About this file
This is an Applicant Consent Form for The University of Texas System's Group Purchasing Organization (GPO) accreditation program. The form establishes the requirements for qualifying GPOs, including Texas State Agencies performing GPO functions, that seek to become approved suppliers of GPO services to UT System or its institutions. Organizations must participate in the accreditation process by completing a referenced survey and certifying the accuracy of all responses. Participating organizations agree to allow UT System or its institutions to conduct audits of their records and processes at UT System's expense and at reasonable times to validate survey responses. Additionally, organizations commit to participate in periodic reviews conducted by UT System regarding the continued accuracy of their survey information. Organizations that fail to secure accreditation or decline to participate will not be used by UT System or its institutions for GPO services.
Organizations must notify UT System in writing within thirty days of any material changes to the information provided in the survey and must discuss such changes with UT System upon request, either separately or during the annual review at UT System's option. UT System reserves the right to revoke accreditation if an organization fails to comply with minimum standards required to maintain accreditation, provided the organization has thirty days following written notice from UT System to cure the failure. The form requires the organization's authorized representative to sign and date the document, acknowledging agreement to all terms and conditions outlined in the attached accreditation program documents.
