About this file
This document is the Respondent's Disclosure of Interested Parties Form (Exhibit E) for Invitation for Bid (IFB) No. 608-26-91504 for a Digital Billboard Advertising Campaign for the Texas Department of Motor Vehicles (TxDMV). The form is designed to disclose potential conflicts of interest, ownership structures, subcontractors, and other pertinent information about the bidding entity. The document requires respondents to provide detailed information about partners, major stockholders, consultants, and interested parties, with specific guidelines for defining controlling interests and intermediaries.
The form emphasizes TxDMV's commitment to transparency and avoiding conflicts of interest, requiring respondents to disclose any gifts, donations, or meetings with agency officials in the past two calendar years. Sections of the form mandate listing ownership percentages, paid consultants over $25,000, subcontractors and suppliers, and any interactions that could potentially influence official conduct. The document serves as a critical compliance tool for the procurement process, with potential consequences of disqualification or contract termination for incomplete or misleading disclosures.
