This is a Statement of Work (SOW) issued by the Colorado Department of Public Health and Environment (CDPHE), Hazardous Materials and Waste Management Division, for waste tire removal services at the Saguache County Landfill in Moffat, Colorado (Project Number WT-105). The contractor is required to remove all illegal waste tires and tire bales from the property located at 25125 Kelly Road, Moffat, CO 81149, to mitigate fire risks and prevent mosquito-borne disease transmission. Primary deliverables include documentation through digital photography taken before, during, and after removal activities from substantially the same locations, completion of Form WT-2 (Uniform Waste Tire Manifest) for each load removed, and certified weight tickets for all hauled materials. All waste tires and tire bales must be transported to a designated waste tire destination facility and must be completed no later than the contract expiration date.
The contractor must be registered as a Waste Tire Hauler with CDPHE's Hazardous Materials and Waste Management Division prior to and throughout the contract period. All collected waste tires and tire bales must be hauled to a registered destination facility that will use 100% of the materials for beneficial purposes such as alternative daily cover, tire-derived fuel, tire-derived aggregate, landscape mulch, or molded products; disposal, retread sales, or non-beneficial uses are prohibited. The contractor must comply with Colorado's Solid Waste Act and Regulations, the Colorado Measurements Standards Act, and must monitor CDPHE websites for regulatory updates throughout the contract term. The contractor is prohibited from bringing additional waste materials onto the property and must notify the HMWMD Project Manager of any delays encountered during the cleanup project.
