About this file
This is a Child Support Certification form issued by the Texas Workforce Commission Vocational Rehabilitation Services under Texas Family Code Section 231.006. The form is required for all bids and applications for state contracts, grants, and loans to ensure compliance with child support obligations. The document establishes eligibility requirements and mandates that vendors and applicants provide the names and Social Security numbers of all individuals with at least 25 percent ownership interest in the business entity or sole proprietors submitting bids or applications. The form requires submission of this information prior to contract award, with no specific response date outlined in the document itself.
The certification prohibits payment of state funds to any person or business entity where an individual is more than 30 days delinquent in child support payments. Ineligibility continues until all arrearages are paid, compliance is established with a written repayment agreement or court order, or a court of continuing jurisdiction grants an exemption as part of a court-supervised effort to improve earnings and child support payments. The Texas Workforce Commission is authorized to terminate contracts if the certification is determined to be inaccurate or false, and vendors become liable to the state for attorney's fees, costs to complete the contract, and any other damages provided by law or contract. The form must be signed by a contractor's authorized representative and dated to be valid.
