About this file

This is a Conflict of Interest Questionnaire form (Form CIQ) provided by the Texas Ethics Commission for vendors conducting business with local governmental entities in Texas. The form is required to be completed and filed with the records administrator of the local governmental entity no later than the seventh business day after a vendor becomes aware of facts requiring disclosure. The questionnaire applies to vendors who have a business relationship with a local government entity as defined under Texas Local Government Code Chapter 176 and must be filed when specific conflict of interest conditions are met.

The questionnaire requires vendors to disclose whether local government officers or their family members are receiving or likely to receive taxable income from the vendor, whether the vendor is receiving taxable income from officers or family members outside of the local governmental entity, and whether gifts exceeding $100 in aggregate value have been provided to officers or their families within a 12-month period. Vendors must also disclose employment or business relationships with officers or family members that result in taxable income exceeding $2,500 during the preceding 12-month period, as well as any family relationships with local government officers. The form emphasizes that vendors are prohibited from knowingly violating disclosure requirements, with violations constituting a misdemeanor offense under Texas law. Updated questionnaires must be filed within seven business days of discovering that a previously filed form was incomplete or inaccurate.
