About this file
This document is a Past Performance Information Sheet template for a federal contract opportunity related to museum services at Scott Air Force Base. The template requires offerors to provide detailed information about three past performance references, including project title, contract value, period of performance, customer/agency contact information, and a comprehensive description of work performed.
The form seeks specific details about project relevance, including scope (barrier systems), magnitude (number of systems, facility size), and complexity (installation challenges, system integration). Offerors must indicate performance assessment types such as CPARS ratings, letters of recommendation, or other customer evaluations. By submitting the form, offerors grant the U.S. Government permission to contact references and verify performance information. This template is part of a solicitation for up to two Indefinite Delivery Indefinite Quantity (IDIQ) contracts for museum/exhibit design firms, specifically focused on the Air Force Cyberspace and Communications (AFCC) Heritage Center at Scott Air Force Base, with a planned 5-year contract duration.
