About this file

This is a Past Performance Questionnaire for a Department of Veterans Affairs real property lease solicitation (RLP #36C24W25R0011).

The questionnaire is designed to evaluate prospective landlords/lessors competing for lease award, not brokers facilitating the transaction. Offerors must complete Section 1 with their company information and indicate whether they have relevant past performance. If an offeror claims no relevant past performance, submission of the blank form satisfies the requirement. If relevant past performance exists, the offeror must distribute copies to three current or former tenant references who independently complete Sections 2 and 3, then return the completed questionnaires to the offeror for inclusion in the proposal. Failure to submit all completed questionnaires results in a non-responsive offer.

Section 2 requires evaluator information including name, title, contact details, lease square footage, organization, and space use classification (office, medical office, or other). Section 3 contains eight evaluation criteria assessed on an Acceptable/Not Acceptable basis: (1) lessor's ability to handle change order pricing during tenant improvement buildout; (2) ability to maintain schedule during buildout; (3) ability to stay within budget during buildout; (4) prompt response to maintenance issues; (5) prompt response to safety issues; (6) prompt response to security issues; (7) adequate cleanliness of building premises and common areas; and (8) overall acceptable performance during lease term. Each criterion includes space for explanatory comments. Responses are treated as confidential and procurement sensitive. Contact information for the Lease Contracting Officer, Mati Weirderpass, is provided for assistance.

DEPARTMENT OF VETERAN AFFAIRS

PAST PERFORMANCE QUESTIONNAIRE

Introduction:

This questionnaire is designed to collect past performance information on Offerors (Landlord/

Lessors) competing for award of this lease. Please note this questionnaire is not based on the past performance of brokers facilitating the deal, but rather for the prospective awardees that will be responsible for the lease under consideration.

Instructions to the Offeror:

Please complete Section 1 on the following page. If Offeror checks the “no relevant past performance” box, then do not proceed further in filling out the form, but do submit the form as part of the offering package. If the “no relevant past performance” box IS NOT checked, then Offeror must forward a copy to 3 references, instructing them to complete sections 2 and 3. References must be current or former tenants of the Offeror. Once the respondents complete the remainder of the form, it must be returned to the Offeror and submitted as part of the Offeror’s proposal contents. Failure to submit all past performance questionnaires will be deemed as a non responsive offer.
