About this file

This document is a Solid Waste Reporting Form issued by the Texas Military Department (TMD) and Texas Army National Guard, dated 13 November 2018. The form is designed as an attachment (Attachment G.5) to be used for reporting solid waste disposal and diversion during a specific project's reporting period. Contractors are required to complete two key reporting sections: 1) the total amount of solid waste disposed of during the reporting period, measured in tons and provided by the receiving vendor contracted by the General Contractor (GC), and 2) the total amount of solid waste diverted from landfills during the same reporting period, also measured in tons and with precision as specified by the receiving vendor.

The form specifies that these solid waste reporting details are essential for pay applications to be considered complete, with definitions of terms like "solid waste", "disposed of", and "diverted from" referenced in Army Regulations 420-1 and 200-1. The document provides space for the contractor to record project details, reporting period, owner's contract number, and requires the contractor's signature, firm name, typed name, and date of submission. This reporting mechanism appears to be part of the Texas Military Department's environmental tracking and waste management documentation process for military construction or maintenance projects.
