About this file

This document is an exhibit detailing mandatory data entry fields for the CDEC Salesforce Database used by the PSSF (Presumably Subsidized/Supportive Services for Families) Program. The file establishes the specific demographic and service information required to be entered into the system, organized sequentially by the steps in which data is collected. Required fields are marked with red asterisks and include household account information such as the primary caregiver's last name, phone number, county, and number of adults and children. Contact records must capture first and last names, gender, birthdates, race/ethnicity, Hispanic/Latinx identification, tribal member status, and primary spoken language for both the primary caregiver and at least one child in the home, with options to decline or select "unknown" for demographic fields. Additional case information includes the case start date, record type designation as "PSSF Case," status, household identification, site, county, and supervisor assignment.

The document further specifies mandatory case activity requirements, including a completed Data System Consent Form prior to entering family information, with required worker or supervisor attestation and signature. CFSA 2.0 Assessments are required for families receiving intensive case management or six or more hours of services within three months, with evaluation dates and baseline, follow-up, or post-assessment designations. Service documentation must include the service type, start date, service category selected from specified options, the individual receiving services, related activities, and end date with closure reasons when applicable. Goal setting activities require a goal name and topic designation, while resource referrals and flex funding documentation are optional. The system accommodates case closure procedures requiring all active services to be closed before the case status is changed, with documentation of closure dates and reasons.
