act, all materials generated by or derived from hazardous wastes must be handled appropriately. Waste materials would include solids and liquids. As such, the appropriate tests must be performed to deem items as non-hazardous prior to disposal. The Installation's sanitary sewer system is serviced by a PTOW (i.e. City of Mansfield) and is not equipped or authorized to handle hazardous waste streams.
