The project is located on the Fermi National Accelerator Laboratory (Fermilab) Site, adjacent to the city of Batavia, Illinois. The project is situated at the former Central Utility Building (CUB) tile field located within the Tevatron ring. The CUB tile field area is identified as Solid Waste Management Unit (SWMU) 12. The proposed project area is depicted on Figure 1. The site can be accessed via the entrance gate on Wilson Road.

Drilling locations in the CUB tile field are accessed via asphalt and gravel roads, as well as two track pathways near the monitoring wells.

1.5 DESCRIPTION OF WORK

The Subcontractor shall furnish all required supervision, labor, materials, tools, plant and appurtenances necessary to perform diligently and fully all work as described in the drawings and technical specifications. All work includes transportation, overhead, bonding costs, safety oversight, quality control oversight, and supervision as required to construct the project described in the drawings, technical specification and the detailed description contained herein.

The Subcontractor’s scope of work is not limited to the general work outline described below. The Subcontractor shall perform all work required to complete the construction work in accordance with the drawings and/or specifications. The description and quantities listed below are general in nature and are only intended to describe the range and complexity of this scope of work. The description and quantities provided below are not to be used as the basis for establishing a cost proposal. Specific quantities and definitions of the scope of work for providing a proposal shall be based solely on estimates developed by the Offer from the drawings, specifications, and the information obtained from examining of the project site.

The scope of work consists of all labor, equipment, transportation, overhead, bonding costs, safety oversight, quality control oversight and supervision as required to mobilize, properly seal and abandon existing monitoring well MW-D1, drill a new borehole for installation of groundwater monitoring well MW-D7, decontaminate downhole equipment between boreholes, restore site, and produce material summary forms and County water well sealing form. The location of MW-D1 and proposed location of MW-D7 are depicted on Figure 2.

GENERAL REQUIREMENTS Project No. XXXXXXX 010010 - 4

The project consists of work to seal and abandoned an existing monitoring well (MW-D1) and install a replacement with similar construction (MW-D7). Based on recent groundwater elevation and chemical analytical data collected from the well, it appears the seal and secondary casing of the existing well is not working effectively.

Consequently, the groundwater elevation data for this deep till well is more closely aligned with nearby monitoring wells installed in the shallow till horizon.

Monitoring well MW-D1 will be sealed and abandoned in accordance with Title 77 of Illinois Administrative Code (IAC) Section 920.120. The entire MW-D1 monitoring well must be overdrilled to ensure the secondary PVC casing and entire well construction to depth is removed. The resulting borehole will be sealed with neat cement grout or bentonite product from the bottom up tremie using a tremie pipe method (or similar) to prevent bridging. The soil boring log and monitoring well construction diagram for MW-D1 are included as Figure 3.

Following sealing and abandonment of MW-D1, the replacement well (i.e., MW-D7) will be installed within 10 feet on the west side of the original well. Fermilab will determine the actual locations of the borings/wells based on utility and operational clearance. The replacement well will be constructed in a similar manner to MW-D1 including use of a secondary casing; however, some changes in well materials and diameter will be made.

A secondary steel casing will be keyed into the silty clay unit previously observed between approximately 16 and 34 feet below grade. The diameter of the steel casing will be determined by the driller but will be no less than six inches. The purpose of the secondary casing is to prevent cross-communication between the shallow and deep till aquifer units. Following the installation and setting of the secondary casing, a 2-inch diameter PVC monitoring well will be installed within the casing. The 5-foot long slotted screen will be installed between approximately 35 and 40 feet below ground surface.

Groundwater samples recently collected from MW-D1 were analyzed for total lead and total chloride. Lead was not detected in the samples at concentrations above the Class II groundwater quality standard (0.1 mg/L). Chloride, however, was detected at concentrations exceeding the quality standard for Class II groundwater (200 mg/L), up to approximately 1,300 mg/L. Supplemental personal protective equipment (PPE) such as nitrile gloves and safety glasses) must be worn in addition to the standard drilling PPE by drilling and sampling personnel. Following well development by the subcontractor, Fermilab personnel will collect samples and will submit them to Fermilab's laboratory subcontractor.

Furthermore, this scope of work will require:

1. All subcontractor personnel working on-site must attend subcontractor orientation training, which will take approximately 30 minutes.

2. Drilling a vertical borehole and installing a monitoring well within a 10-foot radius of the original well. An appropriate drilling method (e.g., hollow-stem auger or mud rotary) will be employed to install the steel casing and 2-inch diameter PVC wells and slotted screens with sufficient sand pack for collection of representative groundwater samples.

a) Sediment samples for logging purposes in the well borehole will be collected with standard split spoons, direct push tooling, or similar equipment, starting continuously at a depth of approximately 32 feet below ground surface to

GENERAL REQUIREMENTS Project No. XXXXXXX 010010 - 5 confirm that MW-D7 is monitoring the same material as described in the original well boring log.

b) The new monitoring well must be sealed above the sand pack with hydrated granular bentonite plug and then bentonite grout. A tremie pipe or rod must be used to ensure that no bridging occurs in the seal. Well construction must be finished with a lockable protective stickup cover secured with a concrete surface pad approximately 2 feet by 2 feet laterally, extending to a depth of at least two feet in the borehole annulus and tapering to at least 4 inches thick at the edges. A 2-foot diameter Sonotube® concrete form is also acceptable if it meets the two-foot depth requirement listed above.

3. Subsurface utilities are not anticipated within the CUB tile field area; however, Fermilab will conduct JULIE clearance and mark estimated field locations immediately prior to drilling/well installation work.

4. The new monitoring well shall be developed no sooner than 24 hours following well completion. Development specifications are included in the appendix to the attached Groundwater Management Plan.

5. Subcontractor will provide an estimate of the anticipated volume of solid and liquid investigation-derived waste (including decon water) and Fermilab will furnish the disposal containers. Fermilab will stage the disposal containers adjacent to the SWMU 12 work area; Subcontractor is responsible for transporting the investigation-derived waste to the containers.

6. The Health Department sealing form shall be completed by the subcontractor and submitted to the Fermilab ESH Section within 7 days of completion of the well sealing.
