This document is Form A - Offeror's/Proposer's Execution of Offer/Proposal and Assurances (with Certification) issued by the Office of the Attorney General (OAG) of Texas. It details the requirements and assurances that must be provided by offerors responding to the OAG's Request for Offer (RFO) for Statewide Automated Victim Notification Services (SAVNS). The RFO is seeking services for the development, implementation, and execution of a statewide automated victim notification system. Responses to the RFO are due by July 18, 2024, at 3 pm Central Time. Roxanne D. Koltermann of the OAG is the sole point of contact for inquiries about this RFO.
The document outlines various certifications and assurances the offeror must provide, including compliance with antitrust laws, child support payments, lobbying prohibitions, suspension and debarment status, and other state and federal requirements. It also requires the offeror to disclose any potential conflicts of interest and provide information on the company's ownership structure, including the names and social security numbers of individuals with at least 25% ownership. The contract resulting from this RFO may be subject to specific pricing, set-aside, and funding requirements, but these details are not included in the provided documentation.
