About this file
This is a Prime/Subcontractor Form (Exhibit B) for ITB No. 26-B-55JS, Contract No. C-2074, submitted to an unnamed county in Florida for the DeLand Regional Library Play Yard Walkway project. The form requires bidders to identify all proposed subcontractors and their respective scopes of work, with specified disciplines including earthwork, concrete, fencing, doors and hardware, and security access control. The prime contractor must complete the form identifying the dollar value and percentage of work to be performed by the prime contractor and each subcontractor, with all entries totaling 100 percent. Failure to complete and return this form results in a non-responsive bid. The prime contractor is responsible for ensuring all subcontractors hold appropriate licenses, and any changes to subcontractors during project completion require prior approval from the County Project Manager. The County reserves the right to seek clarification on subcontractors but will not allow changes to the total submitted base bid pricing.
The DeLand Regional Library Play Yard Walkway project is a federally assisted initiative funded in part through Community Development Block Grant and Volusia ECHO Grant funding. As a federally funded project, it is subject to Davis-Bacon Act prevailing wage requirements, the Copeland Act anti-kickback provisions, weekly certified payroll reporting, bonding requirements per 24 CFR 85.36, and Section 3 and Minority/Women-Owned Business Enterprise (M/WBE) compliance. The prime contractor bears responsibility for wage compliance enforcement and supporting documentation throughout the project duration and may be held liable for wage restitution. The project scope includes construction of an ADA-compliant outdoor playground with both grass and paved play areas with furnished equipment for ages 2-12.
