About this file

This is a Proposer's Disclosure Statement for a state and local contract opportunity involving the Board of Regents (BOR) of Georgia and the University of West Georgia for the J-463 TLC Lecture Hall Renovation and Systems Renewal project in Carrollton, Georgia. The document requires all proposers to submit a completed disclosure statement as part of their proposal submittal to ensure compliance with Georgia state laws, rules, and Executive Orders regarding conflict of interest prevention and procurement integrity. The disclosure statement serves as a mandatory deliverable that must be dated and signed by an authorized representative of the proposing firm.

The disclosure statement requires proposers to provide three specific responses: a description of any business transactions between the proposer's firm and BOR, the Using Agency, or the project end-user within the prior two years; disclosure of any gifts, hospitality, or benefits provided to these entities within the prior one-year period; and identification of any actual or potential conflicts of interest as defined by Georgia standards. The document emphasizes that BOR, as a public agency managing a public project, maintains a strict standard for conflict of interest evaluation based on the perspective of a reasonable person with knowledge of relevant facts. Proposers must certify their responses and include the completed statement with their proposal submittal to be considered for award.
