About this file
This is a Statement of Work (SOW) for HealthySteps Implementation Sites prepared by the Colorado Department of Early Childhood (CDEC). The document outlines the implementation requirements for the HealthySteps program, an evidence-based model providing enhanced well-child care to children ages birth to three years and their families within pediatric primary care settings. The contract period runs from July 1, 2026, through June 30, 2027, with services to be performed at a clinic location to be determined. The scope of work requires implementation of the HealthySteps model to a minimum of "Approaching Basic Fidelity" as defined by the HealthySteps National Office, with an embedded HealthySteps Specialist delivering services to approximately a specified number of families (exact numbers to be inserted site-specifically) across three service tiers: universal screening for all eligible children (Tier 1), brief developmental and behavioral consultations for families with typical concerns (Tier 2), and team-based well-child visits for families with the greatest need through age three (Tier 3).
The work plan encompasses multiple key activities including universal developmental, social-emotional, and autism screening; maternal mental health and family needs screening; establishment of a functional warm line for families; provision of developmental and early learning resources; care coordination and referrals to community resources; developmental and behavioral consultations; comprehensive data collection and reporting to both the State Intermediary Office (Assuring Better Child Health and Development) and the National Office; and mandatory participation in professional development, technical assistance, and community of practice activities. Specific milestones include achieving defined numbers of children served in Tiers 2 and 3 annually, increasing social-emotional screening rates, ensuring 100 percent contact of identified families within three days, and providing positive parenting and early learning guidance at all team-based well-child visits. All deliverables are subject to approval by the CDEC HealthySteps Program manager, who will monitor completion and compliance throughout the contract period.

INTRODUCTION/BACKGROUND
● HealthySteps (HS) is an evidence based model that provides enhanced, comprehensive well‐child care to children ages birth to three years and their families by promoting the establishment of close relationships between healthcare professionals and parents in addressing the physical, emotional, and intellectual growth and development of young children (www.healthysteps.org). HS is delivered in the context of pediatric primary care and is multi-generational and prevention focused. The reach of HS goes beyond the children enrolled and reaches the whole family. The long-term goals of the HealthySteps® program are healthy children who succeed in school and life. These outcomes are achieved by targeting child health, parenting knowledge and practices, and family health and well-being.
● [Insert site specific background here such as clinic’s experience implementing HS or other integrated behavioral health strategies.]
