About this file

This is a Fee Schedule exhibit (Addendum 2) submitted as part of the City of Phoenix's Request for Proposal PS-26-0128 for Occupational Medicine Staffing Services and Electronic Health Records Management for the Phoenix Fire Department Health and Wellness Center. The exhibit establishes the pricing structure for 25 full-time equivalent positions across medical, nursing, behavioral health, rehabilitation, and administrative disciplines, including medical doctors, physician assistants, nurse practitioners, registered nurses, licensed physical therapists, athletic trainers, and support staff. Offerors must provide fully burdened annual salaries for each position based on estimated annual hours, with separate pricing for positions meeting minimum qualifications and those meeting both minimum and preferred qualifications. The positions include specialized roles such as a Board Certified Cardiologist, Board Certified Dermatologist, Director of Rehabilitation Services, and various nursing and therapy specialists. The sealed proposal deadline is March 4, 2026, at 2:00 p.m., with a non-mandatory pre-offer conference and site visit scheduled for February 5, 2026, at 150 S. 12th Street, Phoenix, Arizona 85034. The contract commences October 1, 2026, for an initial three-year term with two optional one-year extensions.

The fee schedule requires offerors to include direct and indirect costs encompassing labor, overhead, administrative charges, licensing, continuing education, profit, insurance, transportation, and taxes in their fully burdened salary proposals. Additionally, Table B of the exhibit requires all-inclusive, not-to-exceed pricing for electronic health records management system implementation, including initial records transfer costs (one-time), ongoing annual IT maintenance costs, and final records transfer costs upon contract termination. All pricing must remain firm and fixed for 240 calendar days from solicitation opening. The solicitation is subject to Disadvantaged Business Enterprise requirements under 49 CFR Part 26, mandating contractors to develop Small Business Participation Plans and conduct good faith outreach to DBE-certified firms. Contractors must obtain a 10% performance surety bond and carry professional liability insurance with $1,000,000 per claim and $2,000,000 aggregate coverage, along with technology errors and omissions coverage and network security and privacy liability insurance.
