Summary: Nevada RFP 41NHA-S3708 - Specialty Managed Care Plan
This Request for Proposals (RFP) document is issued by the Nevada State Purchasing Division on behalf of the Nevada Health Authority's Division of Nevada Medicaid, seeking a qualified vendor to provide specialty risk-based capitated managed care services for children and young adults with complex behavioral health needs and those involved with the child welfare system. The solicitation (41NHA-S3708) was released on February 12, 2026, with a submission deadline of May 1, 2026, at 2:00 P.M. Pacific Time. The State intends to award one contract, with an estimated contract start date of September 8, 2026, following Board of Examiners approval. The resulting contract will operate from July 1, 2027, through December 31, 2030 (three and one-half years), with an option for the State to extend an additional two years. Initial mandatory service areas include urban Washoe and Clark counties, expanding to all counties statewide beginning January 1, 2030, though bidders may opt to expand earlier to January 1, 2029. The evaluation period is estimated from May 1, 2026, through June 5, 2026, with vendor presentations potentially scheduled for June 12, 2026, and a Notice of Intent issued on or about June 13, 2026.
All costs associated with providing covered Medicaid and Nevada Check Up services under the specialty plan—including medical, behavioral health, pharmaceutical, and transportation benefits—are to be included in fixed, actuarially sound capitation rates that will be finalized in a contract amendment through collaboration between State and Bidder actuaries. Vendor responses are evaluated on a Pass/Fail basis for most components, with the exception of Technical Proposal Questions, which are scored across five topic areas: Overall Experience (200 points, 20 percent weight), Care Management (250 points, 25 percent weight), Provider Network (250 points, 25 percent weight), Service Delivery (100 points, 10 percent weight), and Complex Case Scenarios (200 points, 20 percent weight), with possible bonus points available. Vendors must possess or obtain within one year of contract implementation a Certificate of Authority issued by the Nevada Division of Insurance and NCQA accreditation. Submission requirements include a title page, table of contents, vendor information responses, attestations regarding minimum requirements, responses to mandatory and critical items, technical proposal answers, and confidential business information including financial statements and Dun and Bradstreet numbers; client references must be submitted directly by the referencing clients to the single point of contact.

Project overview
The State of Nevada Purchasing Division, on behalf of the Division of Nevada Medicaid within the Nevada Health Authority is seeking proposals from qualified vendors to provide specialty risk-based capitated Managed Care services. This procurement supports the Social Security Act Title XIX (Medicaid) and Title XXI State Children’s Health Insurance Program (CHIP, also known as Nevada Check Up) medical assistance programs, as described below and in the scope of work and attachments.
The State intends to award one (1) contract in conjunction with this request for proposals (RFP), as determined in the best interests of the State. The plan is designed to improve access to comprehensive physical, behavioral health, and pharmacy Medicaid Covered Services for children and young adults with Behavioral Health Disabilities and those who are involved with the Child Welfare System. Nevada Medicaid shall administer contract resulting from this solicitation. The resulting contract is expected to be for a contract term of three and one half (3 ½) years, subject to Board of Examiners approval. The State reserves the right to extend the contract by up to two (2) additional years.
Contracting agency background
The State works in partnership with the Centers for Medicare & Medicaid Services (CMS) to assist in providing quality medical care for eligible individuals and families with low incomes and limited resources. The State recognizes that Medicaid-eligible youth with complex behavioral health needs require a specialized, coordinated, and trauma-informed approach.
The State administers the Medicaid and Nevada Check Up Programs in accordance with the applicable Medicaid and CHIP State Plans, applicable Section 1115 and 1915 federal waivers, all applicable United States Code, Code of Federal Regulations (CFR), Nevada Revised Statutes (NRS), Nevada Administrative Code (NAC), the Medicaid Services Manual (MSM) and the Medicaid Operations Manual (MOM). The State may adopt such regulations and policies as deemed necessary and may amend the Medicaid or CHIP State Plans.
The State operates a fee-for-service (FFS) system and managed care delivery system. The State is responsible for monitoring and oversight of Nevada’s Medicaid and Nevada Check Up managed care program, FFS delivery system, and services provided through the Dental Benefits Administrator and Non-Emergency Transportation (NEMT) broker programs.
Currently, the State’s Standard Medicaid managed care program serves enrolled children, parents and adults without children who live in two of the State’s most populous counties—urban Washoe and Clark Counties. Effective January 1, 2026, the Standard Managed Care program will be available in all counties in the state as a result of contracts with five (5) Managed Care Organizations (MCOs) that were selected through a competitive procurement process in 2024. The MCOs are responsible for covering Medicaid and CHIP State Plan medical benefits, including behavioral health services and prescription drugs for individuals who are enrolled in an MCO. The MCOs must also offer additional benefits unique to managed care, such as Member services and care management. The current and past Contracts from the last contract cycle can be accessed in the RFP Resource Library described in RFP Section 2.7—Resource Library.
