About this file

This is an Execution of Bid form for Invitation for Bid (IFB) No. 3202600146 issued by the Texas Workforce Commission (TWC) for Rehabilitation Supplies for Vocational Rehabilitation Customers. The form serves as the binding document through which bidders acknowledge receipt of all TWC Terms and Conditions, agree to provide the items and services described in the IFB, and commit to abiding by all specified terms. Bidders must complete required information sections including bidder name, mailing address, billing address, contact information for both bid and contract purposes, Employer Identification Number (EIN), and Texas Identification Number (TIN). The form requires an authorized representative's signature and date, with the signatory affirming the bidder's obligations regarding subcontractors as outlined in IFB Form 3 (VetHUB Subcontracting Plan).

The document establishes critical notification and compliance requirements for the successful bidder, including mandatory disclosure of any substantial changes in financial condition within ten days of occurrence, changes in ownership or control within thirty days prior to occurrence, and any pending litigation, investigations, or adverse actions that may materially impact contract performance within thirty days of initiation. Bidders may claim preferences under various Texas statutes, including those for goods produced or services offered by Texas bidders, service-disabled veteran-owned businesses, agricultural products grown in Texas, products from economically depressed areas, recycled or environmentally sensitive materials, energy-efficient products, and foods of higher nutritional value. All companies and individuals must obtain an EIN prior to contract award, and bidders are directed to obtain a TIN through the Texas Comptroller of Public Accounts for payment processing purposes.
