Exhibit 17.1 Geographic Service Areas_02.10.2026.xlsx

This exhibit defines the geographic service areas for a Specialty Managed Care Plan (SMCP) contract with the State of Nevada, effective from July 1, 2027 through December 31, 2030. The SMCP will operate in two phases, with the first phase serving Urban Clark and Urban Washoe ZIP codes, and the second phase expanding to include Rural areas. The document provides a comprehensive listing of Nevada ZIP codes organized by locality and county, designating each area as either Urban Clark, Urban Washoe, or Rural, with further network adequacy classifications ranging from Urban to Frontier designations. The service areas span multiple Nevada counties including Clark, Washoe, Nye, Lincoln, Esmeralda, and numerous others, encompassing both metropolitan areas such as Las Vegas, Henderson, Reno, and Sparks, as well as rural and frontier communities throughout the state.

The exhibit also establishes out-of-state catchment areas for general Medicaid network adequacy purposes in Arizona, California, Idaho, and Utah, though these ZIP codes are not included in the SMCP service area and do not determine SMCP enrollment. The geographic designations for network adequacy appear to serve as benchmarks for evaluating the adequacy of provider networks within each service area, with designations reflecting the population density and service availability characteristics of each region.
