About this file

This document is Exhibit F from a Request for Proposal (RFP202590000636) issued by West Virginia University (WVU) detailing the standard required documentation and submission requirements for construction proposals. The exhibit outlines comprehensive requirements for contractors, including mandatory documentation to be submitted with the solicitation response and notice of intent to award, such as contractor's licenses, safety evaluations, drug-free workplace affidavits, insurance certificates, performance bonds, and subcontractor lists.

Key requirements include compliance with West Virginia state codes and regulations, such as hiring 75% of workers from the local labor market for projects exceeding $500,000, maintaining an Experience Modification Rate (EMR) of 1.20 or less, and providing various bonds and insurance documentation. Contractors must submit a City of Morgantown Contractor's License (if working within city limits), a State of West Virginia Contractor's License, drug-free workplace conformance affidavits, and other specified documents. The university reserves the right to reject submissions that do not meet these stringent requirements, with potential consequences including forfeiture of security deposits or reduction of contract performance time.
