About this file
This document is a Shelter Diversion Quarterly Performance Report template issued by the Department of Economic Security (DES) State Homeless Coordination Office. The report form is designed for contracted shelter diversion service providers to submit performance data and narratives on a quarterly basis. The contract term extends through June 30, 2026. The template includes sections for Q1 (July-September), Q2 (October-December), Q3 (January-March), and Q4 (April-June) reporting periods.
The document establishes reporting requirements across three primary sections: Section A captures participant demographics including totals, age ranges, race/ethnicity, and other characteristics such as LGBTQ identification and disability status; Section B requires reporting on program metrics including the number of unduplicated households served, average assistance per household, and specific categories of assistance provided (rent, utilities, relocation, move-in costs, childcare, legal fees, employment expenses, and other diversion-related services); and Section C mandates narrative responses addressing significant diversion activities, participant success stories, contract-enabled capabilities, community partnerships, evaluation participation, and unmet community needs. All data fields and narrative sections remain blank in this template, indicating it serves as an instructional document for providers to complete with actual performance data. The report's stated purpose is to track progress, identify barriers, and assess solutions to support program success rather than penalize providers.
