About this file

This is a Bid Form (Exhibit B-1) for a contract opportunity issued by Clifton T. Perkins Hospital Center in Maryland to repair damaged piping and a recirculation pump in the facility's central kitchen dietary area. The solicitation encompasses four distinct line items: ceiling access and restoration, piping and insulation repair, recirculation pump replacement, and roof drain pitch pocket repair. Bidders must submit comprehensive pricing that includes all materials, labor, and associated costs for each line item, with the total contract award not to exceed $50,000. The bid form requires all unit prices and extended prices to be entered in dollars and cents with clear decimal points, and all blanks must be completed without alterations or contingencies that could render the bid non-responsive.

All pricing submitted must be fully loaded to include labor, profit, overhead, general operating expenses, administrative costs, and all other expenses necessary to perform the work as specified in the solicitation. Bidders are instructed that sample amounts used for bid calculations are estimates for evaluation purposes only, and the State does not guarantee minimum or maximum usage levels. The bid form must be signed and dated by an authorized representative and include company information, eMMA number, FEIN, CSB certification status, and any MBE/DSB or VSB certifications. Failure to adhere to the bid instructions, including completing all required fields and avoiding unauthorized amendments, may result in the bid being determined non-responsive.
