About this file

The document is a Past Performance Questionnaire (Exhibit N) for Solicitation 36C24524R0089, issued by the NCO5 Network Contracting Office to collect detailed performance information about potential offerors for a Veterans Affairs (VA) leased space contract. The questionnaire is designed to evaluate a contractor's past performance across seven key performance areas: quality of service, schedule/timeliness, customer satisfaction, management/personnel/labor, cost/financial management, safety/security, and general performance. Assessors are asked to rate the contractor's performance using a five-point scale ranging from Exceptional (E) to Neutral/Unknown (N), with the option to provide supplemental comments for any marginal or unsatisfactory ratings.

The solicitation seeks approximately 2,314 ABOA square feet of office space and 24 parking spaces in Leesburg, VA for a VA Readjustment Counseling Service Center. The VA will consider existing buildings, new construction, or multi-tenant buildings, with potential for a build-to-suit option. All vendor questions must be submitted via email to robin.mcdonald3@va.gov by May 9, 2025, at 11:00 AM EST, with no phone calls accepted. The comprehensive questionnaire aims to provide a thorough assessment of a contractor's previous performance to inform the contract award decision.
