About this file

This document is an Exhibit A Statement of Work (SOW) for Prescription Safety Eyewear issued by Colorado Springs Utilities, a four-service municipally owned utility. The solicitation (RFP-YK-006732) seeks a qualified contractor to provide comprehensive prescription safety and computer eyewear services for utility employees. The scope requires the supplier to offer safety eyewear meeting ANSI Z87 specifications, with permanent side shields, and a broad selection of frame styles ranging from premium to reduced-cost options. The contractor will be responsible for administering an employee eyewear program, providing local access for try-ons and maintenance, potentially offering an online ordering platform, and ensuring clear communication about pricing and out-of-pocket expenses.

The contract emphasizes employee safety and protective equipment, with specific requirements that the supplier provide quality materials under warranty and maintain excellent customer service. While the utility will provide vouchers for eyewear, employees will be responsible for their own eye examinations and prescription costs. The supplier must submit required reports, communicate about new technologies, and follow the utility's invoicing and payment procedures. No specific contract term, pricing details, or award dates are explicitly stated in the document, but the focus is on establishing a comprehensive eyewear service program that meets the utility's safety standards and employee needs.
