About this file
This is Amendment No. 4 to Request for Lease Proposal (RLP) No. 36C24W25R0011 for a Veterans Affairs Community Based Outpatient Clinic (CBOC) in Idaho Falls, Idaho, issued August 21, 2026.
The amendment makes two substantive modifications to the RLP: (1) it adds Attachment No. 1 to GSA Form 1364 as Exhibit O to the list of RLP documents, and (2) it requires offerors to submit a completed Attachment No. 1 to GSA Form 1364 for each of three firm term scenarios—10-year, 15-year, and 20-year alternatives. The amendment includes 16 questions and answers addressing clarifications on various aspects of the lease proposal. Key clarifications include: Phase I Environmental Site Assessments are optional with initial offers but may be required by the Lease Contracting Officer at the offeror's expense; Ccure security system connection to the Salt Lake City server is required; Medeco lock systems are required due to Salt Lake Campus connection requirements; janitorial staff may satisfy requirements through IEHA membership or comparable training courses; porter services are task-based rather than time-based, with one person assumed capable of completing required tasks without supervisory staff; LEED or Green Globes certification is not required; janitorial scope is governed by the Amended Service Request (ASR) rather than the lease; blast-resistant materials requirements depend on site-specific risk analysis; stainless steel lab countertops should be solid surface instead; and solid core wood doors with plastic laminate veneer, 24-inch on center studs, and insulated glass (unless dictated by Design Basis Threat) are acceptable. Real estate tax base adjustments will be made in accordance with paragraph 2.06 of the lease. Offerors must acknowledge receipt of this amendment by initialing every page and signing the acknowledgement form with their initial offer.

AMENDMENT NO. 4
REQUEST FOR LEASE PROPOSAL NO. 36C24W25R0011
August 21, 2026
Veterans Affairs (VA)
Community Based Outpatient Clinic (CBOC) Idaho Falls, ID
The Government hereby amends the Request for Lease Proposals as follows:
1. Paragraph 1.06.A, LIST OF RLP DOCUMENTS (MAY 2026), is hereby amended to include the attached Attachment No. 1 to GSA Form 1364 as Exhibit O.
2. Paragraph 3.01.W INSTRUCTIONS FOR OFFEROR AND SUBMITTALS (FEB 2026) – MODIFIED is hereby added as follows:
W. Attachment No. 1 to GSA Form 1364. Provide a completed Attachment No. 1 to GSA Form 1364 for each of the following Firm Term scenarios:
1. Alternative A) 10-Year Firm Term;
2. Alternative B) 15-Year Firm Term; and
3. Alternative C) 20-Year Firm Term.
The Government provides the following responses to questions received regarding the Request for Lease Proposals:
Question 1: Could you please clarify (or confirm) that with RLP submission Round 1 - there is NO Phase 1 ESA report required? R100 - Section 2.10 references at the direction of the LCO and Section 3.01 B submittal list states if required.
Would you be able to confirm, it is not required.
Answer: A Phase I ESA is not a required submittal with initial offers. Under RLP Section 2.10.A.1, the LCO may direct an offeror to provide a current Phase I ESA (ASTM E1527-21, prepared with the Government as the 'user') at the offeror's sole cost and expense. Offerors proposing new construction should anticipate that direction, given that the Government cannot proceed to award until the NEPA process is complete per Section 2.10.B.6. Submitting a completed Phase I with the initial offer is optional and is not a stated evaluation factor under Section 4.03, but it may avoid a schedule delay later.
Question 2: Please clarify if Ccure or Andover Comtinuum is the preferred system used by the VA/VHA in this area.
Answer: Ccure is required because it will need to connect to the Salt Lake City main Ccure server.
Question 3: Please clarify if Medeco is specifically required for this project or if approved equals are allowed.
Answer: Medeco is required due to required connection to Salt Lake Campus. We only need data connection for validator and network connection for that validator. The validator, cores and keys will be provided by the VA.
Question 4: Please clarify that IEHA membership for the janitorial staff is required prior to award?
Answer: There are many "or" statements after this, one of which allows for a comparable training course.
Question 5: Confirm the day porter and required supervisor are required onsite for 8 hours a day.
Answer: The expectation is task based rather than time based. The contractor needs to provide the government with the monthly work schedules to ensure the tasks are able to be completed.
Question 6: Confirm that day porter services are required to be onsite 8 hours a day with this IEHA certification
Answer: Answered above.
Question 7: Please confirm that pricing shall include 2 people during operation hours for porter duties.
Answer: Supervisory staff would only be required if there were multiple housekeeping staff required to complete the tasks. In this case it would be assumed one person could complete the required tasks and a supervisor would not be required.
Question 8: Please clarify if LEED or Green Globes are required for this proposal.
Answer: LEED or Green Globes is not required.
Question 9: Please clarify whether the Lease or ASR supersedes the other regarding janitorial scope of work and frequency required by the lessor.
Answer: ASR.
Question 10: Will the agency please clarify if Blast Resistant materials are required or is the intent to simply provide
Progressive Collapse with steel construction?
Answer: This would be based on the site. To determine this the setbacks and latest Design Basis Threat would need to be established in an initial risk analysis.
Question 11: Please confirm that stainless steel countertops are required in the lab.
Answer: No these should be solid surface.
Question 12: Please confirm if a fence, wall, or gate is required around the entire perimeter of the site.
Answer: Not required.
Question 13: Can the agency confirm if insulated glass will be acceptable in the lobby?
Answer: Not required unless dictated by the DBT.
Question 14: Would the agency accept solid core wood doors with a plastic laminate veneer finish?
Answer: Yes.
Question 15: Would the agency accept 24-inch on center studs?
Answer: Yes.
Question 16: Please clarify if a real estate tax base and tax adjustments will apply to this lease.
Answer: A Real Estate Tax Base and adjustments to that Base will be made in accordance to para 2.06 of the Lease.