The eligibility and enrollment functions for the Medicaid and Nevada Check Up programs are the joint responsibility of the Divisions of Nevada Medicaid (DNM), Consumer Health Services (DCHS), Child and Family Services (DCFS), and Supportive Services (DSS). DSS determines Medicaid Member eligibility for the Federal Medical Coverage (FMC) programs and the Nevada Check Up program and aged out of foster care coverage (AO) group. DCFS determines Medicaid Member eligibility for the FMC programs for children and young adults eligible through Title IV-E of the Social Security Act. DCHS leads all Medicaid eligibility policy development and oversight.
Children currently in the custody of the state’s child welfare agency and former child welfare-involved children who receive an adoption subsidy are covered by Medicaid up to age 21. Young adults who were in the custody of a state’s child welfare system and aged out are also eligible for Medicaid up to age 26. Clark and Washoe Counties provide case management services and oversight for these populations and DCFS provides case management services for all other counties in the State, which are primarily rural and frontier areas. Nevada Medicaid reimburses DCFS and Clark and Washoe Counties for targeted case management services as described under Nevada’s Medicaid State Plan.
Other state agencies, including DCFS, the Nevada Division of Public and Behavioral Health (DPBH), Aging and Disability Services Division (ADSD), the Department of Education (NDOE), and the Department of Corrections (DOC) assist with the administration of various programs that serve youth across Nevada. Individuals in the Medicaid and Nevada Check Up programs enrolled with an MCO may also receive services delivered in coordination with one or more of these agencies or their local counterparts.
Nevada Specialty Managed Care Plan Description
The Specialty Plan will serve a specific child and young adult population (Enrollees) as described in the Scope of Work (SOW) in all counties of the state through a single, integrated managed care plan that includes all covered medical, behavioral health, transportation, and pharmaceutical benefits The geographic scope of this contract will initially be focused on the urban regions of the state and will scale to statewide during the course of the contract.
This Specialty Plan is a core component of the Settlement Agreement reached between the State and the United States Department of Justice on January 2, 2025, and is defined in Section II, Paragraph X of the Agreement. The Specialty Plan shall further the State’s goals of increasing access to behavioral health services delivered in home and community settings and minimizing the utilization of institutional care where possible.
Eligible populations for this plan include youth with Behavioral Health Disabilities and youth engaged with the child welfare system. This includes youth with dual diagnoses of intellectual or developmental disabilities, substance use disorder, and other complex medical health needs. Eligibility for the plan, as specified in the Scope of Work, is broader than the Focus Population in the Settlement Agreement to align with the State’s behavioral health transformation goals.
Goals and Objectives
The primary goals of this RFP and Specialty Plan are to:
Improve overall health outcomes for Enrollees;
Support Enrollees living at home with their family or caregiver in their communities; and Ensure compliance with the DOJ Settlement Agreement.
Nevada Medicaid conducted extensive outreach in preparation for this RFP, including public workshops, stakeholder meetings and interviews, a request for information from interested parties, and other outreach activities. This feedback informed the development of the program objectives and State’s final RFP.
In October 2024, the State launched a Children’s Behavioral Health Working Group, with representatives selected with lived and professional experience navigating or providing care to children and youth in Nevada’s behavioral health system. These representatives include youth and biological, foster, and kinship family representatives. This Working Group has met monthly to review and provide input on benefit design, care management models in the Specialty Plan, screening processes, and other crucial topics in the transformation project. All materials from the Working Group can be found on the Children’s Behavioral Health Transformation website.
In April 2025, the State issued a Request for Public Input designed to seek public input and feedback on the Specialty Plan procurement. This request solicited feedback on six specific areas: Targeted Case Management, Care Management Approach, Benefit Set, Provider Network, Cross-System Collaboration, and Managed Care Pitfalls and Accountability Safeguards. The Request for Public Input was shared at the Working Group and via Medicaid communications channels and was open to any interested party, including (but not limited to) potential Vendors, Medicaid Members, Medicaid Member family members, caregivers, and advocates, community-based organizations, county governments, medical and behavioral health providers, and associations representing providers. Over twenty responses were received and feedback was incorporated within the RFP. A summary of this input can be found on the Transformation website.
In September 2025, Nevada Medicaid also hosted a summit discussion to solicit feedback from Youth and Family Working Group representatives on the Specialty Plan design, particularly regarding the care management model. Nevada Medicaid also conducted individual meetings with county partners, advocacy groups, and other community organizations to solicit detailed feedback and discuss Public Input responses to inform the development of the State’s final RFP.
Through these stakeholder engagement activities and in alignment with the Settlement Agreement, the State has identified the following objectives for this procurement and the new Specialty Plan:
Comply with the Americans with Disabilities Act by ensuring services, programs, and activities offered by the State will be provided in the most integrated setting appropriate to meet Enrollees’ needs.
Prevent Enrollees from being needlessly removed from their family home to obtain treatment, to prevent those Enrollees from unnecessarily entering hospitals and residential treatment facilities, and to transition Enrollees who have been placed in these settings back to their family homes and communities with needed services, when appropriate.
Develop a strengths-based system of community-based services that effectively engages families in service planning and ensures coordinated and family-centered care so that Enrollees can receive flexible services and supports that are of sufficient intensity, duration, and quality to meet their behavioral health needs.
