About this file

This is a Rate Proposal Form (Exhibit E) from the Washington State Department of Corrections for vessel maintenance and dry-dock services. The form is designed to capture unit prices for work not covered by the fixed fee work in the Bid Form (Exhibit D) or change orders, with rates remaining firm for 150 days after receipt.

The form requires contractors to provide unit pricing for various services including daily rates for dry-storage and alongside pier berthing, hourly rates for painting labor, metal fabrication and welding, rigging and lifting, estimation/project management, UT gauging with written reports, and general labor. Additionally, it requests pricing for ABS Grade A or B Steel in 1/4" and 3/16" plate sizes measured by hundredweight (CWT). The form also includes a Time and Material Work section requiring straight time, overtime, and double time labor billing rates per hour. All prices must include support, services, staging, labor, mark-up, material, removal and replacement costs for minor interferences, with DOC reserving the right to have any unit price work performed on a time and material basis instead of the quoted price.

EXHIBIT E

RATE PROPOSAL FORM

The Contractor agrees that the following Unit Prices will prevail in connection with any work required that is not covered by the fixed fee work in the Bid Form (Exhibit D) or as required by change order. Unit Prices shall include all support, services, staging, labor, mark-up, material, removal and replacement of normally experienced minor interferences to complete only that Item. Include only the dry-docking and berthing costs that are in excess of the base Contract. DOC reserves the right to have any of the work in the Unit Price Items performed on a time and material basis in lieu of the quoted price.

The rate proposal is a firm offer for a period of 150 days following receipt, and it may be accepted by DOC without further negotiation (except where obviously required by lack of certainty in key terms) at any time within the 150-day period.
