About this file

This is a Texas Workforce Commission (TWC) Vocational Rehabilitation Services Entity's Physical Location form (VR3442B) used to document and verify the physical locations where contractors provide services to VR customers. The form serves as either an initial application or an update submission for existing contractors establishing or maintaining dedicated service delivery locations. Contractors must complete this form for each physical location where they provide goods, services, or conduct in-person customer meetings. The submission is triggered by either an Electronic State Business Daily (ESBD) posting or direct instruction from the Regional Quality Assurance Specialist (VR) or Regional Program Support Specialist (RPSS). The form requires comprehensive documentation including entity identification information, specific physical location details, occupancy certificates, building permits, and fire inspection reports from the local fire department or the Texas Department of Insurance State Fire Marshal's Inspection Services Division.

The form mandates compliance with minimum safety and accessibility standards before a location can be approved for service delivery. Safety requirements include verification of working smoke detectors with audible and visible signals, non-expired fire extinguishers in accessible locations, posted fire escape route diagrams, evacuation plans for customers requiring physical assistance, emergency medical service arrangements, accessible work zones, hazardous materials management plans, and equipment safety protocols. Accessibility standards require handicapped parking, accessible exterior routes, accessible interior spaces, and accessible bathrooms, with alternative accommodations permitted when these standards cannot be met. The legally authorized representative must certify that all information is accurate, the location meets ADA and safety standards, customer accessibility needs will be addressed, and any safety incidents will be reported to TWC-VR via the VR3446 Incident Report form. Contractors are required to update this form whenever information changes and maintain ongoing compliance with TWC VR Standards for Provider Manual requirements.
