About this file
This is a Past Performance Questionnaire template designed to evaluate contractor performance on past contracts. The questionnaire is used by the Department of Agriculture Rural Housing Service for the solicitation 12SAD226Q0003, which seeks Judicial and Non-Judicial Foreclosure Legal Services and Related Technical Support for North Carolina, South Carolina, and Arkansas. The form is completed by references familiar with the offeror's prior work and collects structured evaluations across multiple performance dimensions using a five-point rating scale: Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory.
The questionnaire evaluates two primary performance categories: Timeliness of Performance and Quality Control/Customer Satisfaction. Under Timeliness, references assess the contractor's resource management and ability to meet schedules. Under Quality Control, the form measures the effectiveness of quality assurance procedures, responsiveness to changes and corrective actions, and overall professionalism and cooperativeness. The template requires references to provide the contractor's company name, contract details, period of performance, and contract value. References must provide written comments for any Marginal or Unsatisfactory ratings and answer whether they would engage the firm again. This is a standard evaluation tool used in source selection but does not address specific products, services, response dates, or pricing terms.
