Block 4:

Paragraph 2 Format: RFV shall be submitted using DD Form 1694.

Block 8:

Approval or rejection shall be granted within five calendar days or three business days. If the Government rejects, comments will be provided; the contractor shall update the RFV and resubmit within three business days. The Government will approve or reject within 10 business days, based on comments being addressed.
