About this file

This document is an Income Verification Calculation Worksheet developed by the Arizona Department of Economic Security (DES) for use in the Rapid Rehousing (RRH) program administered for Youth & Families. The worksheet serves as an official income certification instrument required to determine eligibility for rapid rehousing assistance under the program.

The worksheet establishes that household income must remain at or below 50% of Area Median Income (AMI) to maintain eligibility, with income verification required after one year of financial assistance. Income calculations utilize HUD income limits in effect at the time of verification rather than limits corresponding to the grant year in which funds were awarded. The document requires providers to calculate total annual household income by multiplying gross amounts by pay period frequency, then sum employment income and income from assets to determine total annual income. Eligible income sources include employment, unemployment benefits, TANF, Social Security benefits, SSI/SSDI, General Assistance programs, VA pensions, retirement and disability benefits, child support, spousal support, school grants, and family support. The worksheet includes a Declaration of No Income section requiring clients to certify under penalty of perjury that they have no income from any source and to provide written verification from third-party sources for any bills or expenses being paid on their behalf. The income calculator tool referenced for calculations is available through the HUD Exchange website.
