About this file
This is an insurance requirements exhibit for the County of Volusia, Florida, detailing mandatory coverage for the Tomoka Farms Road Landfill (TFRL) Leachate Evaporation System Demonstration Project. The project involves furnishing and installing leachate evaporation systems in two existing storage ponds at the landfill, designed to achieve an average evaporation rate of 100,000 gallons per day over a six-month performance period. The contractor must submit an approved evaporation system performance measurement, monitoring, and reporting plan within 30 days of the Notice to Proceed. The system may be temporarily leased or installed on a lease-to-own basis depending on performance and County requirements.
The contractor must maintain comprehensive insurance coverage including Workers' Compensation with Florida statutory limits and waiver of subrogation in favor of the County; Commercial General Liability with limits of $1,000,000 per occurrence and $2,000,000 general aggregate; Commercial Auto Liability with $1,000,000 combined single limit; Excess Liability with $1,000,000 per occurrence and aggregate; and Contractors Pollution Liability with $2,000,000 per incident and aggregate. All policies must name the County as an additional insured on a primary and non-contributory basis with waivers of subrogation. The contractor, its subcontractors, and independent contractors are collectively responsible for maintaining these coverage requirements. All insurance policies require thirty days' written notice of cancellation, with certificates of insurance directed to the County of Volusia Purchasing & Contracts Division, Attention: Christy Welch, Risk Management Division, at 123 W. Indiana Avenue, Room 302, DeLand, FL 32720.