Ensure Enrollees receive sufficient services to prevent unnecessary institutionalization and, where possible, remain living at home with their biological, foster, or kinship care family, as applicable, support success in school, enhance their community living skills and resiliency, and contribute toward the development of the skills to function independently upon reaching adulthood.
Develop Specialty Managed Care Plan policies, procedures, and services that will be family-driven, Enrollee-guided, and culturally and linguistically competent, and to ensure that services provided will be individualized and strengths-based.
Specialty Managed Care Plan Contract Information
Any Contract(s) resulting from this RFP will be effective from July 1, 2027, to December 31, 2030, subject to the Board of Examiners’ approval, with the possibility of a two (2) year extension if in the best interest of the State.
The mandatory service areas under the resulting Contract include Urban Washoe and Urban Clark counties from July 1, 2027 to December 31, 2029. Beginning January 1, 2030, the mandatory service areas expand to all counties in the State including urban, rural, and frontier counties. However, Bidders have an option to expand to all counties as early as January 1, 2029. Medicaid and Nevada Check Up will have catchment areas in Arizona, California, Arizona, Idaho, and Utah, which are treated the same as in-state, as further described in Attachment E—Scope of Work.
Consistent with its mission, and the goals of the Settlement Agreement, Nevada Medicaid is continuously improving and enhancing the State’s Medicaid Program to improve clinical outcomes for youth with complex behavioral health needs. Nevada Medicaid currently is working to establish new behavioral health services that will require approval from CMS; once approved, those new services will be reflected in the State’s Medicaid Service Manual. In addition, Nevada Medicaid is working on operational refinements to managed care operations, such as establishing standards for prior authorizations and value-based payment model outcome measurement. See Attachment I—Planned Medicaid Policy Updates Memo for a list of anticipated Medicaid policy and operational updates that are expected to go live before or during the Contract resulting from this RFP.
Any new services and Medicaid populations that may be included under the Specialty Managed Care program during the term of the Contract are to be considered covered for this RFP. If the State expands Covered Services or Medicaid populations, submission of a Proposal to this RFP indicates the Vendor’s willingness to cover those services and/or populations. If necessary, the State will adjust the Capitation Rates in accordance with Actuarial Soundness requirements at the time of the change.
The Contractor must adhere to all authorities including Titles XIX and XXI of the Social Security Act and the implementing regulations at 42 CFR 438 and 42 CFR 457, Medicaid and CHIP State Plans and any amendments thereto, applicable Section 1115 and 1915 federal waivers, Code of Federal Regulations, Nevada Revised Statutes (NRS), Nevada Administrative Code (NAC), and the Medicaid Services Manual.
RFP Resource Library
The State has established an RFP Resource Library, which may be accessed on the State’s website. A link will be provided via an addendum to the RFP.
The RFP Resource Library contains reference materials intended to assist Vendors to prepare a response to this RFP, including, but not limited to, policies and procedures, key forms, and a data book with information on Members.
Vendors are responsible for reviewing the contents of the RFP Resource Library as if the materials were printed in full herein. The State may continue to update the materials in the RFP Resource Library after this RFP is released; however, Vendors will be evaluated based upon the content contained in the documents as of the day the State issues responses to submitted questions (see RFP Section 5 —Timeline).

Standard form contract. The State strongly prefers vendors agree to the terms of the attached standard form contract as is. Ability to agree to contract terms is a high priority to the State. Vendors who cannot agree to the contract as is must include a redlined Word version of the attached standard form contract with their response, with comments justifying the benefit to the State for each requested change. To the extent a vendor has prior contractual dealings with the State, no assumption should be made that terms outside those provided herein have any influence on this project.
Indemnification. Required contract terms on Indemnification: "To the fullest extent permitted by law, Contractor shall indemnify, hold harmless and defend, not excluding the State’s right to participate, the State from and against all liability, claims, actions, damages, losses, and expenses, including, without limitation, reasonable attorneys’ fees and costs, arising out of any breach of the obligations of Contractor under this contract, or any alleged negligent or willful acts or omissions of Contractor, its officers, employees and agents. Contractor’s obligation to indemnify the State shall apply in all cases except for claims arising solely from the State’s own negligence or willful misconduct. Contractor waives any rights of subrogation against the State. Contractor’s duty to defend begins when the State requests defense of any claim arising from this Contract."
Limited liability. Required contract terms on Limited Liability: "The State will not waive and intends to assert available NRS Chapter 41 liability limitations in all cases. Contract liability of both parties shall not be subject to punitive damages. Damages for any State breach shall never exceed the amount of funds appropriated for payment under this Contract, but not yet paid to Contractor, for the Fiscal Year budget in existence at the time of the breach. Contractor’s tort liability shall not be limited."
Insurance schedule. The State strongly prefers vendors agree to the terms of the attached insurance schedule as is. Vendors who cannot agree must explain which areas are causing non-compliance and attach a redline if necessary. Awarded vendor shall maintain, for the duration of the contract, insurance coverages as set forth in the fully executed contract. Work on the contract shall not begin until after the awarded vendor has submitted acceptable evidence of required insurance. Failure to maintain any required insurance coverage or acceptable alternative method of insurance shall be deemed a breach of contract.
