About this file

This price form is associated with Texas solicitation number 582-27-00030 for Laboratory Analysis Services. The solicitation requests laboratory analysis services across four primary groups: Group A (Drinking Water), Group C (Wastewater Liquid), Sediment, and Group D (Solid Waste/RCRA). Respondents must provide fully-loaded, firm, fixed prices for each analyte or test method listed, with pricing structured across five turnaround time categories: routine (up to 20 days), priority (up to 10 days), emergency (up to 5 days), emergency (48-hour), and emergency (24-hour). The price form requires respondents to identify the specific analytical method they will use for each test and designate any subcontractors on their HUB Subcontracting Plan (HSP) Form for analytes not performed directly. Failure to comply with completion instructions, including providing prices for all analytes, indicating scientific impossibility where applicable, or stating "NO CHARGE" for items with no associated costs, will result in proposal disqualification without evaluation.

The pricing structure accommodates multiple service levels with varying unit quantities reflecting different turnaround requirements, allowing flexibility in laboratory capacity and resource allocation. Respondents must provide individual pricing for each analyte within grouped tests, with group pricing rows included for metals, pesticides, PCBs, PAHs, and other compound categories in the wastewater, sediment, and solid waste sections. The solicitation emphasizes comprehensive analytical capability, requiring laboratories to address contaminants ranging from basic parameters such as pH and turbidity to complex analyses including PFAS, dioxins, volatile and semivolatile organic compounds, and RCRA-regulated metals. All costs must be all-inclusive, encompassing materials, equipment, labor, preparation, extraction, cleanup, and disposal services necessary to complete the requested analyses within specified timeframes.

Price Form

SOLICITATION NUMBER: 582-27-00030

SOLICITATION NAME: LABORATORY ANALYSIS SERVICES

PRICE FORM

Name of Laboratory:
Propose your fully-loaded, firm, fixed prices in the spaces provided. Prices shall include all costs of performance,

including materials, equipment, labor, preparation, extraction, clean-up, disposal, etc. to successfully complete all services.

YOU MUST PROVIDE PRICES FOR EACH ANALYTE IN THIS LIST.

IF AN ANALYTE IS SCIENTIFICALLY IMPOSSIBLE TO PERFORM WITHIN THE SPECIFIED TURNAROUND TIMEFRAME.

IF A METHOD IS SCIENTIFICALLY IMPOSSIBLE TO PERFORM WITHIN THE TURNAROUND TIMEFRAME, THIS SHOULD BE INDICATED BY PUTTING AN ASTERISK (*) AND A BRIEF EXPLANATION IN THE CELL.

If there are no charges associated with a particular method, then YOU MUST STATE: “NO CHARGE” for such item.

Analytical method to be used must be listed in "Method Responder will use" Column if multiple methods are listed in the column titled "Method".

A Subcontractor must be listed for Analytes not performed directly by Responder and must be on the Responder's HUB Subcontracting Plan (HSP) Form.

FAILURE TO COMPLY WITH THESE INTRUCTIONS WILL CAUSE THE PROPOSAL TO BE DISQUALIFIED AND WILL NOT BE EVALUATED.
