About this file
This is a Small Procurement Contract template issued by the State of Maryland Department of Health for Clifton T. Perkins Hospital Center. The contract pertains to maintenance services in the Dietary area, specifically piping and recirculation pump repair in the central kitchen. The document serves as a template establishing the contractual framework between the state and a contractor, with provisions for clearly defined scope of work, deliverables, and milestones. The contract term is flexible and specified by the parties upon execution, with the effective dates to be inserted at the time of agreement. No specific response dates, site visits, or bidders' meetings are detailed in this template, as it represents a standardized form awaiting completion with project-specific information.
The contract establishes a maximum compensation limit of $50,000, with the exact amount to be determined based on the specific goods or services rendered. Payment shall be made within thirty days of receipt of proper invoices, which must be submitted in triplicate by the 15th of each month for the preceding calendar month. The contractor must include their tax identification number or Social Security number on all invoices. The contract is governed by Maryland State Finance and Procurement Article provisions and includes standard clauses regarding non-discrimination, anti-bribery compliance, termination for convenience or default, and nonappropriation provisions. Victoria F. Bell is designated as the Procurement Officer responsible for all Department-Contractor communications. Maryland law governs the interpretation and enforcement of this contract, and any disputes shall be resolved in accordance with COMAR 21.10 Administrative and Civil Remedies.
