CONTRACTOR

ENVIRONMENTAL REGULATORY REQUIREMENTS

SITUATION/ENVIRONMENTAL REQUIREMENT:
DATE OF COMPLETION:

OEPA Division of Surface Water

The construction project site impacts more than 1 acre:

· Complete a Construction Storm Water Pollution Prevention Plan. This plan must be on hand in the Environmental Office prior to the submittal of the NOI
Submitted to Base EMO 60 days prior to the start of construction for review
· Complete a Notice of Intent (NOI) to OEPA – Central Office along with a payment for processing
Submitted 30 days prior to the start of construction
· Storm Water Pollution Prevention control inspections submitted to the Environmental Office.
Weekly throughout the project duration

A sanitary sewer line is installed that is 6” or larger and/or not gravity fed:

· Complete a Permit to Install (PTI) and all other applicable forms (i.e., Sanitary Sewer Data Sheet and Engineering Plans): http://www.epa.state.oh.us/dsw/pti/index.aspx . Submit to OEPA – Northwest District Office
60 days prior to the start of construction

Two Sanitary Sewer lines are being connected – regardless of size:

· Complete a Permit to Install (PTI) and all other applicable forms (i.e., Sanitary Sewer Data Sheet and Engineering Plans): http://www.epa.state.oh.us/dsw/pti/index.aspx . Submit to OEPA – Northwest District Office
60 days prior to the start of construction

A lift/pump station is installed or replaced:

· Complete a Permit to Install (PTI) and all other applicable forms (i.e., Sanitary Sewer Data Sheet and Engineering Plans): http://www.epa.state.oh.us/dsw/pti/index.aspx . Submit to OEPA – Northwest District Office
60 days prior to the start of construction

A line is installed that is considered a “main” feed over 2 inches:

· Complete a Permit to Install (PTI) and all other applicable forms (i.e., Sanitary Sewer Data Sheet and Engineering Plans): http://www.epa.state.oh.us/dsw/pti/index.aspx . Submit to OEPA – Northwest District Office
60 days prior to the start of construction

Existing lines are replaced with lines of equal size:

· No Paperwork to OEPA is required

Line taps are tied to existing lines:

· No Paperwork to OEPA is required

Water Line Disinfection per the Recommended Standard for Water and AWWA Standard C651 for Disinfection

For Building Construction, Demolition or Renovation Projects:

Fluorescent Bulbs, Light Ballasts, and Mercury Switches:
The Contractor is responsible for recycling these materials with a certified recycler and providing the 179AW with a copy of recycling tickets/records.
Hazardous Materials/Waste:
The Contractor is responsible for the disposal of all hazardous materials/waste in accordance with all Federal, State, and Local Regulations. These items may not be disposed of on 179AW property.
Asbestos:
The Contractor must perform an AHERA asbestos building survey for all suspect ACBM which may potentially be disturbed during the project.
Lead Based Paint (LBP):
The Contractor must perform a LBP survey for any painted surfaces that may potentially be disturbed during the project.
Ozone Depleting Substances (ODSs) (e.g. Freon):
The Contractor is responsible for the disposal of all ODSs in accordance with all Federal, State, and Local Regulations. These items may not be disposed of on 179AW property.
