About this file

This is Form 1 - Execution of Bid for Invitation for Bid (IFB) 3202600116 issued by the Texas Workforce Commission (TWC) for Caller Name Optimization Services. The form requires bidders to complete comprehensive bidder information including legal name, mailing address, billing address, phone number, fax number, email address, Employer Identification Number (EIN), and Texas Identification Number (TIN). Bidders must designate points of contact for both proposal submission and contract administration. The form obligates bidders to acknowledge receipt of all TWC Terms and Conditions, accept the items and services as described in the IFB, and comply with all specified terms. Any exceptions to the stated terms and conditions must be identified in detail at the time of bid submission, or they will not be considered for evaluation.

The document establishes mandatory requirements for successful bidders, including notification of changes in financial condition within ten days of occurrence, notification of substantial ownership or control changes within thirty days prior to occurrence, and provision of thirty days' notice regarding any pending litigation, investigations, or adverse actions that could materially impact contract performance. Bidders must disclose any financial interests in proposed subcontractors and affirm compliance with subcontractor requirements outlined in IFB Section 3.18. The form includes options for bidders to claim various state preferences related to Texas residency, service-disabled veteran ownership, agricultural products, environmental materials, and other designated categories. Every company or individual must obtain an EIN prior to contract award to minimize identity theft.
