About this file
This document is a Past Performance Questionnaire for a Department of Veterans Affairs (VA) Real Property Lease Solicitation (RLP # 36C24W25R0107). The questionnaire is designed to collect performance information from previous tenants about potential lessors (landlords) competing for a lease award. It includes three sections: Offeror Information, Evaluator Information, and eight detailed performance assessment questions covering areas such as tenant improvement build-out, change order pricing, schedule adherence, budget management, maintenance responsiveness, safety and security issue handling, building cleanliness, and overall lease performance. Evaluators can rate the lessor's performance on a scale from Excellent to Unsatisfactory across these categories, with space to provide additional comments on potential improvements.
The questionnaire is to be completed by the offeror and then forwarded to current or former tenants to provide detailed feedback, with responses treated as confidential and procurement sensitive. The point of contact is John Paul Niega, Lease Contracting Officer, who can be reached at johnpaul.niega@va.gov or 775.389.6616. Handwritten responses are acceptable, and offerors with no relevant past performance can simply check a box indicating such and submit the form as part of their proposal package.
